Unperceived bronchial bleeding complications during percutaneous dilatational tracheotomy: a case report and 3D simulation.

Percutaneous dilatational tracheostomy is an established technique for securing the airway in critically ill patients. One of the most common complications is bleeding around the incision or after injury to major vessels in anatomic proximity. We report a case in which a thrombocytopenic patient experienced life-threatening bleeding during the procedure at the bifurcation between segmental bronchus 9 and 10, apparently caused by an unrecognized guide wire-induced mucosal lesion. Immediate extensive bronchoscopy and hemostatic interventions were required to ensure oxygenation. To better illustra

High-located aberrant innominate artery: an unusual cause of serious hemorrhage of percutaneous tracheotomy.

An aberrant innominate artery located high was observed during a cadaveric neck dissection. To our knowledge, an innominate artery crossing the 4th and 5th tracheal rings has rarely been reported in available literature. Knowledge of this anomaly is important for those who are involved with percutaneous procedures in the neck to avoid major complications.

A novel olecranon-type tracheotomy and expansion forceps for single-person emergency airway access.

BACKGROUND: We developed olecranon-type tracheotomy and expansion forceps (OTEF) for emergency tracheotomy in patients experiencing acute airway obstruction. By using OTEF, medical rescue personnel can perform emergency tracheotomy more quickly and accurately on their own, while conventional tracheotomy requires the cooperation of two surgeons. METHODS: In this study, 24 adult cadavers that had died within 24 h were randomly assigned to the OTEF or PDT groups. Tracheotomies were performed by the same physician, using the OTEF technique for the OTEF group and the percutaneous dilational trache

Dysphagia testing and aspiration status in medically stable infants requiring mechanical ventilation via tracheotomy.

OBJECTIVE: To perform objective testing to determine aspiration status with the goal of initiating safe and timely oral alimentation in medically stable infants who require mechanical ventilation via tracheotomy. Medically compromised infants who require mechanical ventilation via tracheotomy and are nil by mouth are conventionally deemed as being at risk for aspiration and feeding difficulties. There is little information available in the literature regarding diagnostic testing and habilitation intervention to promote safe and timely initiation of oral alimentation when these infants are medi

A fatal case of recurrent amiodarone-induced thyrotoxicosis after percutaneous tracheotomy: a case report.

BACKGROUND: Amiodarone is a widely used antiarrythmic drug, which may produce secondary effects on the thyroid. In 14-18% of amiodarone-treated patients, there is overt thyroid dysfunction, usually in the form of amiodarone-induced thyrotoxicosis, which can be difficult to manage with standard medical treatment. CASE PRESENTATION: Presented is the case of a 65-year-old man, under chronic treatment of atrial fibrillation with amiodarone, who was admitted to the Intensive Care Unit with acute cardio-respiratory failure and fever. He was recently hospitalized with respiratory distress, attribute

[When should tracheotomy be performed in bilateral vocal cord paralysis involving multiple system atrophy?].

OBJECTIVES: Bilateral vocal cord paralysis caused by central nervous system dysfunction results from such diverse causes as cerebrovascular disorder and neurodegenerative disease. Otolaryngologists are often consulted about indications of tracheostomy for such cases, but if their recognition of causative disease is insufficient, it is difficult to judge indications of tracheostomy. We reviewed tracheostomy cases due to bilateral vocal cord paralysis caused by multiple system atrophy (MSA) and considered points to keep in mind in such cases. MATERIALS AND METHODS: We diagnosed 9 cases of vocal

Preoperative tracheotomy in the treatment of upper airway obstruction of patients with advanced stage supraglottic carcinoma.

BACKGROUND: Preoperative tracheotomy is an effective option that secures upper airway patency in laryngeal carcinoma patients suffering from upper airway obstruction, but the influence of this treatment on oncologic outcomes of laryngeal carcinoma remains controversial. The purpose of this study was to determine the impact of preoperative tracheotomy on overall survival in supraglottic carcinoma patients with tumor obstruction of the upper airway, and explore the potential causes. MATERIALS AND METHODS: This retrospective study collected 243 consecutive patients with advanced stage supraglott

[Analysis of 2 cases of dyspnea happening after tracheotomy and the clinical application of Mimics 10.01].

Post-intubation tracheal stenosis was a late time complication after tracheotomy but the happening of dyspnea was unusual. Diagnosing tracheal stenosis after incubation, and figuring out the location and causes of the stenosis were important. Treatment of post-incubation tracheal stenosis relied on accurate diagnosis of the type of tracheal stenosis. Computed tomography (CT) and laryngoscope could be used for detecting the stenosis but not enough. Two patients who were already under the urgent tracheotomy over 1 year were reported. However apnea was found on these two patients for a long time

[Anesthesia in a Patient with Giant Goiter in Whom Tracheotomy Was Performed with Standby of Percutaneous Cardiopulmonary Support].

In a 53-year-old female patient total thyroidectomy for a giant goiter under general anesthesia was scheduled. On talking, airway stenosis sounds were heard. Cervical to thoracic CT revealed left and right lobe tumors measuring 5.3 x 5.6 x 10.0 and 9.1 x 8.6 x 10.0 cm, respectively. The trachea showed stenosis at a site 3.8 to 6.5 cm below the glottis, and the narrowest lumen diameter was 3.1 mm. Due to marked tracheal stenosis, awake intubation was not selected. To maintain the airway, tracheotomy was performed under local anesthesia. Considering the risk of difficulty in ventilation during t

Association of Airway Complications With Free Tissue Transfer to the Upper Aerodigestive Tract With or Without Tracheotomy.

IMPORTANCE: Airway management during microvascular reconstruction of the upper aerodigestive tract is of utmost importance; however, there is considerable debate about optimal management of the airway. OBJECTIVE: To examine if free tissue transfer to the upper aerodigestive tract without tracheotomy was associated with an increased rate of airway complications or death. DESIGN, SETTING, AND PARTICIPANTS: Cohort study of 861 patients undergoing microvascular reconstruction to sites in the oral cavity, oropharynx (excluding the base of tongue), and nasal and/or sinus cavity using data from the

Analysis of Risk Factors for Pneumonia in 482 Patients Undergoing Oral Cancer Surgery With Tracheotomy.

PURPOSE: Elective tracheostomy is a common procedure used in patients with oral cancer. However, secondary tracheotomy-associated pneumonia (TAP) is an important complication after surgery. This study investigated the risk factors related to postoperative TAP complications in patients with oral cancer. MATERIALS AND METHODS: A retrospective study was performed from January 2012 to October 2013. Data on patients who had oral cancer and underwent neck dissection or reconstructive surgery with tracheotomy were collected from the Hospital Information System. The predictive variables were age, gen

Tracheotomy and children with spinal muscular atrophy type 1: ethical considerations in the French context.

Spinal muscular atrophy (SMA) type 1 is a genetic neuromuscular disease in children that leads to degeneration of spinal cord motor neurons. This sometimes results in severe muscular paralysis requiring mechanical ventilation to sustain the child's life. The onset of SMA type 1, the most severe form of the disease, is during the first year of life. These children become severely paralysed, but retain their intellectual capacity. Ethical concerns arise when mechanical ventilation becomes necessary for survival. When professionals assess the resulting life for the child and family, they sometime

[Lateral fixation of the vocal cord instead of tracheotomy in acute bilateral vocal cord paralysis].

UNLABELLED: BACKGROUND AND STATEMENT OF THE PROBLEM: The dyspnea resulting from an acute bilateral paralysis of the recurrent nerve often requires tracheotomy. Such a situation occurs most frequently after thyroid resection. It was analysed how the endo-extralaryngeal laterofixation of one vocal cord can contribute to avoid tracheotomy. PATIENTS AND METHODS: 40 patients (37 women, three men; mean age 42.9 years, range: 20-78 years, standard deviation: 9.2 years) with bilateral paralysis of the recurrent nerve after thyroid resection were treated by performing the technique mentioned below. Longitudinal assessment of hypercapnic ventilatory drive after tracheotomy in a patient with the Prader-Willi syndrome.

The clinical course and changes in hypercapnic ventilatory drive over time were serially assessed before and after tracheostomy placement in a 14 year old, morbidly obese female patient with Prader-Willi syndrome, severe obstructive sleep apnoea, and obesity-hypoventilation syndrome. A tracheostomy became necessary after supplemental oxygen and continuous positive airway pressure (CPAP) had failed to improve the severity of nocturnal hypoventilation. Continued improvement in the slope to rebreathing hyperoxic hypercapnia occurred from 2-10 weeks after tracheotomy in conjunction with night-time
