Investigations on the biology, epidemiology, pathology and control of Tunga penetrans in Brazil. VI. Natural history of the infestation in laboratory-raised Wistar rats.

Tungiasis is endemic in many countries in Latin America, the Caribbean and sub-Saharan Africa, and it is associated with severe morbidity. The pathophysiological and immunological characteristics of the ectoparasitosis are not well understood, and no effective therapy is currently available. The aim of this study was to describe the natural history of tungiasis in laboratory-raised Wistar rats. The rats were exposed in the laboratory to the parasite or were kept in a natural environment with an intense transmission of Tunga penetrans. The time course of the infestation was determined, and lesi

Investigations on the biology, epidemiology, pathology, and control of Tunga penetrans in Brazil: VII. The importance of animal reservoirs for human infestation.

In Brazil tungiasis is endemic in many resource-poor communities, where various domestic and sylvatic animals act as reservoirs for this zoonosis. To determine the role of animal reservoirs in human tungiasis, a cross-sectional study was performed in a traditional fishing community in northeast Brazil. The human and the animal populations were examined for the presence of embedded sand fleas and the prevalence and the intensity of infestation were correlated. The overall prevalence of tungiasis in humans was 39% (95% CI 34-43%). Of six mammal species present in the village, only cats and dogs

Tunga penetrans. A case report and review of the literature.

Tungiasis is an infection caused by infestation of Tunga Penetrans into the epidermis. Although, at the moment, presentation of this condition seems rare in Europe and Northern America, tungiasis is highly endemic in other parts of the world. We describe a case of Tunga Penetrans infection in the foot of a 22-year old Dutch female returning from Kenya, and present a review of the literature. Now that intercontinental travel is increasing, physicians in other parts of the world will be expected to recognize and treat this exotic infection. Knowledge of human tungiasis is mandatory in order to r

High intensity of Tunga penetrans infection causing severe disease among pigs in Busoga, South Eastern Uganda.

BACKGROUND: Towards the improvement of stakeholders' awareness of animal tungiasis, we report 10 unusual severe clinical cases of pig tungiasis which were associated with very high infection intensities of T. penetrans in an endemic area. RESULTS: Morbidity of ten pigs with high sand flea intensities detected during high transmission seasons in an endemic area in Busoga sub region, Uganda is described in detail. The cases of pigs presented with a very high number of embedded sand fleas (median = 276, range = 141-838). Acute manifestations due to severe tungiasis included ulcerations (n = 10),

[Are sandflea infections increasing?].

A report on two cases is given. The patients showed the typical skin affections after trips to Africa. Tungiasis occasionally leads to secondary infections and may be the cause of longterm inflammation or erysipelas. Treatment is discussed and a possible increase of tungiasis because of an increase of tourists to tropical countries is pointed out.

Investigations on the biology, epidemiology, pathology and control of Tunga penetrans in Brazil: III. Cytokine levels in peripheral blood of infected humans.

Tungiasis is caused by penetration of the female jigger flea, Tunga penetrans, into the skin of its host. This parasitic skin disease is almost invariably associated with intense inflammation around embedded fleas, the underlying mechanisms being unknown. A study was undertaken to determine whether the inflammatory process can be attributed to immune activation induced by a biologically active foreign body. We determined the concentrations of Th1-mediated (IFN-gamma, TNF-alpha) and Th2-mediated (IL-4) cytokines in the sera of patients with tungiasis. The results were compared with those of con

Field trial of the efficacy of a combination of imidacloprid and permethrin against Tunga penetrans (sand flea, jigger flea) in dogs in Brazil.

In a field trial in Brazil 17 dogs penetrated by females of the jigger flea, Tunga penetrans, were topically treated with a combination of 10% imidacloprid and 50% permethrin (Advantix), while 17 dogs remained untreated. The follow-up controls on days 7, 14, 21 and 28 post-treatment clearly showed that, beginning from day 7, the flea load in treated dogs decreased, so that most of the dogs became free of tungiasis lesions, while in the untreated group the flea load remained high. Since the dogs distribute the flea eggs throughout the village, leading to a high incidence of tungiasis in humans,

Investigations on the biology, epidemiology, pathology and control of Tunga penetrans in Brazil: IV. Clinical and histopathology.

Tungiasis is a parasitic skin disease caused by the sand flea Tunga penetrans. This ectoparasitosis is endemic in South America, the Caribbean and sub-Saharan Africa, where it is an important but neglected health problem in resource-poor communities. As part of a study of tungiasis-related morbidity in a typical slum in Fortaleza, Brazil, we identified 86 individuals with tungiasis. Lesions were counted, classified according to the stage of development, and clinical pathology was documented. One hundred and nine lesions were biopsied and examined by histological sectioning. The patients had be

Typical histologic features of Tunga penetrans in skin biopsies.

CONTEXT: Tunga penetrans is a flea that burrows into human skin, causing the disease tungiasis. Although the parasite is not endemic in the United States, patients may present with this disease upon returning from tropical locales. Histologic sections contain a variety of flea parts that may present a diagnostic dilemma for pathologists unfamiliar with this disease. OBJECTIVE: To determine the typical histologic features of T penetrans in biopsies from patients with tungiasis. METHODS: We reviewed biopsy specimens from 7 patients with tungiasis and sought 8 distinct structures: the exoskelet

Topographic distribution of the sand flea Tunga penetrans in Wistar rats and humans in two endemic areas in Brazil.

Tungiasis is a zoonosis caused by Tunga penetrans. In Brazil, tungiasis is endemic in many resource-poor communities, in which various domestic and sylvatic animals act as reservoirs. Eighty laboratory-raised Wistar rats were exposed to T. penetrans in areas of intense transmission: a fishing village and an urban shantytown in Ceará State, northeast Brazil. The topographic distribution of lesions in Wistar rats was compared with the distribution of lesions in humans in the same area. Our results show that the topographic distribution of embedded sand fleas was almost identical in Wistar rats a

High exposure to Tunga penetrans (Linnaeus, 1758) correlates with intensity of infestation.

Tungiasis is a parasitic skin disease widespread in resource-poor urban and rural communities in Brazil. Inhabitants of an urban slum in Northeast Brazil were examined for the presence of tungiasis lesions and followed-up twice a week for a period of three weeks. Each time the number, stages, and topographic localization of lesions were recorded on a documentation sheet. The infestation rate (number of newly embedded sand fleas per individual and day) remained stable during the observation period. The infestation rate was significantly related to the intensity of infestation (total number of l

Efficacy of an oral formulation of afoxolaner and milbemycin oxime against Tunga penetrans in naturally infected dogs.

BACKGROUND: The sand flea Tunga penetrans is one of the agents of tungiasis, an important parasitic skin disease affecting humans and several mammalian species. Tungiasis is mainly observed in disadvantaged rural and peripheral urban communities in Latin America and sub-Saharan Africa. The dog is a major reservoir of Tunga fleas. Hematophagous adult female Tunga spp. embed and grow in their host's epidermis and cause cutaneous inflammatory disorders. NexGard Spectra® is an orally administered endectocide for dogs, a co-formulation of the isoxazoline afoxolaner and the macrocyclic lactone milbe

Spatial Distribution of Off-Host Stages of Tunga penetrans in the Soil within the Home Range of Nine Infected Dogs in An Endemic Tourist Area in Brazil.

Tungiasis is a neglected disease caused by the sand flea Tunga penetrans, and dogs are considered the main reservoirs in Brazil. This study aimed to identify the role of dogs as tungiasis dispersers and to investigate the presence of T. penetrans in the soil of an endemic tourist area. Nine dogs infected by T. penetrans were included in this study and received GPS collars to analyze their movement through the village. Duplicate sand samples were collected in different areas of the community. Those areas were classified as peridomicile (n = 110), open area (n = 110), beach (n = 50), and river (

Stowaways with wings: two case reports on high-flying insects.

More people than ever before are traversing continents, either for business purposes or on holiday. Because 3-10 percent of these travelers experience skin, hair, or nail disorders related to these trips there is an increasing likelihood that Western physicians will be expected to treat exotic conditions imported from different countries. Tungiasis and furunculoid myiasis are two typical disorders of intertropical regions. They represent nuisances induced by the presence of arthropod larvae or eggs in the skin. We describe a case of tungiasis, caused by the sand flea Tunga penetrans (TP), and

Infestation of Wistar rats with Tunga penetrans in different microenvironments.

Tungiasis is a zoonotic ectoparasitosis that causes considerable morbidity in affected populations. The type of microenvironment that facilitates infestation of hosts by Tunga penetrans has not been investigated. In this study, we exposed 30 laboratory-raised Wistar rats, a suitable model for the infestation, at six different places characterized by different microenvironments in a hyperendemic fishing village in northeastern Brazil. During a period of two weeks, the animals were monitored and the number of embedded fleas was documented. The number of lesions varied considerably according to t

Travel- and migration-associated epidermal parasitic skin diseases. A review.

Epidermal parasitic skin diseases are a family of parasitic diseases which occur globally or are frequent in special settings. Epidermal parasitic skin diseases occur in travelers and migrants, although epidemiology and/or clinical manifestations differ between these groups. The objective of this study is to summarize the existing knowledge concerning tungiasis, hookworm-related cutaneous larva migrans, scabies, pediculosis capitis and pediculosis corporis in the context of travel and migration, and to identify risk factors for infection. The PUBMED database was searched using the terms tungia

Dynamics of Tunga penetrans infections and severity of associated morbidity among pigs during the dry season in rural Uganda.

BACKGROUND: Tungiasis is a neglected tropical disease which is common in impoverished communities. In sub-Saharan Africa, it is caused by female sand fleas, Tunga penetrans, and pigs are amongst the major domestic animal reservoirs. Depending on the environment, tungiasis occurs throughout the year or preferentially in the dry seasons. This study investigated changes in sand flea abundance and associated morbidity in pigs during a dry season. METHODS: Tunga penetrans lesions were counted and staged in 35 pigs amongst 22 households with at least one affected pig. Five weekly examinations were
