Diaphragm dysfunction assessed by ultrasonography: influence on weaning from mechanical ventilation.

OBJECTIVE: To determine the prevalence of diaphragmatic dysfunction diagnosed by M-mode ultrasonography (vertical excursion <10 mm or paradoxic movements) in medical intensive care unit patients and to assess the influence of diaphragmatic dysfunction on weaning outcome. DESIGN: Prospective, observational study. SETTING: Twenty-eight-bed medical intensive care unit in a university-affiliated hospital. PATIENTS: Eighty-eight consecutive patients in the medical intensive care unit who required mechanical ventilation over 48 hrs and met the criteria for a spontaneous breathing trial were asses

Insights in pediatric ventilation: timing of intubation, ventilatory strategies, and weaning.

PURPOSE OF REVIEW: Mechanical ventilation is a common intervention provided by pediatric intensivists. This fact notwithstanding, the management of mechanical ventilation in pediatrics is largely guided by a few pediatric trials along with careful interpretation and application of adult data. RECENT FINDINGS: A low tidal volume, pressure limited approach to mechanical ventilation as established by the Acute Respiratory Distress Syndrome Network investigators, has become the prevailing practice in pediatric intensive care. Studies by these investigators suggest that high positive end expirator

Respiratory mechanics in the patient who is weaning from the ventilator.

Ventilator management of the patient recovering from acute respiratory failure must balance competing objectives. On the one hand, aggressive efforts to promptly discontinue support and remove the artificial airway reduce the risk of ventilator-induced lung injury, nosocomial pneumonia, airway trauma from the endotracheal tube, and unnecessary sedation. On the other hand, overly aggressive, premature discontinuation of ventilatory support or removal of the artificial airway can precipitate ventilatory muscle fatigue, gas-exchange failure, and loss of airway protection. To help clinicians balan

Breastfeeding and early weaning practices in northeast Brazil: a longitudinal study.

OBJECTIVES: To describe breastfeeding practices from 0 to 12 months of age in 4 small towns that are representative of urban northeast Brazil and to identify factors associated with introduction of other milk in the first month of life. METHODS: From January to August 1998, 364 mothers were interviewed at delivery to ascertain antenatal care; delivery room practices; and their intentions regarding breastfeeding, pacifiers, and introduction of water, teas, and other milk. Their perceptions of home support and the advantages of breastfeeding also were assessed. Thereafter, daily information abo

Premature weaning in east Bhutan: only if mother is pregnant again.

The relationship between breast-feeding and subsequent pregnancy in East Bhutan is examined, against the background of local attitudes to family planning. Ninety-eight mothers who had given birth 30-36 months earlier were interviewed. Semisolid supplementary feeding was introduced at a median age of 3 months. Median total duration of breast-feeding was 28 months, and day and night breast-feeding on demand was continued throughout. Median duration of postpartum amenorrhoea was 12 months, and was associated with the timing of the introduction of supplementary foods. There was a significant assoc

Ventilation-perfusion mismatching in chronic obstructive pulmonary disease during ventilator weaning.

Using the multiple inert gas elimination technique, we studied ventilation-perfusion (VA/Q) relationships in eight patients with chronic obstructive pulmonary disease (COPD) during mechanical ventilation (MV) and again during weaning (spontaneous ventilation [SV] through an endotracheal tube) from MV needed for acute respiratory failure. The patients, seven men and one woman with a mean age of 63 +/- 2.8 (SEM) yr (FEV1 33 +/- 5.2% of predicted), required MV for 9.0 +/- 2.4 days prior to the study. The patients were studied at maintenance FIO2 (0.28 to 0.40) while breathing 100% O2, both during

Effect of temporary external stabilization on ventilator weaning after sternal resection.

The role of mechanical fixation of the chest wall in the treatment of flail chest remains controversial. We report a case of flail chest resulting from major sternal resection. The application of a temporary external stabilization device improved pulmonary mechanics by decreasing the respiratory rate from 36/min to 10/min while increasing tidal volume and vital capacity from 140 +/- 85 ml and 195 +/- 90 ml, respectively, to 450 +/- 110 ml and 905 +/- 310 ml, respectively. The improvement with the temporary device facilitated weaning from mechanical ventilation. We recommend consideration of th

Perinatal caffeine treatment: behavioral and biochemical effects in rats before weaning.

Administration of drinking water containing 0, 0.02%, 0.04% and 0.08% of caffeine to female rats throughout gestation and lactation affects several behavioral parameters in the offspring. Righting reflexes, swimming ability development, motor coordination and muscle tone were affected. The activity of these animals, as measured with an open-field test at weaning (i.e., at the end of the treatment), was reduced. The effects observed were dose-dependent. However, for righting reflexes the dose-dependency was direct (the highest dose producing maximal effects), whereas in all the other tests, the

Systemic Effects of a Phage Cocktail on Healthy Weaned Piglets.

Numerous studies have demonstrated that bacteriophages (phages) can effectively treat intestinal bacterial infections. However, research on the impact of phages on overall body health once they enter the intestine is limited. This study utilized weaned piglets as subjects to evaluate the systemic effects of an orally administered phage cocktail on their health. Twelve 21-day-old weaned piglets were divided into control (CON) and phage gavage (Phages) groups. The phage cocktail consisted of five lytic phages, targeting Salmonella enterica serovar Choleraesuis (S. choleraesuis), Enteropathogenic

Environmental detection of Mycoplasma hyopneumoniae in breed-to-wean farms.

There is a need to develop cost-effective and non-invasive approaches to sample large populations to evaluate the disease status of breeding herds. In this study we assessed the detection of the M. hyopneumoniae genetic material in environmental surfaces and air of farrowing rooms, and skin (udder, snout and vagina) of lactating sows at weaning, in farms having different M. hyopneumoniae infection status (negative, positive sub-clinically infected and positive clinically affected). Mycoplasma hyopneumoniae was detected in air, air deposition particles, dam and stall surfaces of the positive cl

Is There a Benefit to Weaning Pavlik Harness Treatment in Infantile DDH?

BACKGROUND: Following successful treatment of developmental hip dysplasia with a Pavlik harness, controversy exists over the benefit of continued harness use for an additional "weaning" period beyond ultrasonographic normalization versus simply terminating treatment. Although practitioners are often dogmatic in their beliefs, there is little literature to support the superiority of 1 protocol over the other. The purpose of this study was to compare the radiographic outcomes of 2 cohorts of infants with developmental hip dysplasia treated with Pavlik harness, 1 with a weaning protocol and 1 wit

Pathogenesis of Escherichia coli O157:H7 in weaned calves.

Cattle are an important reservoir of Shiga toxin-producing Escherichia coli O157:H7 and other enterohemorrhagic E. coli (EHEC) that cause diarrhea, hemorrhagic colitis, and hemorrhagic uremic syndrome in humans. One strategy for reducing human foodborne EHEC infections is to reduce the levels of EHEC in cattle. Bovine O157:H7 infection models will facilitate identification of virulence factors involved in bovine infections. O157:H7 cause severe diarrhea and attaching and effacing (A/E) mucosal lesions in colostrum-deprived neonatal (< 2 h) calves. We hypothesized that O157:H7 also cause A/E le

Neuromuscular disorders associated with failure to wean from the ventilator.

OBJECTIVE: To determine, by retrospective chart analysis, the frequency, type and significance of neuromuscular disorders in patients whose clinical features suggested a neuromuscular cause of failure to wean. BACKGROUND: Failure to wean is a common and difficult problem in critical care units. While a neuromuscular cause may be suspected in some patients, the frequency and type has not been determined utilizing comprehensive electrophysiological studies of limbs and the respiratory system. Such knowledge may aid in patient management and prognosis. METHODS: The clinical setting was a critic

Acamprosate appears to decrease alcohol intake in weaned alcoholics.

Five hundred and sixty-nine alcoholics were included in a double-blind placebo-controlled randomized multicenter study of the effects of Acamprosate (calcium acetylhomotaurinate (CA), 1.3 g/day) on indicators of alcoholic relapse after withdrawal. One hundred and eighty-one patients in the CA group versus 175 in the placebo group completed the three-month study. The major efficacy criterion was plasma gamma-glutamyl transpeptidase (GGT), as an indicator of recent alcohol ingestion. This analysis was completed by criteria concordance analysis on a number of indicators of alcohol intake. Patient

Effect of chlorpromazine on experimental diarrhoea in just-weaned piglets.

In just-weaned piglets (n = 30, 3-4 weeks) diarrhoea (100%) and vomiting (66%) were provoked by inoculation with transmissible gastroenteritis virus and enterotoxigenic E. coli strains (O149: K91: K88ac; LT, STa and STb enterotoxin positive). This combined infection resulted in a mortality of 71% within 7 days. During this period animals revealed a decrease in body weight, in arterial pressure, in leukocyte count, in plasma pH and in plasma lactic acid concentrations, and an increase in heart rate and in total plasma protein concentration. In shocked and expiring piglets an increase in haemato

Weaning of non COPD patients at high-risk of extubation failure assessed by lung ultrasound: the WIN IN WEAN multicentre randomised controlled trial.

BACKGROUND: Postextubation respiratory failure (PRF) frequently complicates weaning from mechanical ventilation and may increase morbidity/mortality. Noninvasive ventilation (NIV) alternating with high-flow nasal oxygen (HFNO) may prevent PRF. METHODS: Ventilated patients without chronic obstructive pulmonary disease (COPD) and at high-risk of PRF defined as a lung ultrasound score (LUS) ≥ 14 assessed during the spontaneous breathing trial, were included in a French-Chinese randomised controlled trial. PRF was defined by 2 among the following signs: SpO2 < 90%; Respiratory rate > 30 /min; hyp
