[Percutaneous drainage of splenic fluid accumulations. Apropos of 4 cases].

Four patients with solitary or multiple splenic fluid collections were treated by ultrasound guided percutaneous needle aspiration and drainage. There was one unilocular abscess, three infected hematomas, one large hematoma and a complex lesion with locular and perisplenic involvement. Percutaneous drainage and antibiotics were curative in one patient. Splenectomy was performed because of persistent fever and drainage of pus after 15 days in one case and recurrence of hematoma in an other case. Solitary fluid collection can be effectively treated by ultrasound or computed tomography guided per

Isolated spleen tuberculosis in an immunocompetent patient, a rare case report.

INTRODUCTION: Tuberculosis (TB), as a major public health concern, is affecting almost 10 million people globally. At present, diagnostic and screening efforts mainly focus on positive smear results. Therefore, the number of extra pulmonary and negative sputum TB is rising and hampering the diagnosis and treatment process due to the large number of false negatives. Rare cases such as solitary splenic TB are usually seen in patients with splenic abnormalities, spleen trauma, immunosuppression, sickle cell disease, pyogenic infections, etc. PRESENTATION OF CASE: A 40-year-old female with no como

Multiple reflective channels in the spleen: a sonographic sign of portal hypertension.

We consistently observed small "reflective channels" (RCs) in the splenic parenchyma in patients with portal hypertension (PHT). The purpose of this study was to investigate the frequency of this sign in PHT and in splenic disorders unrelated to PHT compared to normal controls. The significance of this sign in the diagnosis of PHT and in differentiating PHT splenomegalies from others was also assessed. A total of 337 patients underwent sonographic examination of the spleen: 147 normal, 75 non-PHT splenic pathologies, and 115 with PHT. The RCs were scored from grade 0 to 3 by counting the numbe

Isolated tuberculous splenic abscess in an immunocompetent individual.

Tuberculosis (TB) of the spleen is an extremely rare clinical entity particularly among immunocompetent persons. We report a case of isolated tuberculous abscess of spleen in a 13- years- old boy. No primary focus of infection was detected in lungs or any other organ. The patient was treated by splenectomy after a therapeutic failure with standard antituberculous medication.

[The clinical picture and diagnosis of complicated forms of abdominal tuberculosis].

The paper presents the diagnostic features of 95 cases with complicated forms of abdominal tuberculosis (AT). The latter has been found to be concurrent with disseminated progressive pulmonary tuberculosis in 97.3% of cases. HIV infection is present in 25.2% of the patients. In 42.1%, a complication of AT is its first onset. The late diagnosis of AT in the phase of development of complications is due to its asymptomatic course at the early stage, the similarity of clinical manifestations with various nonspecific abdominal diseases, the low informative value of radiodiagnostic techniques, and s

Fine needle aspiration diagnosis of isolated pancreatic tuberculosis. A case report and review of literature.

CONTEXT: Tuberculosis is a common disease in the developing world and its incidence is slowly increasing in developed countries where a resurgence has been seen subsequent to the AIDS epidemic. Tuberculosis, in its extrapulmonary form, though emerging as a clinical problem, rarely affects the pancreas. The pancreas is biologically protected from being infected by Mycobacterium tuberculosis. Pancreatic tuberculosis presents with a wide spectrum of symptoms such as abdominal pain, constitutional symptoms, obstructive jaundice, iron deficiency anemia, pancreatic abscess, massive gastro-intestinal

Ultrasound in abdominal tuberculosis.

BACKGROUND: The present study was done to emphasize the importance of ultrasound (US) and US-guided fine-needle aspiration biopsy (FNAB) in the diagnosis of abdominal tuberculosis. METHODS: Sixty-six proven cases of abdominal tuberculosis were selected for this study. The diagnosis was based on clinical features, US observations, FNAB, operative findings, and responses to appropriate antitubercular treatment. The US findings were interpreted with regard to the involvement of lymph nodes, intestine, peritoneum, solid viscera, and abdominal abscesses. Patients with disease limited to the muscul

Listeria-Vectored Vaccine Expressing the Mycobacterium tuberculosis 30-Kilodalton Major Secretory Protein via the Constitutively Active prfA* Regulon Boosts Mycobacterium bovis BCG Efficacy against Tuberculosis.

A potent vaccine against tuberculosis, one of the world's deadliest diseases, is needed to enhance the immunity of people worldwide, most of whom have been vaccinated with the partially effective Mycobacterium bovis BCG vaccine. Here we investigate novel live attenuated recombinant Listeria monocytogenes (rLm) vaccines expressing the Mycobacterium tuberculosis 30-kDa major secretory protein (r30/antigen 85B [Ag85B]) (rLm30) as heterologous booster vaccines in animals primed with BCG. Using three attenuated L. monocytogenes vectors, L. monocytogenes ΔactA (LmI), L. monocytogenes ΔactA ΔinlB (Lm

Increase of tuberculous infection in the organs of B cell-deficient mice.

Protective immunity against infection with Mycobacterium tuberculosis is imparted by T cells rather than antibodies, but B cells can play a role as antigen-presenting cells and in granuloma formation. We re-evaluated the role of B cells in the course of tuberculous infection in mu-chain knock-out (Ig-) mice. Surprisingly, the organs of M. tuberculosis-infected Ig- mice were found to have three- to eight-fold elevated counts of viable bacilli compared with normal littermates at 3-6 weeks post-infection. Splenic interferon-gamma responses to whole antigen were unimpaired, whilst proliferation to

Systemic lupus erythematosus with diffuse splenic calcification: A case report.

OBJECTIVE: Systemic lupus erythematosus (SLE) is a systemic autoimmune disease with the main clinical characteristics of multisystem and multiorgan involvement of the whole body. It is commonly seen in skin mucosa, skeletal muscle, and the respiratory system while rarely involving the spleen. In this case, we present a young female patient with SLE with the symptom of progressing splenic calcification. METHODS: A 21-year-old female patient was admitted to our hospital in 2012 with complaints of "fever, abdominal pain, diarrhea, and knee pain", and then was diagnosed with systemic lupus erythe

Differentiating between active and latent tuberculosis with chest computed tomography.

PURPOSE: The purpose of this study was to evaluate the capabilities of chest computed tomography (CT) in distinguishing between active and latent tuberculosis in patients positive for interferon-gamma release assay (IGRA) testing, and to compare the performance of CT with that of quantitative IGRA testing in a low incidence setting. MATERIALS AND METHODS: Patients with latent or active tuberculosis define by an IGRA positive test were retrospectively recruited. Sensitivity, specificity and accuracy were determined for CT variables and quantitative IGRA results. Final diagnosis of active tuber

The role of B cells in an early immune response to Mycobacterium bovis.

Mycobacterium tuberculosis is the main etiological agent of tuberculosis. The Bacillus Calmette-Guérin (BCG) microbes that are primarily used as a vaccine against tuberculosis also constitute the dominant infection model for studying the interaction of mycobacteria with the host cell types. The majority of interaction experiments have been conducted using macrophages and monocytes as prototype phagocyte cell types. Here, we report that M. bovis BCG infects mouse primary B cells as well as human B cell line. The complement receptors, along with B cell receptors, are engaged in the process of ba

Melioidosis Presenting As a Chronic Splenic Abscess: A Diagnostic Challenge.

Melioidosis is a potentially fatal infection caused by Burkholderia pseudomallei, often presenting as various clinical manifestations and is commonly misdiagnosed in endemic regions, leading to delayed diagnosis. We report the case of a middle-aged female patient with uncontrolled diabetes mellitus and occupational exposure as a farmer, who presented with chronic fever and abdominal pain of one year duration. She had a prior history of a treated lumbar abscess where microbiological diagnosis was missed. Imaging revealed a multiloculated splenic abscess with features of impending rupture. Possi

Prolonged paradoxical response to anti-tuberculous treatment after infliximab.

A 56-year-old woman with ankylosing spondylitis, treated for 3 months with infliximab, developed miliary tuberculosis with mediastinal lymphadenopathies and brain and splenic lesions. After initial improvement under anti-tuberculous therapy, she suffered an unexpectedly prolonged paradoxical worsening with several episodes of lymphadenopathy, including life-threatening ones, over a period of more than 14 months of follow-up. The outcome was favorable as a result of corticosteroid and surgical treatments. This phenomenon reflects a paradoxical reaction precipitated by infliximab withdrawal.

Lymphoreticular diseases in Nigerians.

BACKGROUND: The lymphoreticular system plays a major role in both the innate and adaptive immune responses. This study reviews retrospectively cases of lymphoreticular diseases seen at a tertiary institution in Nigeria. MATERIALS AND METHODS: This is a retrospective study in which biopsies from the bone marrow, spleen, lymph nodes and extranodal lymphoid tissues reported within a period of 16 years were reviewed with respect to age, sex and pathological diagnosis. Statistical analysis was performed for differences in proportion using Chi square by SPSS version 12. RESULTS: Nine hundred and f

Tuberculosis mimicked by melioidosis.

BACKGROUND: In regions endemic for tuberculosis (TB) such as India, presumptive anti-tuberculosis treatment is often prescribed. Melioidosis, caused by Burkholderia pseudomallei, is underdiagnosed in India, due to lack of awareness and a low index of suspicion. SETTING: A tertiary care hospital in south India. OBJECTIVE: To present our analysis of a series of 22 cases of suspected TB that was later confirmed as melioidosis. DESIGN: Twenty-two patients with culture-proven melioidosis, who were initially given empirical anti-tuberculosis treatment, were retrospectively analysed regarding clin
