One-Day Aminoglycosides versus Three-Day Ceftazidime for Perioperative Prophylaxis and Postoperative Sepsis Risk in Patients with Urinary Calculi: A Propensity Score-Matched Retrospective Cohort Analysis.

BACKGROUND: Perioperative antimicrobial prophylaxis reduces infectious complications after urinary stone removal, but the optimal agent to prevent postoperative sepsis remains uncertain amid rising drug-resistant gram-negative bacteria. We compared aminoglycoside prophylaxis versus ceftazidime for prevention of postoperative sepsis. METHODS: In this retrospective propensity score-matched cohort study, clinical data were retrospectively extracted from the electronic medical record system after completion of routine clinical care. Among 420 matched patients undergoing endourological or percutan

Performance of Quick Sequential (Sepsis Related) and Sequential (Sepsis Related) Organ Failure Assessment to Predict Mortality in Patients with Acute Pyelonephritis Associated with Upper Urinary Tract Calculi.

PURPOSE: The Third International Consensus Definitions for Sepsis and Septic Shock Task Force proposed a new definition of sepsis based on the SOFA (Sequential [Sepsis-related] Organ Failure Assessment) score and introduced a novel scoring system, quickSOFA, to screen patients at high risk for sepsis. However, the clinical usefulness of these systems is unclear. Therefore, we investigated predictive performance for mortality in patients with acute pyelonephritis associated with upper urinary tract calculi. MATERIALS AND METHODS: This retrospective study included 141 consecutive patients who w

Source of a micro-nutrient in a semi-synthetic basal diet as a causative factor in inducing urinary calculi in rats and its inhibition by PSC 833, a potent inhibitor of P-glycoprotein.

We report a serendipitous finding of urinary calculi in rats fed a semi-synthetic basal diet. This observation was made during ongoing studies to evaluate the inhibitory effect of PSC 833, a potent inhibitor of P-glycoprotein, on development of tumors in rodent tumor model systems. A large number of specific-pathogen-free (SPF) female Sprague-Dawley and SPF male Fischer 344 rats being fed the diet were euthanized when it became evident clinically that they were uremic. At necropsy, the renal pelvis, ureters, and urinary bladder contained numerous calculi. The presence of urinary calculi was de

Urolithiasis/nephrolithiasis: what's it all about?

Urolithiasis (urinary tract calculi or stones) and nephrolithiasis (kidney calculi or stones) are well-documented common occurrences in the general population of the United States. The etiology of this disorder is multifactorial and is strongly related to dietary lifestyle habits or practices. Proper management of calculi that occur along the urinary tract includes investigation into causative factors in an effort to prevent recurrences. Urinary calculi or stones are the most common cause of acute ureteral obstruction. Approximately 1 in 1,000 adults in the United States are hospitalized annua
