Safety and feasibility of bilateral lung transplantation with video-assisted thoracic surgery.

BACKGROUND: The clamshell incision remains the most common approach for bilateral lung transplantation because it provides excellent exposure of bilateral pleural cavities and the mediastinum. However, it is associated with significant morbidity, including sternal dehiscence and instability. Video-assisted thoracic surgery (VATS) offers a less invasive alternative. This study aimed to evaluate the feasibility, safety, and early postoperative outcomes of the VATS approach. METHODS: We retrospectively analyzed 136 patients who underwent bilateral lung transplantation with extracorporeal membran

Effect of Dexmedetomidine on Postoperative Analgesia After Video-assisted Thoracoscopic Lobectomy Surgery.

BACKGROUND: Dexmedetomidine is a highly selective α2-adrenergic receptor agonist that provides sedative and analgesic effects and may reduce opioid requirements without significant respiratory depression. However, evidence supporting its use in patients undergoing video-assisted thoracoscopic (VATS) lobectomy remains limited. This study aimed to evaluate the effects of perioperative dexmedetomidine on postoperative analgesic requirements and recovery outcomes in patients receiving combined epidural and general anesthesia. MATERIALS AND METHODS: This prospective, randomized, double-blind study

Effects of preoperative surgeon warm-up in video-assisted thoracoscopic surgery lobectomy.

BACKGROUND: In various surgical specialties, preoperative surgical warm-up has been demonstrated to affect a surgeon's performance and the perioperative outcomes for patients. However, the influence of warm-up activities on video-assisted thoracoscopic surgery lobectomy (VATSL) remains largely unexplored. This study aims to investigate the potential effects of preoperative surgical warm-up on VATSL. METHODS: A cohort of 364 patients diagnosed with lung cancer through pathology and undergoing VATSL at the Thoracic Surgery Department of Xuzhou Medical University from January 2018 to September 2

Paravertebral vs Epidural Anesthesia for Video-assisted Thoracoscopic Surgery: A Randomized Trial.

BACKGROUND: The choice of postoperative pain management for patients who experience moderate to severe acute pain after thoracoscopic surgery is debatable. This study aimed to determine whether paravertebral block (PVB) provides more benefits than thoracic epidural analgesia (TEA) for thoracoscopic surgery. METHODS: From February 2020 to April 2022, patients without chronic pain who were scheduled to undergo thoracoscopic surgery were randomly assigned to the PVB group or the TEA group. The visual analogue scale score was used to measure the degree of pain when the patients were at rest or co

Early Water Seal of Chest Tubes Following Video-Assisted Thoracic Surgery Pleurodesis.

INTRODUCTION: Early water seal following minimally invasive pulmonary lobectomy has been shown to reduce chest tube duration and postoperative length of stay (LOS). We evaluated chest tube duration and postoperative LOS following a standardized chest tube management protocol change (water seal on postoperative day 1) after video-assisted thoracic surgery (VATS) pleurodesis. METHODS: We identified adult patients undergoing VATS pleurodesis from August 2013 to December 2021. The chest tube protocol was changed in January 2017 such that patients were placed to water seal on the morning of postop

Non-intubated video-assisted thoracic surgery for subxiphoid anterior mediastinal tumor resection.

BACKGROUND: Subxiphoid approach for mediastinal tumor resection was reported to provide a better view and less postoperative pain. Non-intubated video-assisted thoracic surgery (NI-VATS) without muscle relaxant would decrease the possibility of postoperative airway collapse for anterior mediastinal mass operation. Herein, we sought to describe the use of NI-VATS through subxiphoid approach for anterior mediastinal tumor resection. METHODS: In this retrospective cohort study, patients that underwent subxiphoid VATS resection for anterior mediastinal tumor between December 2015 and September 20

Complete Video-assisted Thoracoscopic Surgery for Rib Fractures: Series of 35 Cases.

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) for internal fixation of rib fractures is a promising approach for treating rib fractures and flail chest. Currently the standard practice is to make 1 or several incisions on the chest wall, which will inevitably aggravate the original trauma. METHODS: We retrospectively analyzed the data of patients with rib fractures who were treated with memory alloy for internal fixation by complete VATS using a thoracoscopic transthoracic memory alloy rib coaptation board and an implantation tool through the clip applier method or the puncture, tra

Sleeve resection uniportal video assisted thoracoscopic surgery, feasible and reproducible: case report.

Sleeve resection is used in central lung cancer and the objective is to preserve normal lung tissues. Uniportal VATS approach is a complex and challenging new technique that offers the benefits of minimal invasion, without increasing the risks. We presented a 41-year-old male patient, with history of cough with hemoptysis due to an exophytic lesion of the right main bronchus. Diagnosis of neuroendocrine tumor type 1 (carcinoid tumor) was confirmed with bronchoscopy. Complete circumferential resection was performed, with terminal-terminal anastomosis and reconstruction. Satisfactory postoperati

Lobectomy for lung cancer by video-assisted thoracic surgery in situs inversus.

A 62-year-old man with situs inversus totalis and non-small-cell lung cancer underwent a right lower lobectomy by video-assisted thoracoscopic surgery. Lobectomy in a situs inversus totalis patient can be performed safely using this minimally invasive procedure. Preoperative evaluation of the pulmonary artery and veins by three-dimensional computed tomography and the bronchial tree by bronchoscopy, is essential to avoid unanticipated complications during the procedure.

New noncontact sensor for detecting pulmonary tumors during video-assisted thoracic surgery.

BACKGROUND: Small pulmonary tumors are difficult to localize during video-assisted thoracic surgery (VATS) because of lack of direct tissue contact. However, in partial lung resection, tumor localization is quite important. The aim of this study was to evaluate the safety and feasibility of a new noncontact sensor for detecting pulmonary nodules during VATS using human and porcine models. METHODS: The sensor, based on the principle of phase differences, comprises an air nozzle for producing air pulse jets and an optical fiber sensor to measure phase differences and visualize object stiffness.

Rethinking N2 disease in the era of uniportal video-assisted thoracic surgery.

Third Mediterranean Symposium on Thoracic Surgical Oncology, Catania, Italy, 21-22 April 2016 The primary justification for mediastinal lymphadenectomy is that it provides more complete nodal staging to help select best adjuvant treatments. There is a secondary argument that dissection of nodes might remove otherwise unrecognized nodal disease to increase the chance of cure. They have to be thought through again as patients look for less invasive treatments for their cancers such as videothoracoscopy and stereotactic radiotherapy. Evidence from analysis of Surveillance, Epidemiology and End Re

[Video-assisted thoracic surgery diagnosis of mucinous (colloid) lung adenocarcinoma: Case report].

BACKGROUND: Mucinous colloid lung adenocarcinoma is an uncommon variant of lung carcinomas with similar features to tumours seen in the gastrointestinal tract. To distinguish between these tumours and other mucinous lung tumours, such as mucinous bronchioloalveolar cell carcinomas and metastatic mucinous lesions could be difficult with small biopsy specimens from fine needle aspiration. CLINICAL CASE: The case is described of a 49-year-old female with history of dyspnoea and cough with bloody sputum and weight lose. Chylothorax complicating video-assisted thoracoscopic surgery for non-small cell lung cancer.

BACKGROUND: Chylothorax is an infrequent but well-known complication in lung cancer surgery. Previous published studies on this topic are limited, and thoracotomy has been the main surgical approach for treatment. However, chylothorax after lung cancer surgery performed solely by video-assisted thoracoscopic surgery (VATS) has rarely been investigated. The purpose of this study is to evaluate chylothorax after VATS for lung cancer. METHODS: The records of 776 patients with primary non-small-cell lung cancer (NSCLC) who underwent VATS for pulmonary resection and mediastinal lymph node dissecti

Quantitative sensory testing of persistent pain after video-assisted thoracic surgery lobectomy.

BACKGROUND: Video-assisted thoracic surgery (VATS) lobectomy may potentially reduce the risk of post-thoracotomy pain syndrome (PTPS). However, it may still carry a risk of intraoperative nerve damage and thereby development of PTPS. Thus, our aim was to present a detailed long-term neurophysiological characterization of PTPS after VATS. METHODS: Quantitative sensory testing, using thermal and mechanical stimuli, was performed in 13 PTPS patients and 35 pain-free patients recruited 33 months after VATS lobectomy. RESULTS: When comparing the operated side with the control side in PTPS patient

Bronchial arteriovenous malformation with large aneurysm, resected by video-assisted thoracic surgery.

Bronchial arteriovenous malformation with a large aneurysm appearing as a mass shadow in the right hilum on chest imaging was successfully extirpated thoracoscopically without any lung resection. Preoperative angiography showed tortuous, enlarged right bronchial arteries with an aneurysm connecting to the pulmonary artery, which could not be totally embolized via the transcatheter approach because of the length of the abnormal bronchial artery and possibility of embolotherapy-induced pulmonary infarction. Histological examination revealed defects of the media and internal elastic lamina of the
