Recognition and Treatment of Cognitive Dysfunction in Major Depressive Disorder.

Major Depressive Disorder (MDD) is a prevalent, chronic, disabling, and multidimensional mental disorder. Cognitive dysfunction represents a core diagnostic and symptomatic criterion of MDD, and is a principal determinant of functional non-recovery. Cognitive impairment has been observed to persist despite remission of mood symptoms, suggesting dissociability of mood and cognitive symptoms in MDD. Recurrent impairments in several domains including, but not limited to, executive function, learning and memory, processing speed, and attention and concentration, are associated with poor psychosoci

Pharmacological treatment for generalized anxiety disorder in adults: an update.

INTRODUCTION: Modest response and remission rates for the selective serotonin reuptake inhibitors and the serotonin-norepinephrine reuptake inhibitors, coupled with mounting evidence that the tolerability of the antidepressants (ADs) may have been overstated in the literature, has contributed to changes in prescribing patterns for generalized anxiety disorder (GAD). New interest in the absence of evidence that supports these standard therapies as superior to benzodiazepines stimulated a review of the literature. AREAS COVERED: A literature search was conducted in the MedLine database with sea

[The mediational role of cognitive function on occupational outcomes in persons with major depressive and bipolar disorder].

BACKGROUND: Improving functioning in adults with major depressive disorder (MDD) and bipolar disorder (BD) is a priority therapeutic objective. METHODS: This retrospective post hoc secondary analysis evaluated 108 patients with MDD or BD receiving the antidepressants vortioxetine, ketamine, or infliximab. The analysis aimed to determine if changes in objective or subjective cognitive function mediated the relationship between depression symptom severity and workplace outcomes. Cognitive function was measured by the Perceived Deficits Questionnaire (PDQ-5), the Digit Symbol Substitution Test (

Biased 5-HT1A agonist NLX-101 improves executive function under NMDA receptor hypofunction and preferentially enhances cortical ERK2 phosphorylation.

Cognitive inflexibility and working memory deficits are major contributors to functional impairment in neuropsychiatric disorders and remain insufficiently addressed by current treatments. NLX-101, a preferential postsynaptic 5-HT1A receptor biased agonist, engages cortical serotonergic signaling and ERK1/2-related pathways, offering a mechanistically distinct approach to improving prefrontal cortex-dependent cognition. Here, we examined whether NLX-101 counteracts executive function deficits induced by NMDA receptor blockade in BALB/c mice. Working memory was assessed using the 8-arm radial m

Neural - adipose crosstalk: Shared genetic architecture of visceral fat and neuropsychiatric disorders.

INTRODUCTION: Numerous studies have identified a close association between visceral adipose tissue mass (VAT) and neuropsychiatric disorders (NPDs). Both VAT and NPDs exhibit high heritability, yet their shared genetic architecture remains unclear. OBJECTIVES: We sought to delineate genetic correlations, locus-level pleiotropy, causal effects, tissue/pathway context, and therapeutic hypotheses linking VAT with ten neuropsychiatric traits (nine NPDs plus cognitive function) using large-scale GWASs. METHODS: We conducted genetic correlation, polygenic overlap, colocalization, transcriptome-wid

Cortical thickening in remitters compared to non-remitters with major depressive disorder following 8-week antidepressant treatment.

OBJECTIVE: Little is known about the relationship between antidepressant treatment outcomes and underlying neurobiological mechanisms in patients with major depressive disorder (MDD). In this prospective study, we aimed to investigate how cortical thickness and subcortical volumes differed between remitter and non-remitter patients with MDD. METHODS: Fifty-eight patients with MDD with a score of at least 17 on the 17-item Hamilton Depression Rating Scale and free of medication for at least 2 months and 41 healthy controls underwent structural magnetic resonance imaging. At the baseline, patie

Adolescent obsessive-compulsive disorder presenting with internal dialogue: a case report.

BACKGROUND: Obsessive-compulsive disorder (OCD) in adolescents may present with predominantly internal cognitive symptoms rather than overt rituals. Intrusive thoughts experienced as an inner dialogue may resemble psychotic symptoms, particularly if depressive symptoms and functional impairment are present. Such presentations may be difficult to recognize in clinical practice. CASE PRESENTATION: This case report describes a 16-year-old girl with an eight-month history of school refusal and intrusive thoughts. She experienced repetitive mental reviewing and vivid internal dialogues but recogni

Rap1-Dependent pathways in depression: Genetic, Epigenetic, and neuroinflammatory mechanisms shaping synaptic resilience.

Rap1, a small GTPase integral to synaptic plasticity, has emerged as a pivotal regulator in the pathophysiology of depression, bridging molecular, genetic, and environmental mechanisms. This review synthesizes evidence highlighting Rap1's role in long-term depression (LTD) and dendritic spine remodeling, where isoform-specific interactions (Rap1 vs. Rap2) modulate AMPA receptor trafficking and synaptic resilience. Genetic studies implicate Rap1-associated pathways, such as DOCK2 and DIRAS2, in postpartum depression, while postmortem analyses reveal reduced Rap1 activity in mood-related brain r

Beyond stimulants: a systematic review of randomised controlled trials assessing novel compounds for ADHD.

Introduction: Despite stimulants being highly efficacious in short-term randomized controlled trials (RCTs), not all patients respond or can successfully tolerate them. A number of novel non-stimulant options are currently in the pipeline for the treatment of attention-deficit/hyperactivity disorder (ADHD). Areas covered: The authors conducted a systematic review of RCTs registered in ClinicalTrials.gov in the past 5 years (January 2014 and February 2019), supplemented by searches in PubMed, Web of Science, and drug manufacturer websites to find recent RCTs on novel non-stimulant ADHD medicati

Psychiatric disorders and sexual dysfunction.

Sexual problems are highly prevalent among patients with psychiatric disorders. They may be caused by the psychopathology of the psychiatric disorder but also by its pharmacotherapy. Both positive symptoms (e.g., psychosis, hallucinations) as well as negative symptoms (e.g., anhedonia) of schizophrenia may negatively interfere with interpersonal and sexual relationships. Atypical antipsychotics have fewer sexual side-effects than the classic antipsychotics. Mood disorders may affect libido, sexual arousal, orgasm, and erectile function. With the exception of bupropion, agomelatine, mirtazapine

The Interface between Depression and Alzheimer's Disease. A Comprehensive Approach.

Depression and Alzheimer's disease (AD) are frequent interacting diseases in the elderly with a negative impact on the quality of life of patients and caregivers. Late-life depression may be regarded either as an early symptom of AD or a risk factor for AD, depending on the context. This review was focused on the latest developments in the fields of the neurobiological basis and treatment of depression in AD. We found that some plausible hypotheses are emerging to correlate with depression in AD, such as neuroinflammation and dysimmune regulation. It seems that depression is not related to amy

Exploring depression in Alzheimer's disease: an Italian Delphi Consensus on phenomenology, diagnosis, and management.

BACKGROUND: In Alzheimer's disease (AD), the progressive cognitive impairment is often combined with a variety of neuropsychiatric symptoms, firstly depression. Nevertheless, its diagnosis and management is difficult, since specific diagnostic criteria and guidelines for treatment are still lacking. The aim of this Delphi study is to reach a shared point of view among different Italian specialists on depression in AD. METHODS: An online Delphi survey with 30 questions regarding epidemiology, diagnosis, clinical features, and treatment of depression in AD was administered anonymously to a pane

Whole blood and oral fluid microsampling for the monitoring of patients under treatment with antidepressant drugs.

Patients suffering from major depression and related pathologies (feeding and eating disorders, obsessive-compulsive disorder, post-traumatic stress disorder, anxiety disorders, etc.) are usually treated with antidepressant agents belonging to several pharmacological and chemical classes; the most recent of these agents are collectively known as "new-generation antidepressants". In these patients, therapeutic drug monitoring (TDM) with the determination of drug and metabolite blood levels is one of the most useful procedures to optimise and personalise the treatment, enhancing both effectivene

Update Psychopharmacotherapy.

Despite unmet meeds regarding efficacy, tolerability and time of onset and the high importance of mental disorders very few new psychotropics have been introduced in Germany recently. Lurasidon and the new multimodal antidepressant vortioxetine demonstrating clinical efficacy in the improvement of cognition have been withdrawn from the German market due to economic reasons based on an official committee judgement „missing additional benefit“. Among new substances introduced are the selective opioid modulator nalmefene for reduction of alcohol consumption, the selective alpha-2-receptor agonist

Concept article: Antidepressant-induced destabilization in bipolar illness mediated by serotonin 3 receptor (5HT3).

OBJECTIVES: Antidepressants used by patients with bipolar disorder have been associated with destabilization with an increase in mania, depression, and cycling. The most commonly proposed mechanism, that antidepressants 'overshoot' their antidepressant effect to create a manic or mixed state, is unlikely since antidepressants have actually been found to be ineffective in treating bipolar depression. Beginning with known bipolar-specific pathophysiologic abnormalities provides the greatest likelihood of insight. METHODS: PubMed was queried with 'bipolar', 'sodium', 'intracellular sodium', 'ser

A Systematic Review of Antidepressants and Psychotherapy Commonly Used in the Treatment of Late Life Depression for Their Effects on Cognition.

Cognitive dysfunction is common in late life depression (LLD) and is a major risk factor for dementia. Recent studies show limited improvement in cognition with commonly employed treatments for LLD, contradicting the notion that cognition "returns to normal" with treatment. However, findings differ with the treatments used. The aim of this study is to perform a systematic review of studies of antidepressants and psychotherapies commonly employed in LLD to determine their effects on cognition, particularly processing speed, memory, and executive function. We searched for trials of acute phase t

Gastrointestinal side effects associated with antidepressant treatments in patients with major depressive disorder: A systematic review and meta-analysis.

Gastrointestinal side effects (SEs) are frequently observed in patients with major depressive disorder (MDD) while taking antidepressants and may lead to treatment discontinuation. The aim of this meta-analysis is to provide quantitative measures on short-term rates of gastrointestinal SEs in MDD patients treated with second-generation antidepressants. An electronic search of the literature was conducted by using MEDLINE, ISI Web of Science - Web of Science Core Collection, and Cochrane Library databases. Eligible studies had to focus on the use of at least one of 15 antidepressants commonly u

Effectiveness of serotonergic drugs in the management of problem behaviors in patients with neurodevelopmental disorders.

Neurodevelopmental disorders often result in disabilities associated with auto- and / or hetero-aggressive behaviors, that can be defined as "problem behaviors" (Lacy 2007). Therapeutic interventions are mainly directed towards the use of neuroleptic drugs or benzodiazepines, to ensure a rapid and significant sedation in most of cases. These pharmacological devices exposes the patient to clinical risks and/or long-term management difficulties. The main problem of the chronic use of benzodiazepines is the development of tolerance and dependence; furthermore benzodiazepine withdrawal or their ab

Cognitive remission: a novel objective for the treatment of major depression?

BACKGROUND: Cognitive dysfunction in major depressive disorder (MDD) encompasses several domains, including but not limited to executive function, verbal memory, and attention. Furthermore, cognitive dysfunction is a frequent residual manifestation in depression and may persist during the remitted phase. Cognitive deficits may also impede functional recovery, including workforce performance, in patients with MDD. The overarching aims of this opinion article are to critically evaluate the effects of available antidepressants as well as novel therapeutic targets on neurocognitive dysfunction in
