Endoscopic third ventriculostomy in post-tubercular meningitic hydrocephalus: a preliminary report.

Hydrocephalus is a common sequel of tubercular meningitis. Endoscopic third ventriculostomy (ETV) was performed in thirty-five patients. According to the duration of illness, six patients were in the early (less than 6 weeks), nineteen were in the intermediate (6 weeks to 6 months) and ten patients were in the late phase (more than 6 months) of tubercular meningitis (TBM). Six patients were in stage I, seven patients in stage II and twenty-two patients were in stage III. The overall success rate of ETV in TBM was 77 %. Sixty percent had early and seventeen percent had delayed recovery. Obstruc

Spontaneous regression of a symptomatic pineal cyst after endoscopic third-ventriculostomy.

With the advent of modern diagnostic tools for neuroimaging, the incidental detection of pineal cysts in asymptomatic subjects has increased. Only rarely do pineal cysts present with the clinical signs and symptoms of increased intracranial pressure or with neurological deficits in relation to compression and distortion of the adjacent nervous structures and cerebrospinal fluid pathways. While asymptomatic cysts are considered to be normal variants for which no further investigations are usually required, surgical treatment is suggested for symptomatic cysts, with the goal of eliminating the b

Endoscopic third ventriculostomy: a study of intracranial pressure vs. haemodynamic changes.

Fourteen paediatric patients with obstructive hydrocephalus were studied. They underwent endoscopic third ventriculostomy under general anaesthesia. Their ages ranged from 1 to 144 weeks (mean 34+/-36 weeks) and weight from 2 to 22 kg (mean 10.2+/-5.4 kg). In an attempt to identify the possible mechanisms of the intraoperative haemodynamic changes associated with endoscopic third ventriculostomy, we studied the intracranial pressure measured in the third ventricle versus the haemodynamic changes. The intracranial pressure was measured using a pressure transducer attached at one end to the endo

Spontaneous third ventriculostomy in patients undergoing fetal surgery for myelomeningocele correction.

INTRODUCTION: Spontaneous third ventriculostomy (STV) is characterized by the spontaneous rupture of one of the ventricle walls due to increased pressure in the third ventricle caused by obstructive hydrocephalus. Clinically, STV results in resolution of signs and symptoms of intracranial hypertension and head circumference stabilization. No spontaneous STV cases in patients with myelomeningocele have been reported in the literature. The objective of this study was to report three cases of STV in patients with type 2 Chiari malformation who underwent intrauterine treatment. CASE PRESENTATION:

Endoscopic Third Ventriculostomy for Obstructive Hydrocephalus Secondary to Delayed Intracerebellar Hematoma.

Nowadays, endoscopic third ventriculostomy (ETV) in neurosurgery has yielded good clinical results in various conditions. Intraventricular endoscopic procedures can be performed in different pathologies and not only in non-communicating hydrocephalus. This is presented accordingly in this clinical case. We present the case of a patient who suffered a blunt traumatic brain injury (TBI) in the occipital region. Upon his arrival at the medical facility, he displayed altered neurological status and showed symptoms of aggressiveness, slurred speech, and gait ataxia. Initial non-contrast brain compu

Intraventricular Pressure in Non-communicating Hydrocephalus Patients Before Endoscopic Third Ventriculostomy.

BACKGROUND: In patients with non-communicating hydrocephalus impairment of cerebral compliance can occur pre- but also intraoperatively. METHODOLOGY: In such patients (n = 6) undergoing endoscopic third ventriculostomy (ETV), the present study aimed to investigate the effect of ETCO2 (e.g 40 mmHg and 60 mmHg) and positive end-expiratory pressure (PEEP) (e.g. 6 cm and 12 cm H2O) on intraventricular pressure (IVP). FINDINGS: Before but not after ETV, hypercapnia in contrast to PEEP increased IVP. BEFORE ETV: (PEEP-6/ ETCO2-40: 2.6 ± 2.4 mmHg) vs. (PEEP-6/ ETCO2-60: 12 ± 6.4 mmHg*); (PEEP-12/

Endoscopic Third Ventriculostomy before Posterior Fossa Tumor Surgery in Adult Patients.

OBJECTIVE: Obstructive hydrocephalus in patients with posterior fossa tumors is frequently seen. Treatment options include immediate tumor removal or prior cerebrospinal fluid (CSF) diversion procedures. The necessity and feasibility of an ETV in these situations has not yet been proven in adult patients. METHODS: We retrospectively reviewed our prospectively maintained database for ETVs before surgery of posterior fossa tumors in adults. The primary focus of data analyses was the question of whether the ETV was suitable to treat the acute situation of hydrocephalus without an increased rate

Subdural hematoma after endoscopic third ventriculostomy: Struggling against the Laplace law.

BACKGROUND: Endoscopic third ventriculostomy (ETV) is an ideal treatment for obstructive hydrocephalus. Although ETV is a safe procedure, several complications related to this procedure have been reported in the literature. We present a rare case of late onset symptomatic bilateral subdural hematoma after an uneventful ETV that proved particularly difficult to solve. CASE DESCRIPTION: A 61-year-old male patient presented in our neurosurgery department three months after ETV (aqueductal stenosis) with progressive headaches and anomic aphasia. The MRI revealed bilateral chronic subdural hematom

Factors affecting surgical outcome of endoscopic third ventriculostomy in congenital hydrocephalus.

Endoscopic third ventriculostomy (ETV) is an accepted modality of treatment for obstructive hydrocephalus, with good results in adult patients. However in the pediatric age group results vary from poor to similar to the adult population. This study evaluates the outcome of ETV in congenital hydrocephalus of both early and delayed presentation, and investigates factors that determine the outcome. Patients with congenital hydrocephalus who underwent ETV between January 2006 and December 2011 were retrospectively analyzed. Any conditions potentially influencing the need for redo surgery (persiste

[Hyponatremia caused by SIADH following endoscopic third ventriculostomy: a case report].

A 25-year-old man complained of disorientation and gait disturbance during the past 2 weeks. The patient had been treated for cerebellar astrocytoma by open surgery thrice, at ages 3, 5, and 11. Ventriculo-peritoneal shunt was performed for postoperative hydrocephalus at the age of 11. Magnetic resonance imaging(MRI)showed enlargement of both lateral ventricles, ballooning of the third ventricle, and obstruction of the aqueduct of Sylvius. The patient was diagnosed with recurrent hydrocephalus due to shunt malfunction, and treated by endoscopic third ventriculostomy(ETV)using a flexible endosc

Role of endoscopic third ventriculostomy at infected cerebrospinal fluid shunt removal.

OBJECT: Cerebrospinal fluid shunt infection is distressing, especially in the pediatric population. Usually, infected CSF shunts are removed, and after temporary external CSF drainage, reinsertion of the CSF shunt is necessary. Unfortunately, it is not rare to encounter CSF reinfection after shunt renewal, and furthermore, the reinserted CSF shunt is at a considerable risk of malfunction. Endoscopic third ventriculostomy (ETV) is a potent option in managing CSF shunt infection, although ETV failure may occur more frequently when it is used to remove an infected shunt. The authors retrospective

Intraoperative bradycardia and postoperative hyperkalemia in patients undergoing endoscopic third ventriculostomy.

During our initial experience with endoscopic third ventriculostomies, we observed intraoperative bradycardia and postoperative hyperkalemia. The present study was carried out to verify the consistency of these initial observations. Intraoperative heart rate (HR) changes and postoperative serum K + changes were studied prospectively in 20 patients of endoscopic third ventriculostomy. Another 6 patients who underwent endoscopic procedures other than ventriculostomy acted as controls. The anaesthetic technique and intraoperative and postoperative fluid regimen were similar in all patients. Serum

Role of preoperative midsagittal magnetic resonance imaging in endoscopic third ventriculostomy.

We measured the distance between the infundibular recess and the tip of the basilar artery (BA) on midsagittal magnetic resonance (MR) images in 217 subjects (mean age, 54.7 years) without intracranial disorders and 8 patients with hydrocephalus treated neuroendoscopically. The mean distance measured in the 217 subjects without intracranial disorders was 10.5 +/- 2.3 mm. There were no significant differences in this distance among age groups. In 28 of these 217 subjects (12.9%), the tip of the BA had shifted anterior to the mamillary bodies on MR images. Twenty-six of these 28 subjects (93%) w

Treatment failure after endoscopic third ventriculostomy with choroid plexus cauterization, endoscopic third ventriculostomy alone, and ventriculoperitoneal shunt in congenital hydrocephalus: a network and time-to-event meta-analysis.

PURPOSE: Congenital hydrocephalus is commonly treated with endoscopic third ventriculostomy (ETV), ETV with choroid plexus cauterization (ETV + CPC), or ventriculoperitoneal shunting (VPS), yet outcomes vary, and treatment failure is frequent. This study examines treatment failure between procedures in patients with congenital hydrocephalus. METHODS: We conducted a database search from inception to February 2025 for head-to-head studies comparing the primary treatments. A network meta-analysis (NMA) was employed to assess failure rates, and pseudo-individual patient data from published Kaplan

Endoscopic Third Ventriculostomy and Cortical Biopsy in Patients With Normal Pressure Hydrocephalus.

Introduction Normal pressure hydrocephalus (NPH) has conventionally been treated by placement of a ventriculoperitoneal shunt. However, it can also be treated with a less invasive technique, an endoscopic third ventriculostomy (ETV). Unfortunately, there is a lack of evidence on the characteristics of NPH patients who are most likely to benefit from ETV. This study seeks to identify if patients at risk of dementia with NPH should be candidates for an ETV. Methodology Thirty-six NPH patients who underwent ETV at two institutions between July 2007 and December 2014 were pre-surgically assessed f
