Design and development of a novel automatic valve system for long-term catheterized urinary incontinence patients.

It has been estimated that over 3 million patients in the UK suffer from urinary incontinence, the result of which is often long-term catheterization. However, many catheters block prematurely through encrustation and their continuous drainage limits bladder rehabilitation. Although evidence shows that a catheter valve may overcome such weaknesses, only manual valves are currently available and many patients are not able to benefit from these owing to a lack of manual dexterity. A novel electronically controlled automatic valve system, the Shan-Lai (SL) valve system, has been designed and prot

Risk factors for bacteriuria and clinical urinary tract infection in patients treated with clean intermittent catheterization.

To study factors that may predict the occurrence of bacteriuria and clinical urinary tract infection, a total of 302 patients using clean intermittent catheterization was followed. Bacteriuria was found equally in men and women, while clinical urinary tract infection was significantly higher among women. Predictive factors of clinical urinary tract infection were low age and high mean catheterization volume in women. In men low age, neurogenic bladder dysfunction and nonself-catheterization were predictors in addition to urine leakage in patients with neurogenic dysfunction. Bacteriuria was a

Genomics reveal Staphylococcus aureus persists during long-term urinary catheterization despite antimicrobial therapy and catheter exchanges.

Urinary catheters, the most frequently placed medical devices in the US, increase the risk of developing symptomatic catheter-associated urinary tract infection (CAUTI) and asymptomatic bacteriuria (ASB) - the presence of bacteria in the urine - with those requiring long-term urinary catheters (LTUCs) at highest risk. While ASB and CAUTI are caused by a broad range of uropathogens, most remain understudied. We use whole-genome sequencing to investigate the understudied uropathogen, Staphylococcus aureus. Analysis of 153 longitudinal S. aureus isolates previously collected from urinary catheter

Factors associated with recurrent urinary tract infections in spinal cord injured patients who use intermittent catheterization.

INTRODUCTION: Urinary Tract Infection (UTI) has been cited as the primary cause of morbidity in patients with history of spinal cord injury (SCI). Despite the significance of recurrent UTI (rUTI) in this population, the causative physiologic and patient characteristics are not well described. We sought to assess associations between demographic, clinical and urodynamic variables and rUTI. MATERIALS AND METHODS: The records of 136 individuals with SCI who perform clean intermittent catheterization (CIC) were retrospectively reviewed. All had a video urodynamics study (VUDS) available for analy

Incidence of postoperative urinary retention (POUR) after joint arthroplasty and management using ultrasound-guided bladder catheterization.

BACKGROUND: Postoperative urinary retention (POUR) following lower limb arthroplasty is a common complication. The aim of this observational study was to establish the incidence of POUR and assess the usefulness of an ultrasonographic nurse-driven protocol, thereby avoiding elective bladder catheterization. METHODS: Two-hundred and eighty six consecutive patients undergoing elective hip and knee arthroplasty were retrospectively studied. None of the patients received elective bladder catheterization. Data on risk factors for POUR, urinary tract infections, length of hospital stay and analgesi

Intravesical Gentamicin for Prevention of Recurrent Urinary Tract Infections in Patients With Neurogenic Lower Urinary Tract Dysfunction Performing Intermittent Catheterization: A Systematic Review.

INTRODUCTION: Patients with neurogenic lower urinary tract dysfunction (NLUTD) performing clean intermittent catheterization (CIC) are at increased risk of recurrent urinary tract infections (rUTIs). Oral antimicrobial prophylaxis has limited efficacy and fosters resistance. Intravesical gentamicin (IVG) has emerged as an alternative, but evidence remains limited and heterogeneous. This systematic review evaluated the efficacy and safety of IVG for rUTI prevention in adults with NLUTD and summarized reported instillation protocols and additional clinically relevant outcomes. METHODS: A system

Reducing Intermittent Catheterization in Cases of Postvoid Residual Urine Without Increasing Hospital-Acquired Urinary Tract Infections.

BACKGROUND: Postvoid residual (PVR) urine is common in older patients. It is often managed with intermittent catheterization because of its assumed association with the risk of urinary tract infection (UTI). This association is uncertain, however, whereas the association between intermittent catheterization and increased risk of UTI is clear. Evidence further suggests that the threshold for intermittent catheterization in patients awaiting spontaneous urination after surgery can be raised without increasing the risk of UTI or voiding difficulties. PURPOSE: The purpose of this quality improvem

Evaluation of Urinary Catheterization Competency and Self-Efficacy in Nursing Students Using the Flipped Classroom Approach.

Urinary catheterization is a complex procedure. Traditional teaching in classroom, such as using a simulator, is not enough to guarantee the necessary learning outcomes. It is therefore necessary to implement new active learning approaches such as the flipped classroom. The aim of this study was to examine the effect of the use of the flipped classroom on the level of self-efficacy and the assessment of clinical competencies in nursing students performing the urinary catheterization procedure. A quasi-experimental study of a single group with pre-/post-testing measures. A convenience sample of

The Effect of Bladder Catheterization Technique on Postoperative Urinary Tract Infections After Primary Total Hip Arthroplasty.

BACKGROUND: Urinary bladder catheters are potential sources of infection after total hip arthroplasty (THA). Therefore, the goal of this study was to determine if intermittent catheterization provides a decreased risk of postoperative urinary tract infections (UTIs) compared with indwelling catheterization in THA patients. METHODS: Patients undergoing THA at 15 hospitals within a large health system were prospectively collected between 2017 and 2019 and then stratified based on catheterization technique: no-catheter; indwelling catheter-only; intermittent catheter-only; and both intermittent

Prognostic significance of early urinary catheterization after acute stroke: Secondary analyses of the international HeadPoST trial.

BACKGROUND: An indwelling urinary catheter (IUC) is often inserted to manage bladder dysfunction, but its impact on prognosis is uncertain. We aimed to determine the association of IUC use on clinical outcomes after acute stroke in the international, multi-center, cluster crossover, Head Positioning in Acute Stroke Trial (HeadPoST). METHODS: Data were analyzed on HeadPoST participants (n = 11,093) randomly allocated to the lying-flat or sitting-up head position. Binomial, logistic regression, hierarchical mixed models were used to determine associations of early insertion of IUC within seven
