Intercostal cryoanalgesia for acute pain after video-assisted thoracic surgery lung resection: A randomized controlled preliminary trial.

OBJECTIVE: Video-assisted thoracoscopic surgery (VATS) is associated with significant postoperative pain. Multimodal analgesia, including single-shot paravertebral blocks, is widely used but provides limited analgesic duration. Intercostal cryoanalgesia, which offers prolonged pain relief, presents a promising adjunctive option. This study aimed to assess the analgesic benefit of intercostal cryoanalgesia in VATS lung cancer surgery. The primary outcome was thoracic pain during cough 24 hours postsurgery, measured via a verbal numerical rating scale. METHODS: In a randomized, double-blind, co

Confronting Upside-Down Video-Assisted Thoracic Surgery Approach for Hemorrhagic Bronchogenic Cyst Manifested by Sudden Back Pain.

INTRODUCTION: Bronchogenic cysts, arising from an aberrant bronchial primordium inclusion during the fetal period, are typically located in the mediastinum but can develop in ectopic regions. While generally asymptomatic, these cysts may become symptomatic due to infection or, rarely, hemorrhage. This report details a case of a hemorrhagic bronchogenic cyst in the supradiaphragmatic region, successfully resected using video-assisted thoracic surgery (VATS) with a confronting upside-down monitor setting. CASE PRESENTATION: An 18-year-old female presented with a fever and sudden left-sided back

Risk factors for postoperative nausea and vomiting after video-assisted thoracic surgery esophagectomy: a prospective cohort study.

Video-assisted thoracic surgery esophagectomy (VATS-E) may increase the risk of postoperative nausea and vomiting (PONV) because it uses a high dosage of anesthesia through a long operative duration. However, no study has examined the risk factors for PONV after VATS-E. Therefore, we investigated the risk factors for PONV to support the appropriate risk management of PONV after VATS-E. This prospective cohort study included 155 patients who underwent VATS-E at the Showa University Hospital between April 1st, 2020 and November 30th, 2022. The primary outcome was the incidence of PONV within 24

Effects of Preserving the Pulmonary Vagus Nerve Branches on Cough After Pneumonectomy During Video-Assisted Thoracic Surgery.

BACKGROUND: Cough is one of the most common complications of early-stage non-small cell lung cancer (NSCLC) after video-assisted thoracoscopic surgery (VATS). The vagus nerve plays an important role in pulmonary inflammation and the cough reflex. In this study, we attempted to reduce the incidence of postoperative chronic cough and other complications by preserving the pulmonary vagus nerve branches. PATIENTS AND METHODS: This study was a randomized controlled double-blinded trial of subjects and observers. A total of 158 NSCLC patients were enrolled. We randomly assigned 79 patients to Group

Post-traumatic chilothorax treated with thoracic duct closure guided by an indocyanine green-sensitive camera system during uniportal video-assisted thoracic surgery.

Chylothorax is caused by lymphatic chyle fluid leaking through the thoracic duct and accumulating in the pleural cavity. It is related to a thoracic duct injury or occlusion. Our 26-year-old patient sustained a traumatic injury to the thoracic duct.  We used a novel approach to treat this patient, that is, we identified the leaking chyle by combining uniportal video-assisted thoracic surgery with the injection of indocyanine green bilaterally in the inguinal lymph nodes.

Uniportal approach as an alternative to the three-portal approach to video-assisted thoracic surgery for primary spontaneous pneumothorax.

BACKGROUND: In recent years, uniportal video-assisted thoracic surgery (U-VATS) has been used for primary spontaneous pneumothorax (PSP). This study compared the perioperative outcomes of U-VATS and three-port VATS (3P-VATS) and sought to determine the risk factors for postoperative recurrence. METHODS: From October 2010 to February 2017, 232 patients with PSP undergoing surgical treatment were enrolled in this study. The patients were divided into two groups: U-VATS (n=161) and 3P-VATS (n=71) depending on the period of surgery. Retrospective analysis of the perioperative results and the risk

Anesthetic considerations for video-assisted thoracoscopic surgery in a child with Glenn shunt for thoracic duct ligation and pleurodesis.

Glenn shunt is an anastomosis between superior vena cava and right pulmonary artery to palliate patients with single ventricle physiology of the heart. Chylothorax is a frequent and troublesome complication after the creation of this shunt, which if not controlled with medical management, might require pleurodesis, and thoracic duct ligation. Video-assisted thoracoscopic surgery (VATS) causes less postoperative pain, earlier mobilization, lower overall morbidity, a shortened hospital stay with reduced cost, and a cosmetic incision. A comprehensive understanding of physiology of Glenn shunt and

Comparison of intrathecal morphine versus thoracic paravertebral block for video-assisted thoracoscopic surgery: a randomized non-inferiority trial.

BACKGROUND: Although widely performed, video-assisted thoracoscopic surgery (VATS) is associated with significant postoperative pain. Thoracic paravertebral block (TPVB) is a guideline-favored regional technique, while intrathecal morphine (ITM) is a technically less demanding and more widely applicable neuraxial alternative. We hypothesized that ITM would provide analgesic efficacy non-inferior to TPVB in terms of 24-hour intravenous morphine milligram equivalents (IV-MME). METHODS: This was a prospective, randomized, controlled, observer-blinded non-inferiority trial. Adults undergoing elec

Efficiency of tapering programmed intermittent epidural analgesia in video-assisted thoracic surgery: a double-blind, prospective, randomized study.

UNLABELLED: OBJECTIVE: Programmed intermittent epidural bolus (PIEB) is an effective analgesic method owing to the redistribution of the drug solution, which leads to a gradual improvement in postoperative pain over time. This study aimed to compare the postoperative analgesic effects of patient-controlled epidural analgesia (PCEA) with those of tapering PIEB (t-PIEB), in which the drug dosage is decreased over time, and continuous epidural infusion (CEI). MATERIALS AND METHODS: Patients undergoing video-assisted thoracoscopic surgery (VATS) with general and epidural anesthesia were randomized

Effects of Stroke Volume Maximization Before One-Lung Ventilation on Video-Assisted Thoracic Surgery: A Randomized Controlled Trial.

Background/Objectives: The use of goal-directed fluid therapy (GDFT) guided by stroke volume (SV) variation during thoracic surgery, particularly with one-lung ventilation (OLV) and protective ventilation strategies, is not well established. This study aimed to determine whether maximizing stroke volume (SV) before initiating one-lung ventilation (OLV) reduces the incidence of intraoperative hypotension requiring vasoactive agents during video-assisted thoracoscopic surgery (VATS). Methods: Sixty patients undergoing VATS were randomly assigned to an SVM group (n = 30) or a control group (n = 3

Successful Anesthetic Management of Video-Assisted Thoracic Surgery in a Patient With Cardiac Complications After Arterial Switch Operation.

The arterial switch operation with the Lecompte maneuver has become the therapy of choice for dextro-transposition of the great arteries, contributing to the improved survival rate of patients. Nevertheless, there have been few case reports addressing anesthetic management using one-lung ventilation in patients with an arterial switch. A 24-year-old man who underwent an arterial switch operation at 10 days of age was admitted for a right spontaneous pneumothorax. Preoperative transthoracic echocardiography revealed significant neo-aortic regurgitation, tricuspid regurgitation, and estimated ri

Clinical evaluation of laryngeal mask airways in video-assisted thoracic surgery: a meta-analysis of randomized controlled trials.

BACKGROUND: Endotracheal intubation is often associated with postoperative complications such as sore throat discomfort and hoarseness, reducing patient satisfaction and prolonging hospital stays. Laryngeal mask airway (LMA) plays a critical role in reducing airway complications related to endotracheal intubation. This meta-analysis was performed to determine the efficacy and safety of LMA in video-assisted thoracic surgery (VATS). METHODS: The PubMed, Embase, Cochrane Library, Medline and Web of Science databases were searched for eligible studies from inception until October 5, 2023. Cochra

Efficacy of intraoperative thoracoscopic intercostal nerve blocks in nonintubated and intubated video-assisted thoracic surgery: A randomized study.

BACKGROUND: The efficacy of thoracoscopic intercostal nerve blocks (TINBs) for noxious stimulation from video-assisted thoracic surgery (VATS) remains unclear. The efficacy of TINBs may also be different between nonintubated VATS (NIVATS) and intubated VATS (IVATS). We aim to compare the efficacy of TINBs on analgesia and sedation for NIVATS and IVATs intraoperatively. METHODS: Sixty patients randomized to the NIVATS or IVATS group (30 each) received target-controlled propofol and remifentanil infusions, with bispectral index (BIS) maintained at 40-60, and multilevel (T3-T8) TINBs before surg

Uniportal video-assisted thoracic surgery approach for simultaneous lung cancer and thymic carcinoma: Case report and literature review.

Thymic carcinoma is an epithelial tumor derived from thymic epithelial cells. Thymic tumors may be associated with other simultaneous and/or metachronous extra-thymic tumors (e.g., lung cancer). Here, we report a case of simultaneous surgical management of lung and mediastinal neoplasm together with a review of the literature. During radiological follow-up for prostate and colorectal cancer, an 82-year-old man was diagnosed with lung cancer with simultaneous mediastinal suspected neoplasm. Both were surgically removed with a single intervention performed via a uniportal video-assisted thoracic
