Indwelling and Retrieval Complications of Denali and Celect Infrarenal Vena Cava Filters.

PURPOSE: To compare indwelling and retrieval complications of Denali and Celect filters placed in the infrarenal inferior vena cava (IVC). MATERIALS AND METHODS: A retrospective study was conducted over 2 years at a single institution in which 171 Denali and 162 Celect filters were placed in 333 patients with a mean age of 62.3 years ± 15.7 (161 men; 48.3%). Filter indications included venous thromboembolic disease (n = 320; 96.1%) and surgical prophylaxis (n = 13; 3.9%). A jugular approach was used to place 303 filters (91.0%). Computed tomography (CT) follow-up, complications, and retrieval

Medical literature, vena cava filters and evidence of efficacy. A descriptive review.

Up to 15% of all patients with venous thromboembolism (VTE) receive an inferior vena cava filter, and prophylactic placements are increasing. To determine whether current use of filters is based on robust evidence, a global review of the recent (2001-2012) literature on filters was undertaken. The MEDLINE database was searched for articles related to filters appearing during the period 2001-2012, updating a prior search of literature from 1975-2001. All retrieved articles were analysed, classified into predetermined categories and compared to the prior analysis; randomised and large (>100 pati

Removal of retrievable vena cava filters in routine practice: a multicenter study.

UNLABELLED: Vena cava filters (VCFs) are used to prevent pulmonary embolism when anticoagulation is contraindicated or in the event of progression of thrombosis despite adequate anticoagulation. Retrievable VCFs provide a potential advantage over permanent VCFs, but the appropriateness of their use and the frequency with which they are removed is not well established. OBJECTIVES: Document the indications for insertion of retrievable VCFs, filter removal in hospital practice. METHODS: Observational study conducted in three academic medical centers. Consecutive patients undergoing retrievable

Design optimization of vena cava filters: an application to dual filtration devices.

Pulmonary embolism (PE) is a significant medical problem that results in over 300,000 fatalities per year. A common preventative treatment for PE is the insertion of a metallic filter into the inferior vena cava that traps thrombi before they reach the lungs. The goal of this work is to use methods of mathematical modeling and design optimization to determine the configuration of trapped thrombi that minimizes the hemodynamic disruption. The resulting configuration has implications for constructing an optimally designed vena cava filter. Computational fluid dynamics is coupled with a nonlinear

Retrievable inferior vena cava filters in high-risk patients undergoing bariatric surgery.

BACKGROUND: Placement of retrievable inferior vena cava filters (rIVCF) may be beneficial in high-risk morbidly obese patients undergoing bariatric procedures. Patients with a previous history of venous thromboembolism (VTE) are at high risk for postoperative deep venous thrombosis (DVT) and pulmonary embolism (PE). METHODS: A prospective database of bariatric surgery patients was studied from April 2003 to May 2007. A total of 791 patients underwent bariatric procedures, of which 30 (4%) had a previous history of VTE. These patients underwent preoperative venous duplex and concurrent placeme

Insertion of inferior vena cava filters in patients with the antiphospholipid syndrome.

BACKGROUND: The antiphospholipid syndrome (APS) is a disease with a high prevalence of thromboembolic events, especially pulmonary emboli (PE). These events may recur despite anticoagulation therapy. In such cases, placement of an inferior vena cava (IVC) filter may be considered to prevent propagation of a distal thrombus toward the pulmonary vessels. It is unclear whether the placement of such a filter is beneficial in patients with APS. OBJECTIVE: Retrospective evaluation of the value of IVC filter placement as prophylaxis against recurrent pulmonary emboli in patients with medically treat

Twenty-one-year trends in the use of inferior vena cava filters.

BACKGROUND: Improved inferior vena cava (IVC) filters have led to liberalization of the indications for insertion. Increased use, however, has been followed with a potential for unwarranted insertion. There are only sparse data on trends in the use of IVC filters in patients with pulmonary embolism (PE), patients with deep venous thrombosis (DVT) alone, and patients at high risk. We analyzed the National Hospital Discharge Survey (NHDS) database for such trends. METHODS: We used data from the NHDS, which is based on a national probability sample of discharges from short-stay nonfederal hospit

The Vena Tech filter: evaluation of a new inferior vena cava interruption device.

Expanded indications for caval interruption and earlier diagnosis of deep venous thrombosis have resulted in increased use of transvenous caval interruption devices and have intensified the search for the ideal caval filter. The Vena Tech vena cava filter is a percutaneous, transvenous caval interruption device which was recently introduced in the United States. We reviewed our experience with this filter. During the period of September 1989 to July 1990, 41 patients underwent placement of the Vena Tech filter. Indications for filter insertion included deep venous thrombosis with a contraindic

Double vena cava filter insertion in congenital duplicated inferior vena cava: a case report and literature review.

The complex embryogenesis of the inferior vena cava (IVC) may result in several anomalies, often presenting as an incidental radiological finding. In addition to the differential diagnosis with pathological lesions, recognizing IVC defects is crucial for invasive procedures. This report describes a patient with a right femoral vein thrombosis who could not be given anticoagulant therapy due to a concomitant acute cerebral hemorrhage. He was found to have an asymptomatic duplicated IVC with interiliac communication. A filter had to be inserted in each vena cava to prevent pulmonary embolism. A

Superior vena cava perforation and cardiac tamponade after filter placement in the superior vena cava--a case report.

The purpose of this paper is to report the complication of perforation of the superior vena cava (SVC) leading to cardiac tamponade after the insertion of a Trapease IVC filter in the SVC position. A 29-year-old man was hit by motor vehicle and sustained numerous injuries including a left skull fracture, intracerebral hemorrhage, and left open tibial shaft fracture. During his hospitalization, he developed an extensive symptomatic right upper extremity deep venous thrombosis involving the brachial, axillary, subclavian, internal jugular, and brachiocephalic veins. Owing to an intracerebral ble

Use of a temporary vena cava filter in a woman with septic abortion and inferior vena cava thrombosis. A case report.

BACKGROUND: Ovarian vein and inferior vena cava (IVC) thrombosis are rare but life-threatening complications of severe obstetric infections. IVC filters have been used to reduce the risk of pulmonary embolus in patients who have recurrent thrombotic and embolic events or underlying risk factors. More recently, retrievable IVC filters have been used in patients in whom the risk is transient. CASE: At 18 weeks' gestation a multigravida with a fetus with anencephaly became septic after Laminaria placement and rupture of the membranes. She developed ovarian vein and IVC thrombosis and was treated

[Interventional therapy of inferior vena cava thrombosis in pregnancy--use of a new kind of temporary vena cava filter].

Several changes occur during pregnancy that cause hypercoagulability such as venous stasis, increased levels of clotting factors, and decreased fibrinolytic activity. Nearly half of all maternal mortality can be attributed to thromboembolic disease. Recurrent embolism from venous thrombosis in pregnancy constitutes a major diagnostic and management problem. Treatment of deep venous thrombosis by anticoagulation alone may not be sufficient to prevent fatal pulmonary embolism. Because pulmonary embolism is a potential preventable and treatable condition, early and accurate diagnosis and treatmen

Non-retrieved inferior vena cava filters: causes and long-term follow-up.

BACKGROUND: Temporary inferior vena cava (IVC) filters are not always retrieved. Information about long-term outcomes of patients with indwelling filters is scarce. Aims of our study were to assess reasons that preclude retrieval of temporary IVC filters and long-term outcomes and causes of death in patients with indwelling filters. MATERIALS AND METHODS: Retrospective observational study including all consecutive patients undergoing IVC filter insertion from January 2009 through December 2018. Patients with permanent filters and those with temporary filters not retrieved were followed from i

Indications, complications and outcomes of inferior vena cava filters: A retrospective study.

INTRODUCTION: Inferior vena cava filters are used to prevent embolization of a lower extremity deep vein thrombosis when the risk of pulmonary embolism is thought to be high. However, evidence is lacking for their benefit and guidelines differ on the recommended indications for filter insertion. The study aim was to determine the reasons for inferior vena cava filter placement and subsequent complication rate. MATERIALS AND METHODS: A retrospective cohort of patients receiving inferior vena cava filters in Edmonton, Alberta, Canada from 2007 to 2011. Main outcome was the indication of inferio

Considerations for Imaging the Inferior Vena Cava (IVC) with/without IVC Filters.

Deep venous thrombosis (DVT), thrombosis of the inferior vena cava, and pulmonary embolism (PE) constitute a continuum that includes venous thromboembolic (VTE) disease. VTE is the third most common cardiovascular disorder that affects all races, ethnicities, gender, and ages. VTE predominantly affects the elderly population, exponentially increasing in incidence with increasing age. Venous thromboembolism is not only a singular event but a chronic disease and has been found to have a rate of recurrence approaching 40% among all patients after 10 years. Whether symptomatic or asymptomatic, onc

Perforated inferior vena cava filters as the cause of unclear abdominal pain.

Inferior vena cava filters are considered a valuable therapeutic option in patients with deep vein thrombosis, subsequent pulmonary emboli, and contraindication for anticoagulation. However, these filters bear the risk of rare but serious complications (e.g., symptomatic caval perforation). We report our experiences with retrievable vena cava filters by means of an actual case and review the recent literature with special regard to filter-dependent delayed symptomatic vena cava perforations. Here, an inferior vena cava filter could be identified as the source of a patient's abdominal pain; aft

Interventional approaches in VTE treatment (vena cava filters, catheter-guided thrombolysis, thrombosuction).

Venous thromboembolism (VTE), including deep vein thrombosis (DVT) and pulmonary embolism (PE), is the third most common cardiovascular pathology after coronary disease and cerebrovascular diseases and is responsible for significant morbidity and mortality in the general population. Full-dose anticoagulation is the standard therapy for VTE, both the acute phase and the prolonged treatment. The latest guidelines of the American College of Chest Physicians recommend treatment with a full-dose of unfractionated heparin (UFH), low-molecular-weight-heparin (LMWH), fondaparinux, vitamin K antagonist
