Stimulation of the human ventral tegmental area increases strategic betting.

Learning is a fundamental aspect of human behaviour and is essential for adapting to new environments and situations. The ventral tegmental area is a critical brain area containing neurons that release dopamine to signal reward, drive learning and bias decision-making. Human data on the ventral tegmental area's effects on cognition are scarce, and no studies have causally manipulated the human ventral tegmental area. Here we studied a unique group of patients who had deep brain stimulation surgery in the ventral tegmental area to improve pain due to trigeminal autonomic cephalalgias refractory

Headache characteristics in acromegaly: Only a secondary disorder?

OBJECTIVE: To investigate the characteristics and nature of headache in a population of patients with acromegaly. BACKGROUND: Headache is frequently described by patients with pituitary adenomas. Although it is mainly considered a secondary disorder, it can persist despite effective therapy for pituitary disease. A proper description of headache according to the subtype of pituitary adenoma is not available in literature. In this light, we aimed to analyze headache characteristics in a population of patients with acromegaly. METHODS: In this registry-based retrospective cohort study, headach

Efficacy and safety of CGRP monoclonal antibodies for cluster headache: a systematic review and network meta-analysis of randomized controlled trials.

INTRODUCTION: Cluster headache (CH) is a severe unilateral trigeminal autonomic cephalalgia with limited well tolerated therapies. Calcitonin gene related peptide monoclonal antibodies, including galcanezumab, fremanezumab, and eptinezumab, have established efficacy in migraine and are under evaluation in CH. This study assesses their efficacy and safety using pairwise and network meta analysis. METHODS: PubMed, Embase, Scopus, and ClinicalTrials.gov were searched through April 2025. Randomized controlled trials enrolling adults with episodic or chronic CH were included. The primary endpoint

Ultrasound guided pulsed radiofrequency neuromodulation of sphenopalatine ganglion treatment for allergic rhinitis in childern: A case report.

Allergic rhinitis (AR) is a chronic noninfectious inflammation of the nasal mucosa mediated primarily by allergen-specific immunoglobulin E (IgE) in atopic individuals after exposure to allergens, with the involvement of non-IgE-mediated mechanisms and neuroimmune dysregulation. Conservative treatment of AR is ineffective in children who lack compliance, and traditional surgical procedures are risky, making treatment of this community challenging. The sphenopalatine ganglion (SPG), aka pterygopalatine ganglion, is the largest of the four parasympathetic ganglia located within the head region,

Effectiveness of Combined Peripheral Nerve Blocks Versus Isolated Greater Occipital Nerve Block in the Management of Cluster Headache.

INTRODUCTION: Cluster headache (CH) is a primary headache disorder classified under trigeminal autonomic cephalalgias, characterized by severe, disabling pain attacks. In treatment-resistant cases, where pharmacological interventions are insufficient, peripheral nerve blocks have emerged as a promising interventional option. This study aimed to compare the efficacy of isolated greater occipital nerve (GON) block with combined GON, supraorbital nerve (SON), and sphenopalatine ganglion (SPG) blocks in reducing attack frequency, pain intensity, and treatment response in patients with CH. METHODS

Primary headache epidemiology in children and adolescents: a systematic review and meta-analysis.

INTRODUCTION: Headache is the most prevalent neurological manifestation in adults and one of the leading causes of disability worldwide. In children and adolescents, headaches are arguably responsible for a remarkable impact on physical and psychological issues, yet high-quality evidence is scarce. MATERIAL AND METHODS: We searched cross-sectional and cohort studies in Embase, Medline, Web of Science, and Cochrane databases from January 1988 to June 2022 to identify the prevalence of headaches in 8-18 years old individuals. The risk of bias was examined with the Joanna Briggs Institute (JBI)

Characteristics of the first 1000 headaches in an outpatient headache clinic registry.

OBJECTIVE: To analyze the incidence and characteristics of the first 1000 headaches in an outpatient clinic. BACKGROUND: Headache is a common cause of medical consultation, both in primary care and in specialist neurology outpatient clinics. The International Classification of Headache Disorders, 2nd Edition (ICHD-II), enables headaches to be classified in a precise and reproducible manner. METHODS: In January 2008, an outpatient headache clinic was set up in Hospital Clínico Universitario, a tertiary hospital in Valladolid, Spain. Headaches were classified prospectively in accordance with I

Therapeutical approaches to paroxysmal hemicrania, hemicrania continua and short lasting unilateral neuralgiform headache attacks: a critical appraisal.

BACKGROUND: Hemicrania continua (HC), paroxysmal hemicrania (PH) and short lasting neuralgiform headache attacks (SUNCT and SUNA) are rare syndromes with a difficult therapeutic approach. The aim of this review is to summarize all articles dealing with treatments for HC, PH, SUNCT and SUNA, comparing them in terms of effectiveness and safety. METHODS: A survey was performed using the pubmed database for documents published from the 1st January 1989 onwards. All types of articles were considered, those ones dealing with symptomatic cases and non-English written ones were excluded. RESULTS: In

Primary headache syndromes in the elderly: epidemiology, diagnosis and treatment.

Although secondary headaches due to e.g. temporal arteritis or a brain tumor are common among the elderly, primary headache disorders also occur in this age group, albeit less frequently than in younger individuals. A different presentation in the older age groups often makes a diagnosis difficult. Some headache syndromes, such as hypnic headache, are typical for the elderly. Furthermore, age-related physiologic changes, co-morbidities and contra indications require appropriate and targeted treatment in the elderly. Although treatments for the most common primary headaches are available, many

Primary headaches in children under the age of 7 years.

Recurrent headache is increasingly recognised in young children. Migraine and tension-type headache feature commonly amongst the primary headache disorders seen at this age. Headaches at this age are more likely than in older patients to be 'unclassifiable', possibly a reflection of the difficulties in obtaining a detailed headache history from a young child. Together with recent epidemiological data this review highlights the more prevalent primary headache types with advice on making a focussed headache assessment and guidance on management in this age group.

Understanding the Diagnosis of Primary Headaches in Outpatients Referred to Japanese Headache Specialists.

Background The timely and accurate diagnosis of primary headaches is crucial for enhancing patients' quality of life. However, primary headaches have been reported to be misdiagnosed and underdiagnosed worldwide, especially by nonspecialists. In Japan, despite the high prevalence of primary headaches, research on the extent of headache misdiagnosis and underdiagnosis remains limited. Therefore, we aimed to assess the rates of concordant primary headache diagnoses between previous physicians and headache specialists among outpatients who were referred to the headache center in Japan. Treatment of Cluster Headache: The American Headache Society Evidence-Based Guidelines.

BACKGROUND: Cluster headache (CH), the most common trigeminal autonomic cephalalgia, is an extremely debilitating primary headache disorder that is often not optimally treated. New evidence-based treatment guidelines for CH will assist clinicians with identifying and choosing among current treatment options. OBJECTIVES: In this systematic review we appraise the available evidence for the acute and prophylactic treatment of CH, and provide an update of the 2010 American Academy of Neurology (AAN) endorsed systematic review. METHODS: Medline, PubMed, and EMBASE databases were searched for doub

Prevalence and Features of a Probable Diagnosis in First-Visit Headache Patients Based on the Criteria of the Third Beta Edition of the International Classification of Headache Disorders: A Prospective, Cross-Sectional Multicenter Study.

OBJECTIVES: This study aimed to determine the characteristics and significance of a probable diagnostic entity for primary headache disorder (PHD). BACKGROUND: A diagnosis of probable primary headache disorder (PPHD) is given when a patient's headache fulfills all but one criteria of the third beta edition of the international classification of headache disorder (ICHD-3β). Despite the uncertainty regarding this diagnosis, the inclusion of a probable diagnosis entity in this manual may aid in the accurate classification of headache disorders and allow effective treatment strategies to be start

Episodic cluster headache with suspected giant cell arteritis, sinusitis-related headache, and dietary triggers: a case report.

BACKGROUND: Cluster headache is a primary trigeminal autonomic cephalalgia characterized by severe, strictly unilateral orbital/periorbital pain with ipsilateral autonomic features. It is rare for a patient with cluster headache to have coexisting multiple headache disorders and experience unusual triggers. CASE PRESENTATION: A 53-year-old male with a 14-year history of episodic cluster headache presented with his third cluster episode. He exhibited strictly unilateral left orbital/periorbital/temporal drilling pain [Visual Analogue Scale (VAS) 9-10/10], ipsilateral conjunctival injection, la

Neurophysiological tests and neuroimaging procedures in non-acute headache (2nd edition).

BACKGROUND AND PURPOSE: A large number of instrumental investigations are used in patients with non-acute headache in both research and clinical fields. Although the literature has shown that most of these tools contributed greatly to increasing understanding of the pathogenesis of primary headache, they are of little or no value in the clinical setting. METHODS: This paper provides an update of the 2004 EFNS guidelines and recommendations for the use of neurophysiological tools and neuroimaging procedures in non-acute headache (first edition). Even though the period since the publication of

Primary headache disorders.

Primary headache disorders include migraine, tension-type headaches, and the trigeminal autonomic cephalgias (TACs). "Primary" refers to a lack of clear underlying causative pathology, trauma, or systemic disease. The TACs include cluster headache, paroxysmal hemicrania, and short-lasting neuralgiform headache attacks with conjunctival injection and tearing; hemicrania continua, although classified separately by the International Headache Society, shares many features of both migraine and the TACs. This article describes the features and treatment of these disorders.
