Evaluation of Secondary Overtriage at a Rural Level 1 Trauma Center.

OBJECTIVE: To retrospectively assess the prevalence of secondary overtriage (SO) within a rural regional Appalachian health care system. METHODS: Trauma registry data was extracted for all trauma activation transfer patients from 2017 to 2022. Transferred patients were then stratified into two groups, non-secondary overtriage (non-SO) or SO. Patients were considered SO if they met three criteria following transfer: an Injury Severity Score (ISS) of less than 15, no required operative intervention, and discharge within 48 hours of arrival. Descriptive statistics were compared for age, length o

Successful conviction of intoxicated drivers at a level I trauma center.

INTRODUCTION: Conviction rates for drivers driving under the influence (DUI) and in motor vehicle collisions (MVC) presenting to trauma centers are based primarily on data from the 1990s. Our goal was to identify DUI conviction rates of intoxicated drivers in MVCs presenting to a trauma center and to identify factors associated with the failure to obtain a DUI conviction. METHODS: Retrospective study of adults (>18 years) presenting to a trauma center emergency department (ED) in 2007. Eligible subjects were drivers involved in a MVC with an ED blood alcohol level (BAL) ≥ 80mg/dL. Subjects we

Self-inflicted penetrating injuries at a Level I Trauma Center.

INTRODUCTION: Although gunshot and stab wounds are a common cause of self-inflicted injury, very little is understood about this mechanism of injury. The aim of this study was to characterise the epidemiology and outcomes of patients who injured themselves with a gun or sharp object. METHODS: After IRB approval, the LAC+USC Trauma Registry was utilised to identify all patients who sustained a self-inflicted injury caused by firearm (GSW) or stabbing (SW) from 1997 to 2007. Demographic data, injury characteristics, surgical interventions, and outcomes were abstracted and analysed. RESULTS: Du

Driving under the influence--a level-I trauma center's experience.

Alcohol-impaired driving is a major issue confronting today's society. New legislation is emerging to help curtail this ongoing problem. To evaluate the legislative effects in terms of outcome pertaining to injured drivers, we analyzed the records of 116 consecutive motor vehicle drivers who were admitted to our trauma center over a 16-month period. Medical reports, police reports, and drivers' abstracts were reviewed. Of the 116 drives, 61 (53%) had blood alcohol concentrations that exceeded the legal limit (blood alcohol level greater than or equal to 100 mg/dL) on arrival at the emergency d

Gunshot wounds to the extremities. Experience of a level I trauma center.

Gunshot wounds to the extremities are more common today, largely because civilian handgun injuries have increased. Such injuries, which are usually caused by low-velocity missiles, should be treated differently from those caused by high-velocity (military) missiles. Bone involvement, including fractures and joint injuries, necessitates orthopaedic management. At the University of Medicine and Dentistry of New Jersey-New Jersey Medical School's (UMDNJ-NJMS) University Hospital, a level I trauma center, 44 gunshot wounds (32 lower-extremity and 12 upper-extremity) with orthopaedic complications

Vascular injuries in the urban battleground: experience at a metropolitan trauma center.

The increasing frequency and severity of urban violence and vehicular injuries have brought with them a rise in the number of complex vascular injuries. To examine the cause, incidence, management, and outcome of this problem, we created a vascular trauma registry which includes all such cases treated at a Level I metropolitan trauma center over the past nine years. This constitutes a summary report of that registry. During the period 1979-1988, 411 patients (355 men, 56 women) with 478 vascular injuries were treated. There were 18 deaths (4%). Primary diagnosis was grouped by anatomic region:

Structures to improve the implementation of mental health practices in adult trauma centers: A Canadian nationwide practice survey.

BACKGROUND: Up to one third of trauma patients experience mental disorders, placing them at greater risk of morbidity and healthcare utilization. However, few studies have examined knowledge-to-practice gaps when it comes to improving their mental health. This study aims to assess how trauma program leaders perceive the implementation of structures to facilitate the adoption of recommendations aimed at improving mental health outcomes in trauma patients. METHODS: We conducted a cross-sectional practice survey across Levels I to III Canadian trauma centers (n = 69). The survey focused on empir

Adoption of brain injury guidelines in mild traumatic brain injury: challenges at both referring and receiving trauma centers.

INTRODUCTION: Emerging data suggest Brain Injury Guidelines (BIG) can be used to manage patients with mild to moderate traumatic brain injury (TBI) without neurosurgical consultation. However, many Level 3 trauma centers without neurosurgical specialists continue transferring these patients to higher-level facilities. This study aimed to evaluate the transfer outcomes of TBIs and contribute to the discussion on the feasibility of observing mild TBI patients and evaluate barriers to BIG adoption at both smaller and larger hospitals. METHODS: A retrospective chart review was performed on patien

Effect of the Uniform Accident and Sickness Policy Provision Law on alcohol screening and intervention in trauma centers.

BACKGROUND: Alcohol screening and intervention in trauma centers are widely recommended. The Uniform Accident and Sickness Policy Provision Law (UPPL) exists in most states, and allows insurers to refuse payment for treatment of injuries in patients with a positive alcohol or drug test. This article analyzed the UPPL's impact on screening and reimbursement, measured the knowledge of legislators about substance use problems in trauma centers, and determined their opinions about substance use-related exclusions in insurance contracts for trauma care. METHODS: A nationwide survey of members of t
