Diagnosis and management of familial Mediterranean fever: integrating medical genetics in a dedicated interdisciplinary clinic.

Familial Mediterranean fever is an autosomal recessive genetic disorder characterized by recurrent febrile polyserositis, especially prevalent in individuals of Mediterranean descent. Familial Mediterranean fever can have nonspecific manifestations that mimic many common acquired disorders such as infections, acute appendicitis, cholecystitis, and arthritis, which can delay diagnosis for many years and subject patients to extensive evaluations and even unnecessary surgery. Untreated familial Mediterranean fever can result in serious complications such as end-stage renal disease and malabsorpti

[Colchicine: rationale for its use in liver disease].

Colchicine is a substance derived from the Colchicum Autumnale plant, its pharmacological uses have been demonstrated over the years. Initially used for the treatment of acute gout, it has been utilized for a wide variety of diseases, including mediterranean familial fever, alcoholic, posthepatitic and primary biliary liver cirrhosis. We have demonstrated in different studies that colchicine reduces markedly mortality rates in cirrhotics and the levels of interleukin-1 in patients with primary biliary cirrhosis.

Discovery of a Novel Coumarin/Thiazole Chalcone Hybrid as a Potent Dual Inhibitor of Tubulin and Carbonic Anhydrases IX & XII with Promising Anti-Proliferative Activity.

Multitarget-directed ligands offer a promising strategy for overcoming tumor complexity through simultaneous modulation of complementary oncogenic pathways. In this work, a novel (E)-6-(3-(4-methyl-2-thioxo-2,3-dihydrothiazol-5-yl)-3-oxoprop-1-en-1-yl)-2H-chromen-2-one (compound 6) was synthesized and evaluated as a dual inhibitor of tubulin polymerization and tumor-associated carbonic anhydrases (CAs) IX and XII. Compound 6 displayed potent antiproliferative activity, particularly against MDA-MB-231 triple-negative breast cancer cells (IC50 = 0.37 µM), with excellent selectivity toward non-tu

Design and Synthesis of Combretastatin A4-Chalcone Hybrids as Tubulin Polymerization Inhibitors.

A new series of combretastatin A4-chalcone hybrids were designed and synthesized to explore their potential as cytotoxic agents. The compounds were evaluated for cytotoxic activity, tubulin inhibition, and anti-angiogenic properties. The designed hybrids exhibited significant cytotoxic effects across all the tested cancer cell lines. Among them, two compounds, 5d and 5o, exhibited potent cytotoxicity, with IC50 values of 0.02 and 0.03 μM, respectively, against the prostate cancer-3 cell line. Further biological investigations suggested that treatment with 5d and 5o induces apoptosis, as eviden

Quinolone analogue inhibits tubulin polymerization and induces apoptosis via Cdk1-involved signaling pathways.

Cancer chemotherapeutic agents that interfere with tubulin/microtubule function are in extensive use. Quinolone is a common structure in alkaloids and its related components exhibit several pharmacological activities. In this study, we have identified the anticancer mechanisms of 2-phenyl-4-quinolone. 2-Phenyl-4-quinolone displayed anti-proliferative effect in several cancer types, including hormone-resistant prostate cancer PC-3, hepatocellular carcinoma Hep3B and HepG2, non-small cell lung cancer A549 and P-glycoprotein-rich breast cancer NCI/ADR-RES cells. The IC(50) values were 0.85, 1.81,

Inhibition of protein-protein interactions among tubulins in vitro and in cellulo using C-terminal-binding nanoparticles.

Protein-protein interactions (PPIs) are fundamental to cellular function, yet selectively inhibiting them remains a significant challenge. This difficulty arises because PPI interfaces involve surface areas (1000-5000 Å2) significantly larger than those involved in small-molecule ligand recognition. Here, we report the development of molecularly imprinted nanoparticles (MINPs) designed to target the disordered C-terminal tails (CTTs) of α- and β-tubulins. By modulating hydrogen-bonding functional monomers and cross-linkers, we tuned the MINP binding properties to achieve either dual α- and β-t

The Role of MAPRE2 and Microtubules in Maintaining Normal Ventricular Conduction.

BACKGROUND: Brugada syndrome is associated with loss-of-function SCN5A variants, yet these account for only ≈20% of cases. A recent genome-wide association study identified a novel locus within MAPRE2, which encodes EB2 (microtubule end-binding protein 2), implicating microtubule involvement in Brugada syndrome. METHODS: A mapre2 knockout zebrafish model was generated using CRISPR/Cas9 (clustered regularly interspaced short palindromic repeats/clustered regularly interspaced short palindromic repeat-associated protein 9) and validated by Western blot. Larval hearts at 5 days post-fertilizatio

Machine learning-driven computational drug repurposing to identify new tubulin inhibitors against cancer.

Tubulin is a validated anticancer target, yet the clinical translation of colchicine-binding site inhibitors remains limited by toxicity and resistance. To accelerate the discovery of safer tubulin-targeting agents, we employed a machine learning (ML)-driven drug repurposing strategy integrating computational and experimental validation. Robust AutoQSAR classification models were trained on 279 curated tubulin inhibitors and used to screen 4500 US FDA-approved drugs, predicting 1800 compounds as potential tubulin inhibitors. These candidates were subjected to multistage structure-based virtual

Design and evaluation of dual c-Met/β-tubulin inhibitors for triple-negative breast cancer: An In Silico approach.

Triple-negative breast cancer (TNBC) remains one of the most aggressive and treatment-resistant subtypes of breast malignancies, defined by the absence of hormone receptors and HER2 amplification. This study introduces an integrated computer-aided drug design (CADD) framework that combines Monte Carlo-optimized QSAR modeling, molecular docking, and in silico pharmacokinetic assessment to identify dual c-Met/β-tubulin inhibitors with potential anti-TNBC activity. Robust conformation-independent QSAR models were developed using SMILES- and graph-based descriptors, achieving high internal consist

Activities of fenbendazole in comparison with albendazole against Echinococcus multilocularis metacestodes in vitro and in a murine infection model.

The current chemotherapeutic treatment of alveolar echinococcosis (AE) in humans is based on albendazole and/or mebendazole. However, the costs of treatment, life-long consumption of drugs, parasitostatic rather than parasiticidal activity of chemotherapy, and high recurrence rates after treatment interruption warrant more efficient treatment options. Experimental treatment of mice infected with Echinococcus multilocularis metacestodes with fenbendazole revealed similar efficacy to albendazole. Inspection of parasite tissue from infected and benzimidazole-treated mice by transmission electron

Efficacy and tolerability of switching from coated to compressed colchicine preparations in patients with Behçet's syndrome.

OBJECTIVES: Beneficial results have been reported among patients with familial Mediterranean fever who switched from coated colchicine to compressed colchicine preparations due to inefficacy or intolerance. In this study, we aimed to assess whether this approach is also beneficial in patients with Behçet's syndrome (BS) with mucocutaneous and/or joint involvement who had an inadequate response to or were intolerant of coated colchicine preparations. METHODS: We conducted a retrospective chart review of BS patients who switched to compressed colchicine preparation between 2017 and 2023. Endpoi

Protracted arthritis in a Japanese patient with familial Mediterranean fever.

The most common arthritic involvement in familial Mediterranean fever (FMF) is acute self- limiting monoarthritis which typically lasts for 72 h. Hip joint involvement is uncommon in FMF and can result either from a process specific to this disease or from a coexisting inflammatory joint disease. We describe a 37-year-old woman with FMF and right osteoarthritis secondary to congenital hip dislocation. Periodic fever with right coxalgia lasting for 6 months was treated using colchicine. Genetic analysis revealed homozygous mutation in the MEFV gene (L110P-E148Q/L110P-E148Q), confirming the FMF

Rationale and design of the COlchicine for Prevention of the Post-pericardiotomy Syndrome and Post-operative Atrial Fibrillation (COPPS-2 trial): a randomized, placebo-controlled, multicenter study on the use of colchicine for the primary prevention of the postpericardiotomy syndrome, postoperative effusions, and postoperative atrial fibrillation.

BACKGROUND: The efficacy and safety of colchicine for the primary prevention of the postpericardiotomy syndrome (PPS), postoperative effusions, and postoperative atrial fibrillation (POAF) remain uncertain. Although preliminary data from a single trial of colchicine given for 1 month postoperatively (COPPS trial) were promising, the results have not been confirmed in a large, multicenter trial. Moreover, in the COPPS trial, colchicine was given 3 days postoperatively. METHODS: The COPPS-2 study is a multicenter, double-blind, placebo-controlled randomized trial. Anticancer Activity of Novel Trifluoromethylquinoline Derivatives Designed to Target the Tubulin Colchicine Binding Site.

Colchicine binding site inhibitors (CBSIs) have developed rapidly in recent years as antitumor agents. Currently, no inhibitors of this specific class are approved for clinical use. In this study, a series of novel 2-trifluoromethyl-4-aminoquinoline derivatives were designed, synthesized, and evaluated for their antiproliferative activity. Among them, compound 8a was the most potent against LNCaP cells with an IC50 value of 1 nM. Mechanistic studies demonstrated that 8a disrupted tubulin polymerization by binding to the colchicine site of tubulin. Meanwhile, 8a could induce cell apoptosis. Mol

In Silico Exploration of Novel Tubulin Inhibitors: A Combination of Docking and Molecular Dynamics Simulations, Pharmacophore Modeling, and Virtual Screening.

Microtubules play a critical role in mitosis and cell division and are regarded as an excellent target for anticancer therapy. Although microtubule-targeting agents have been widely used in the clinical treatment of different human cancers, their clinical application in cancer therapy is limited by both intrinsic and acquired drug resistance and adverse toxicities. In a previous work, we synthesized compound 9IV-c, ((E)-2-(3,4-dimethoxystyryl)-6,7,8-trimethoxy-N-(3,4,5-trimethoxyphenyl)quinoline-4-amine) that showed potent activity against multiple human tumor cell lines, by targeting spindle
