The spine journal : official journal of the North American Spine Society2013eng
BACKGROUND CONTEXT: The evidence surrounding the topic of adjacent segment degeneration and disease has increased dramatically with an abundant amount of literature discussing the incidence of and techniques to avoid it. However, this evidence is often confusing to discern because of various definitions of both adjacent segment degeneration and disease.
PURPOSE: To organize and review the recent evidence for adjacent segment degeneration and disease.
RESULTS: Although multifactorial, three distinct causes of adjacent segment disease in both the lumbar and cervical spine have been discussed:

Total disc arthroplasty (TDA) has emerged as a viable surgical option for intractable/disabling lumbar discogenic pain in suitable patients. The surgical aims of TDA are to maintain the natural biomechanics of the intervertebral segment, decrease symptoms, preserve motion and reduce further degeneration at adjacent segments. Approach-related complications associated with anterior retroperitoneal surgical access include sexual dysfunction (retrograde ejaculation), vascular injury (arterial and venous injuries, thrombosis), abdominal wall hematoma, and infections. Besides approach related compli

STUDY DESIGN: This is a systematic review and meta-analysis.
OBJECTIVE: The aim of this study was to evaluate the efficacy and safety of multiple-level cervical disk replacement (CDR) over single-level CDR for the treatment of cervical spondylosis.
SUMMARY OF BACKGROUND DATA: Some authors advocate for the multiple-level CDR instead of anterior decompression and fusion in cervical multiple-level spondylosis. However, whether the efficacy and safety of multi-level CDR are as favorable as that of single-level CDR remains controversial.
METHODS: MEDLINE, EMBASE, and Cochrane library databases w

Degenerative disc disease is a significant cause of chronic low back pain, often leading to disability and high health care costs. Current treatments, including physical therapy, pain management, and surgical interventions such as spinal fusion and total disc replacement, do not reverse degeneration. Bioactive therapies offer a potential alternative by targeting the underlying degenerative process. Cell-based therapies, including the use of mesenchymal stem cells and platelet-rich plasma, aim to restore disc structure and function by promoting extracellular matrix production and reducing infla

CASE: Eighteen months after implantation of a C6-C7 cervical artificial disc (ProDisc-C; DePuy Synthes), a 44-year-old woman presented with right-sided C7 radiculopathy. Imaging revealed posterior subsidence of the prosthesis, with C7 foraminal stenosis. An ultrasonic curet was used to safely and effectively extract the prosthesis without neurologic complication; safe access for the insertion of an intervertebral cage was facilitated.
CONCLUSION: The ultrasonic curet was a safe and effective adjunct for revision artificial cervical disc replacement surgery; this technique may help to reduce p

A primary cause of chronic low back pain and disability is lumbar degenerative disc disease (DDD). Surgical treatments are available for DDD including fusion or lumbar total disc replacement (TDR). Lumbar TDR is performed as a motion-preserving alternative to lumbar fusion. There are several marketed lumbar TDR devices including the Aesculap Implant Systems activL Artificial Disc, Charité Artificial Disc and the ProDisc-L Total Disc Replacement. Areas covered: Considering the various surgical options by which lumbar DDD can be managed, here we evaluate the biologic and biomechanical benefits a

BACKGROUND: Cervical disc replacement (CDR) has emerged as an alternative to anterior cervical discectomy and fusion for the surgical treatment of degenerative cervical disc disease. Although comparison of the 2 techniques has been studied in the literature, a thorough assessment of all artificial discs between each has not been performed. The objective of the present study was to examine the long-term outcomes of 5 artificial discs.
METHODS: An electronic literature search was conducted for studies of CDR devices for all years available. Only articles in English were included. Heterotopic os

OBJECTIVE: The purpose of this study is to compare the effectiveness and safety of cervical disc arthroplasty with anterior cervical discectomy and fusion for treatment of symptomatic cervical disc disease. Anterior cervical discectomy and fusion (ACDF) is the conventional surgical treatment for symptomatic cervical disc disease. Recently, cervical disc arthroplasty (CDA) has been developed to address some of the shortcomings associated with ACDF by preserving function of the motion segment. Controversy still surrounds regarding whether CDA is better.
METHODS: We systematically searched six e

Wear debris-induced osteolysis is a common cause of arthroplasty failure in several joints including the knee, hip and intervertebral disc. Debris from the prosthesis can trigger an inflammatory response that leads to aseptic loosening and prosthesis failure. In the spine, periprosthetic pain also occurs following accumulation of wear debris through neovascularization of the disc. The role of the immune system in the pathobiology of periprosthetic osteolysis of joint replacements is debatable. Areas covered: We discussed the stimulation of pro-inflammatory and pro-protective and pro-regenerati

The modern surgical treatment of cervical degenerative disc disease can be traced back to the advent of anterior cervical decompression and fusion. With the emergence of fusion-related complications, different scholars have promoted the gradual transformation of cervical degenerative disc diseases from "fusion fixation" to "non-fusion reconstruction" through in-depth fusion with materials science, engineering mechanics and other disciplines. The innovation of this treatment concept is consistent with the original intention of "structural remodeling, functional reconstruction, maximum repair and reco

BACKGROUND: Cervical disk arthroplasty is now a widely accepted alternative to anterior cervical interbody fusion, which is known to reduce normal cervical motion and increase the incidence of adjacent segment disease. Although multiple studies report the use of cervical disk arthroplasty to treat multilevel cervical disease, this is the first report in the literature detailing the placement of multiple, noncontiguous artificial disks.
CASE DESCRIPTION: We describe a 41-year-old male who presented with myelopathy and left upper extremity radiculopathy resulting from 2 cervical disk herniation

Artificial cervical disc replacement surgery has become an effective and main treatment method for cervical disease, which has become a more common and serious problem for people with sedentary work. To improve cervical disc replacement surgery significantly, a 6-DOF parallel bone-grinding robot is developed for cervical bone-grinding by image navigation and surgical plan. The bone-grinding robot including mechanical design and low level control is designed. The bone-grinding robot navigation is realized by optical positioning with spatial registration coordinate system defined. And a parametr
