Clinical infectious diseases : an official publication of the Infectious Diseases Society of America2009eng
BACKGROUND: The diagnosis of acute hepatitis C virus (HCV) infection is imprecise because antibody testing does not differentiate between acute and chronic infection. Although virologic features, such as viral load fluctuations and low levels of viremia, have been noted to be characteristic of acute HCV infection, these parameters have not been used for diagnosis.
METHODS: We validated the use of these novel parameters (ie, viral load fluctuations >1 log and HCV RNA levels <100,000 IU/mL) in a cohort of acute HCV seroconverters. We then applied standard diagnostic criteria for acute HCV infec

Cervical cancer is believed to have a co-factorial etiology in which high-risk human papillomavirus (HPV) infections are considered an essential factor and other elements play an ancillary role. Besides the importance of specific HPV genotypes, other viral cofactors as viral load may influence the progression likelihood. In this study the relationship between HPV 16 viral load with respect to the grade of cervical disease in Honduran women was investigated. A real-time PCR allowing quantification of both HPV 16 genome and beta-globin gene to normalize the measuring HPV 16 load in cervical cell

OBJECTIVE: To determine the (HR-HPV) high risk HPV viral load in squamous intra-epithelial lesions and association with p16INK4a expression.
METHODS: A series of 109 cervical biopsies were studied (57 normal tissue, 26 low grade squamous intra-epithelial lesions [LSIL] and 26 high grade squamous intra-epithelial lesions [HSIL]). Detection of high risk HPV and viral load in cervical cells was made by molecular biology using hybrid capture 2nd generation collected before the biopsy. The p16INK4a was identified by immunohistochemistry using the p16INK4a kit (clone E6H4).
RESULTS: High risk HPV

This study used a modified version of the Behavioral Model for Vulnerable Populations to examine the predisposing, enabling, and need factors associated with detectable viral load (VL). HIV status was measured using saliva and confirmed by blood. Of 797 persons enrolled, 193 were HIV positive and provided VL counts. A hierarchical multivariate logistic regression approach demonstrated that the predisposing factors of homelessness and recent substance abuse, particularly methamphetamine abuse, had a negative association with VL. The negative association of homelessness on VL was weakened with t

OBJECTIVE: Psychosocial factors (e.g., depression, avoidant coping, life stress) have been related to disease progression in HIV. This study examined the relationship between the Big Five Conscientiousness factor and HIV disease progression (CD4 cell and viral load) over 1 year in 119 seropositive participants. The study also examined whether Conscientiousness effects were mediated by adherence, perceived stress, depression, or coping measures.
DESIGN: In a 1-year longitudinal design, participants completed the NEO Five-Factor Inventory Conscientiousness scale (P. T. Costa & R. R. McCrae, 199

The prevalence of oral lesions was assessed in a five-center subset of the Women's Interagency HIV Study (WIHS) and correlated with other features of HIV disease. Oral examinations were performed by dental examiners on 729 women (577 HIV-positive and 152 HIV-negative) during baseline examination. Significant differences between the groups were found for the following oral lesions: pseudomembranous candidiasis, 6.1% and 2.0%, respectively; erythematous candidiasis, 6.41% and 0.7%, respectively; all oral candidiasis, pseudomembranous and/or erythematous, 13.7% and 3.3%, respectively. We have developed a quantitative real-time PCR (TaqMan) assay aimed at measuring the cellular human herpesvirus 8 (HHV-8) DNA load in various clinical samples. Standard curves were obtained by serial dilutions of a control plasmid containing both HHV-8 (ORF73 gene) and the cellular target (human albumin gene). The assay appeared to be very sensitive (100% detection rate for at least 10 copies per well) and specific and was easily reproducible (less than 3% intra-assay variability, 5% interassay variability). This method allowed us to quantify precisely the average HHV-8 copy number per cell in

BACKGROUND: Elevated maternal HIV viral load (VL) increases vertical transmission risk for breastfeeding children. This randomized controlled trial in Johannesburg primarily evaluated whether 3-monthly point-of-care testing, with laboratory-based standard-of-care testing (arm 2), compared with 6-monthly laboratory-based VL testing (arm 1) in postpartum women living with HIV receiving first-line tenofovir-emtricitabine-efavirenz antiretroviral treatment improved VL suppression, factors associated with nonsuppression, and drug resistance in those with virologic failure.

Dried blood spots (DBS) have been proposed as an alternative diagnostic technique for chronic viral hepatitis. The aim of this observational study was to correlate serologic HBV, HCV, and HDV status and reflex the respective viral load testing by PSC-DBS samples from capillary blood vs conventional plasma samples in patients with chronic viral hepatitis. Besides, we apply these tests in a prospective study for chronic viral hepatitis diagnosis in a rural region of sub-Saharan Africa. In total, 124 HBsAg-positive patients, 75 anti-HCV positive, 2 with HBV-HCV coinfection, and 13 anti-HDV positi

Risk factors for heterosexual HIV transmission are not fully understood. In fact, a proportion of people with sexual exposure to HIV remain uninfected despite multiple and continuous intercourse with HIV-infected partners. In this work, we have analyzed those virologic parameters potentially involved in the transmission of HIV through heterosexual contact. Thirty-eight couples with continuous unprotected sexual intercourse were included. HIV transmission occurred in 10 of 38 couples. No differences in clinical characteristics, exposure time, sexual practices, CD4 counts, or polymorphism in CCR

BACKGROUND: Pregnancy is a unique physiological condition with the cellular immune functions compromised at some extents to allow the mature of growing fetus. Whether pregnancy may influence the replication of hepatitis B virus (HBV) is less studied. The present study aimed to investigate the influence of pregnancy on the replication of HBV and expression of viral antigens by comparing the levels of HBV DNA and viral antigens in pregnant and non-pregnant women.
METHODS: A total of 727 HBsAg-positive serum samples, collected from 214 pregnant women and 513 non-pregnant women of childbearing ag

This study characterized the biology of Bovine Papillomavirus (BPV) in cattle from Pernambuco, Brazil, comparing viral load and gene expression between papillomas (warts) and blood. A total of 89 samples (46 for viral load, 43 for gene expression) were analyzed, all previously positive for BPV-1, -2, or -6. After simultaneous DNA/RNA extraction and cDNA synthesis, viral load was quantified via qPCR targeting the L1 gene, while expression of regulatory (E5/E7) and structural (L1) genes was assessed by RT-PCR. BPV-2 exhibited significantly higher viral load in warts (15.20 ± 45.50 log₁₀ copies/µ

INTRODUCTION: Persistent high-risk human papillomavirus (HR-HPV) causes cervical precancerous lesions and cancer. While molecular HPV DNA testing offers superior sensitivity over cytology as a primary screening method, its limited specificity leads to unnecessary follow-up procedures. Therefore, identifying surrogate biomarkers to distinguish transient infections from clinically relevant, persistent ones is essential for improving risk stratification. A comprehensive literature search across PubMed/MEDLINE, Embase, Scopus, and Web of Science databases up to December 2025 identified studies eva

Little is known on the dynamics of human papillomavirus (HPV) viral load during pregnancy and on the impact of viral load on HPV vertical transmission. We described viral loads for several genotypes during pregnancy and analyse its association with vertical transmission. Data were analysed from the HERITAGE study, a cohort of pregnant women recruited between 2010 and 2016 in three centres in Canada. Vaginal samples were collected at the first and third trimesters of pregnancy, placental samples were collected at birth, and conjunctival, oral, pharyngeal, and genital samples were collected in c

This prospective study aims to examine the impact of changes in viral load on the occurrence of cervical lesions and to evaluate viral load as a biomarker for predicting cervical lesions and triaging HPV-positive patients. From September 2022 to August 2023, 1150 women aged 25-60 were enrolled at the Changzhou Maternal and Child Health Hospital. All participants tested positive for HPV and negative for both cytology and pathology. A follow-up was conducted 6 months later to reassess HPV status and perform colposcopy. BMRT was employed to detect various HPV types and their viral loads. The ROC

INTRODUCTION: Viral load monitoring among pregnant and breastfeeding women (PBFW) can reduce vertical transmission by identifying PBFW with unsuppressed viral load and promoting re-suppression. However, the impact of varying degrees of adherence to viral load monitoring guidelines on the prevention of vertical transmission (PVT) is unknown.
METHODS: We developed a microsimulation model of HIV progression and PVT care for PBFW living with recently acquired HIV in Kenya. We used this model to evaluate viral load monitoring in two key maternal populations: newly positive pregnant women who initi
