Assessing efficacy of voice treatments: a guideline.

The proposal of this guideline or basic protocol is an attempt to reach better agreement and uniformity concerning the methodology for functional assessment of pathological voices. The purpose is to allow relevant comparisons with the literature when presenting/publishing the results of voice treatment, e.g. a phonosurgical technique, or a new/improved instrument or procedure for investigating the pathological voice. Meta-analyses of results of voice treatments are generally limited--and even impossible--due to the major diversity in assessing functional outcomes. A minimal, multidimensional s

Psychological approaches in understanding and treating voice disorders: a primer for compassion focused therapy (CFT).

There is increasing evidence for the usefulness of psychological concepts in voice practice and voice therapy. Existing psychological conceptualisations of voice disorders feature principles from cognitive-behavioural therapy, psychodynamic psychotherapy, and personality psychology, to formulate how voice problems might develop and/or become persistent. Voice practitioners are encouraged to apply these principles when working with people with voice disorders to aid rehabilitation and improve psychosocial wellbeing as it relates to voice use. To this end, this tutorial offers a primer for compa

Voice quality changes following phonatory-respiratory effort treatment (LSVT) versus respiratory effort treatment for individuals with Parkinson disease.

Perceptual ratings of hoarseness and breathiness were used to assess the efficacy of two intensive methods for treating dysarthrophonia in individuals with idiopathic Parkinson disease. One method emphasized phonatory-respiratory effort (the Lee Silverman Voice Treatment, LSVT) and the other emphasized respiratory effort alone (RET). Perceptual ratings were performed by two expert listeners based on random order presentation of the patients' pretreatment and posttreatment recordings of the "Rainbow Passage." The listeners were blinded to the patients and their treatment group. Statistically si

Pushing exercise program to correct glottal incompetence.

A voice therapy program using pushing exercises to correct glottal incompetence is described. The program utilizes various types of instrumentation to determine whether or not a given patient is likely to benefit from the treatment. The program also provides feedback of target voices. Three cases with incomplete glottal closure and subsequent vocal dysphonia characterized by an asthenic breathy quality are used to illustrate the program. Details of the program, termination criteria, and problems and precautions learned from treating 29 patients over a 3-year period are presented.

Vocal fatigue and dysphonia in the professional voice user: Bogart-Bacall syndrome.

Over the past 5 years, the authors have treated 67 adult professional voice users with a musculoskeletal tension disorder involving the larynx and supporting structures and leading to vocal dysfunction. Common clinical features in both sexes were muscle tension in the neck, poor control of the breath stream, and an abnormally low-pitched speaking voice. Most of the men sounded like Humphrey Bogart and the women like Lauren Bacall. These cases represent a discrete clinical vocal fatigue syndrome, the treatment of which is patient education and voice therapy.

Effects of Voiced Gargling on the Electrical Activity of Extrinsic Laryngeal Muscles and Vocal Self-assessment.

OBJECTIVE: To verify the immediate effects of voiced gargling on the electrical activity of extrinsic laryngeal muscles and vocal self-assessment. METHODS: A sample of 20 individuals of both sexes, mean age of 31.95 (±11.57) years, were equally divided in two groups according to the presence or absence of vocal complaint. Both groups were evaluated by surface electromyography of the suprahyoid (SH) and infrahyoid (IH) muscle areas during rest, phonation of the sustained vowel [Ɛ] in habitual and strong intensities; phonation of rising and falling glissando; and counting from 1 to 10. Aerodynamic changes as a result of vocal function exercises in elderly men.

OBJECTIVES/HYPOTHESIS: Voice therapy can improve the vocal quality of elderly patients with voice problems, but the changes in vocal aerodynamics associated with physiologic voice therapy are not well documented. The purpose of the present study was to determine the changes in vocal aerodynamics as a result of the management program known as Vocal Function Exercises (VFEs). STUDY DESIGN: Pre- and post-treatment differences in VFE maximum phonation times (MPT) and measures of vocal aerodynamics were analyzed. METHODS: There were 19 participants, aged 60 to 78 years, who performed VFEs twice a

[Vocalab: a new software for voice evaluation and therapy].

When handling vocal pathologies, speech therapists need tools to visualise voice and speech, in order to correlate personal auditory analysis with accurate data. Vocalab2 is a spectral analysis tool designed for speech therapists, developed in partnership with INSA Toulouse, France. Vocalab is a simple and user-friendly tool with important features to be used during evaluation and therapy. The software is based on a set of phoniatric data and sophisticated real-time signal analysis (Fourier transform, fundamental detection). One of the key tools of Vocalab2 is the real-time spectrum for visual

Persistent dysphonia in two performers affecting the singing and projected speaking voice: a report on a collaborative approach to management.

The projected speaking voice and the singing voice are highly sensitive to external and internal influences, and teachers of spoken voice and singing are in a unique position to identify subtle and more serious vocal difficulties in their students. Persistent anomalies may herald early onset of changes in vocal fold structure, neurophysiological control, or emotional stability. Two cases are presented to illustrate the benefits of a collaborative approach to diagnosis and management. The first, a 21-year-old male drama and singing student with an abnormally high speaking voice and falsetto sin

Laryngeal adduction in resonant voice.

The primary question in this study was whether subjects with nodules and subjects with healthy larynges would produce "resonant voice" with a similar laryngeal configuration. A second question regarded whether the electroglottographic closed quotient (EGG CQ) could be used to noninvasively distinguish resonant from other voice types. Twelve adult singers and actors served as subjects, including 6 persons with healthy larynges and 6 persons with nodules. Performers were used as an attempt to maximize token validity and stability. Subjects produced repeated tokens of resonant, pressed, normal, a

Spectral moments of the long-term average spectrum: sensitive indices of voice change after therapy?

Voice clinicians require an objective, reliable, and relatively automatic method to assess voice change after medical, surgical, or behavioral intervention. This measure must be sensitive to a variety of voice qualities and severities, and preferably should reflect voice in continuous speech. The long-term average spectrum (LTAS) is a fast Fourier transform-generated power spectrum whose properties can be compared with a Gaussian bell curve using spectral moments analysis. Four spectral moments describe features of the LTAS: Spectral mean (Moment 1) and standard deviation (Moment 2) represent

Pre and Post-operative Voice Therapy Intervention for Benign Vocal Fold Lesions: A Systematic Review.

UNLABELLED: Benign vocal fold lesions cause dysphonia by preventing vocal fold closure, causing irregular vibration and increasing compensatory muscle tension. Voice therapy delivered in addition to phonosurgery may improve voice and quality of life outcomes but the evidence base is lacking and what constitutes voice therapy for this population is not defined. The purpose of this systematic review is to critically evaluate the evidence for pre and post-operative voice therapy to inform the development of an evidence based intervention.
