Indocyanine green (ICG) fluorescence-guided laparoscopic adrenalectomy.

OBJECTIVE: Laparoscopic adrenalectomy has become the standard of care for many adrenal tumors. However, the success of the operation hinges on identifying the adrenal vein and complete tumor resection. We demonstrate the use of a commercially available near infrared fluorescent imaging system to clearly delineate the vascular anatomy of adrenal neoplasms and enhance the border between tumor and normal tissue. We hypothesize that this will increase the safety of laparoscopic adrenalectomy. MATERIALS AND METHODS: We performed laparoscopic adrenalectomy utilizing indocyanine green (ICG) and a sp

Imaging of groin masses: inguinal anatomy and pathologic conditions revisited.

The groin region is subdivided into two distinct anatomic areas: the inguinal canal and the femoral triangle. A series of cross-sectional imaging cases illustrate that a good understanding of the local anatomic characteristics of the groin allows confident characterization of groin pathologic conditions. Such conditions can be classified into five major groups: congenital abnormalities, noncongenital hernias, vascular conditions, infectious or inflammatory processes, and neoplasms. Congenital entities include hernias, cysts, undescended testis, and retractile testes. Ultrasound (US) is useful

Mesenchymal tumors of the uterus. V. Intravenous leiomyomatosis. A clinical and pathologic study of 14 cases.

Fourteen examples of intravenous leiomyomatosis in the FIP files were studied. Growth beyond the uterus occurred in 10 of the 14 examples, largely in vessels in the broad ligament and in uterine and iliac veins. Metastasis to the heart occurred in 1 example. The study provides evidence that is consistent with both theories of origin of intravenous leiomyomatosis; namely, that it may arise either from the wall of veins within the myometrium or be a result of unusually extensive vascular invasion from a leiomyoma of the myometrium. Intravenous leiomyomatosis should be distinguished from a leiomy

Preoperative transarterial embolization of spinal column neoplasms.

PURPOSE: To determine the safety and value of vertebral column embolization before surgical resection of vascular neoplastic disease. PATIENTS AND METHODS: Thirty preoperative embolization procedures were performed in 20 patients with vascular neoplasms of the vertebral column (C-2 to sacrum). Fourteen patients had metastatic renal cell carcinoma. Distal embolic agents were used in 27 cases and were coupled with more proximal agents in six. Gelatin pledgets alone were used in three cases. Twenty-six of the 27 surgical procedures involved partial to complete tumor resection. RESULTS: Seventy-

Pancreatic islet-cell neoplasia, with secretion of a parathormone-like substance and hypercalcemia.

In a 47-year-old woman with a pancreatic mass associated with hypercalcemia and mental confusion, medical measures failed to restore her serum calcium level to normal. To do so, radical resection of a locally invasive vascular neoplasm arising from the body and tail of the pancreas was necessary. The neoplasm was a pancreatic islet-cell tumour. Serum parathormone assays demonstrated abnormally high secretion of a parathormone-like substance. Ectopic secretion of such substances from islet-cell tumours should be considered in association with refractory metabolic disturbances. In view of the of

Spindle cell tumors of the mediastinum.

Spindle cell tumors originating in the mediastinum are extremely rare. Due to the profusion of structures and organs located in the mediastinum, a wide variety of spindle cell neoplastic processes can develop in this location. These include various different tumor types including epithelial, vascular, lipomatous, fibroblastic and neural tumors among others. Many of these different tumor types are associated with specific immunohistochemical and molecular genetic profiles that help differentiate them from each other. Although spindle cell morphology has traditionally been associated with mesenc

Nasal smooth muscle tumor of uncertain malignant potential: A case report.

Smooth muscle tumors that cannot be histologically classified as leiomyomas or leiomyosarcomas are defined as smooth muscle tumors of uncertain malignant potential. The location of these tumors in the nose is very rare, and the appropriate surgical extent to manage these neoplasms has not been adequately defined. Here we describe the case of a 16-year-old female adolescent who consulted due to a vascular-like tumor in the right nasal cavity who was successfully treated with intranasal surgery. The histological diagnosis was smooth muscle tumor of uncertain malignant potential. Given that these

Clinical response to pazopanib in a patient with endolymphatic sac tumor not associated with von Hippel-Lindau syndrome.

Endolymphatic sac tumors (ELSTs) are rare, locally invasive, vascular tumors of the temporal bone. These lesions are associated with von Hippel-Lindau syndrome but may arise sporadically. Early surgical intervention is recommended to prevent permanent neurologic deficits; however, many ELSTs are unresectable or are subtotally resected due to neurovascular compromise. Chemotherapeutic salvage therapy in trials of neoplasms of associated syndromes has targeted angiogenesis with variable response. We present the case of a sporadic ELST, previously minimally responsive to bevacizumab, treated with

Description of a concurrent thyroid haemangioma and a follicular adenoma.

UNLABELLED: Thyroid gland vascular proliferations pose a challenge to pathologists, radiologists and surgeons. This is illustrated in the case of a 36-year-old woman who underwent a diagnostic thyroid lobectomy for a suspected follicular neoplasm according to cytology. Histological diagnosis confirmed a follicular adenoma as well as a cavernous haemangioma or venous malformation. CONTRIBUTION: Thyroid vascular lesions are rare. Evolving nomenclature and application of the International Society for the Study of Vascular Anomalies classification are discussed. Pertinent radiological clues are h

Epithelioid angiomatosis: a distinct vascular disorder in patients with the acquired immunodeficiency syndrome or AIDS-related complex.

Unusual cutaneous vascular neoplasms distinct from Kaposi's sarcoma were observed in five patients with the acquired immunodeficiency syndrome (AIDS) or human immunodeficiency virus (HIV)-1 infection. The cutaneous lesions were solitary or multiple papules and nodules. In some patients the lesions also affected internal organs. Histologically the neoplasms were composed of proliferating blood vessels and cells with epithelioid features. Immunoperoxidase studies of one lesion showed that the cells expressed both factor VIII antigen, a maker for endothelial cells, and alpha 1-anti-chymotrypsin,

Vascular retardation in dormant growth-stunted malignant melanomas.

Progression and metastatic spread of primary cutaneous melanoma (PCM) is largely predicted by the thickness of the primary tumor. However, the accretive or proliferative pattern of growth of PCM is another aspect that might affect the prognosis. We retrieved from our histopathological files 11 superficial spreading PCM which had been documented to show an almost stable size for at least 3 years before excision. The area of the PCM at the skin surface had been measured by planimetry on the excision specimens. Histological sections were used to measure the maximum thickness of the neoplasms. A P

Inferior vena cava--right atrial tumor thrombus in malignant pelvic bone tumors in children.

Two patients with IVC and right atrial tumor thrombus associated with malignant bone tumors are described. Other tumors such as Wilms', renal cell CA, testicular adrenal and hepatic neoplasms have also been reported to invade the IVC. Ultrasound, CT and MRI are helpful in evaluating the presence of vascular thrombosis as well as its longitudinal extent, and intracardiac involvement.

An unusual intracerebral connective tissue mass.

A sharply circumscribed, vascular, connective tissue mass which replaced the cortex of several adjoining cerebral gyri is described. The histology of the lesion differs from that of previously reported central nervous system maldevelopments and neoplasms. However, there are a number of similarities to the pathology found in late phases of syphilitic "colloid" or "hyaline" degeneration of the brain.

Neurocytoma-like neoplasm of the thoracic spine in a 15-month-old child presenting with diffuse leptomeningeal dissemination and communicating hydrocephalus. Case report.

This unusual mixed glioneuronal neoplasm of the spine resembling central neurocytoma is only the second reported example of a neoplasm of this type involving the spinal cord and is, seemingly, the first to present with diffuse leptomeningeal dissemination and communicating hydrocephalus. This 15-month-old boy presented with somnolence, bilateral sixth nerve palsy, and lower-extremity weakness and was found to harbor a primary neoplasm within the thoracic spine and associated syrinx, widespread leptomeningeal dissemination, and communicating hydrocephalus. The patient underwent cerebrospinal fl

Primary Kaposiform Hemangioendothelioma of the Humerus: A Case Report.

Kaposiform hemangioendothelioma (KHE) is a neoplasm originating mainly from vessels and has a mild proclivity for malignancy. This neoplasm mainly involves somatic soft tissue and retroperitoneum. Histological findings include a nodular arrangement of oval-to-spindle cells containing pale cytoplasm. Vascular spaces are in the form of slit-like channels in which red blood cells are evident. Here, we report the case of a two-year-old male who presented with Erb's palsy and a mass lesion in the right humerus. Tissue biopsy features were compatible with KHE.

Intravascular Papillary Endothelial Hyperplasia of the Jejunum as a Cause of Obscure-Overt Gastrointestinal Bleeding.

Intravascular papillary endothelial hyperplasia (IPEH), or Masson's tumor, is a rare, benign vascular lesion characterized by reactive endothelial proliferation with venous stasis and thrombus. Lesions typically arise in vascular regions of the head and neck or extremities, but involvement of other organ systems has been reported. Clinically, IPEH mimics other benign lesions (hemangiomas) and malignant neoplasms (angiosarcoma) depending on the affected site. Diagnosis is essential because local excision of certain IPEH subtypes is curative. IPEH in the gastrointestinal tract is rare with few r

Primary vascular tumors of the lungs: a review.

Unlike their counterparts in other organ systems, primary vascular neoplasms of the lung are rare. Most of these lesions have only been reported as isolated case studies or small series. When dealing with malignant lesions, metastasis from extrapulmonary sites will have to be excluded before a primary location in the lungs can be confirmed. In this review, the clinicopathologic, immunophenotypical, ultrastructural, and molecular biologic characteristics of primary vascular tumors of the lungs are discussed. The tumoral conditions that will be addressed include hemangioma, lymphangioma, epithel

[Spontaneous hemothorax in a neurofibromatosis type I patient - a case report].

Spontaneous hemothorax is a rare and potentially lethal neurofibromatosis' complication. Several pathological mechanisms may explain the associated vasculopathy: a) direct vascular invasion from adjacent tumors such as Shwannoma, neurofibroma or neurofibrosarcoma; b) vascular dysplasia with stenotic or aneurysm formation. Other mechanisms involved may include pleuropulmonar pathologies (necrotizing infections, pulmonary embolism, endometriosis, neoplasms) and blood dyscrasias. The authors describe a case of a 33 years old female, who went to the ER Service complaining with right persistent hem

The nasal arteries.

Thorough knowledge of the normal gross and radiographic anatomy of the nasal fossa is a prerequisite for correct interpretation of external carotid angiograms in patients with lesions in or adjacent to the nasal cavity. This report describes in detail the normal gross and angiographic vascular anatomy of the nasal fossa. The appearance of the sphenopalatine artery and nasal branches of the ethmoidal arteries in a variety of lesions that affect the nasal cavity, nasopharynx, and adjacent structures is also discussed. Typical changes in acute rhinitis, sinusitis, sphenopalatine neuralgia, vascul
