The effects of sulfur oxides on nasal and lung function in adolescents with extrinsic asthma.

Ten adolescent subjects with extrinsic asthma were exposed during intermittent exercise to filtered air, 0.5 ppm of sulfur dioxide (SO2), or 100 micrograms/m3 of sulfuric acid (H2SO4) on 5 separate days. The purpose of the study was to compare changes in nasal power (the work of nose breathing) with pulmonary functional changes depending on the route of inhalation of the sulfur oxide pollutants, oral inhalation through a rubber mouthpiece or oronasal inhalation via a face mask. Nasal power was measured with a modified skin diving mask equipped with two differential pressure transducers. Statis

Sleep Stage Classification of Pediatric Patients with Sleep-Disordered Breathing using Airflow Signals.

Sleep staging is a critical task for sleep health assessment. Recent work has shown that breathing variability may be characteristic of particular sleep stages. In this work, we train machine learning models on sleep airflow signals from pediatric patients to assess how well sleep stage can be classified from airflow signals, particularly in subjects with sleep-disordered breathing. We perform a 3-class classification task (Wake/NREM/REM) and achieve 66.9% balanced accuracy (BA) when classifying the sleep stage of healthy subjects, 61.4% BA when classifying the sleep stage of subjects with mil

Respiratory cycle-related EEG changes during sleep reflect esophageal pressures.

STUDY OBJECTIVES: Respiratory cycle-related EEG changes (RCREC) have been demonstrated during sleep by digital analysis and hypothesized to represent subtle inspiratory microarousals that may help to explain daytime sleepiness in patients with sleep-disordered breathing. We therefore examined for the first time associations between RCREC and esophageal pressure swings (deltaPes) that reflect work of breathing. DESIGN: Retrospective analysis. SETTING: Academic sleep laboratory. PATIENTS: Forty adults referred for suspected sleep disordered breathing. INTERVENTIONS: Polysomnography with esop

Worker exposure to aromatic volatile organic compounds in dry cleaning stores.

This study evaluated worker exposure to aromatic compounds and perchloroethylene (PERC) in dry cleaning stores relative to four different solvents. For benzene and toluene, there was no significant difference among the indoor air concentrations of the four different solvents. For ethylbenzene, m,p-xylene, and o-xylene, the air concentrations were significantly higher in the dry cleaning stores using Solvent V, YuClean, and Super New Cleaner than in those using a PERC mixture. Breath concentrations measured prior to and immediately after work were not significantly different for benzene and tol

Muscle mass as a factor limiting physical work.

Maximal exercise has been performed by eight men and eight women, using four types of ergometer (2-leg, 1-leg, arm + shoulder, and arm) while breathing room air and while breathing 12% O2. Results have been related to anthropometric estimates of muscle mass in the active limbs. Although significant sex differences of O2 transfer and power output are shown, the sex-specific aerobic performance was roughly proportional to active muscle volume (both when comparing individuals on a given type of ergometer and when comparing average scores of the several types of ergometer). However, the relationsh

Effort of breathing in children receiving high-flow nasal cannula.

OBJECTIVE: High-flow humidified nasal cannula is often used to provide noninvasive respiratory support in children. The effect of high-flow humidified nasal cannula on effort of breathing in children has not been objectively studied, and the mechanism by which respiratory support is provided remains unclear. This study uses an objective measure of effort of breathing (Pressure. Rate Product) to evaluate high-flow humidified nasal cannula in critically ill children. DESIGN: Prospective cohort study. SETTING: Quaternary care free-standing academic children's hospital. PATIENTS: ICU patients y

Hypercapnic respiratory failure during weaning: neuromuscular capacity versus muscle loads.

Patients who fail a weaning trial develop hypercapnia as a result of alveolar hypoventilation, which, in turn, is caused by an imbalance between the respiratory muscle load and capacity. In some patients, especially those with obstructive lung diseases, respiratory muscle performance is impaired as a result of dynamic hyperinflation and paradoxical motion of the rib cage and abdomen. Worsening of pulmonary mechanics causes further embarrassment of the respiratory muscles and can lead to marked alterations of oxygen use by the peripheral tissues. The development of rapid shallow breathing toget

Flow resistance in patients with chronic obstructive pulmonary disease in acute respiratory failure. Effects of flow and volume.

The flow and volume dependence of the total resistance of the respiratory system (Rrs) was investigated in six mechanically ventilated patients with chronic obstructive pulmonary disease (COPD) using a simple, rapid method. Isovolume Rrs-flow (V) relationships obtained at different inflation volumes (range 0.1 to 1 L) fitted (p less than 0.001) the following function: Rrs = a/V + b + cV, where a, b, and c are constants. The term "a/V" in this equation represents the hyperbolic decrease in thoracic tissue resistance with increasing flow; the term "cV" represents the linear increase in airway re

Inspiratory flow pattern in humans.

The theoretical estimation of the mechanical work of breathing during inspiration at rest is based on the common assumption that the inspiratory airflow wave is a sine function of time. Different analytical studies have pointed out that from an energetic point of view a rectangular wave is more economical than a sine wave. Visual inspection of inspiratory flow waves recorded during exercise in humans and various animals suggests that a trend toward a rectangular flow wave may be a possible systematic response of the respiratory system. To test this hypothesis, the harmonic content of inspirato

Oxygen cost of breathing and weaning process in newborn infants.

Newborn infants may have a high oxygen cost of breathing (OCB) at the time of being weaned from mechanical ventilation. We hypothesized that this increase in oxygen consumption (V'O2) could be reduced by using certain weaning ventilatory modes. We designed a study to assess V'O2 during three weaning ventilatory modes: patient triggered ventilation, synchronous intermittent mandatory ventilation (SIMV) and continuous positive airway pressure in 16 newborn infants before being weaned from mechanical ventilation In seven infants whose OCB was high. V'O2 was not significantly different between CV

[Measurement of the reserve function of inspiratory muscle and its clinical significance].

The principles of measuring inspiratory muscle tension-time index (TTim) and the ratio of the works of inspiration over the maximal works of inspiration (Wi/Wi(max)) were investigated and their formulae were deduced, i.e. TTim = (Pi x Ti)/(MIP x Ttot) and Wi/Wi(max) = (Pi x VT)/(MIP x IC). The importance of the inspiratory pressure and the maximal inspiratory pressure (MIP) measured at function residual capacity (FRC) level was emphasized. Both TTim and Wi/Wi(max) were measured in 35 normal subjects and 89 patients with chronic obstructive pulmonary disease (COPD). The results showed that norm

Ability of asthmatics with and without respiratory arrest to detect added resistive loads.

Asthmatics who sometimes experience respiratory arrest during an exacerbation are a particular concern. To date no adequate explanation exists for this phenomenon. Impaired perception of resistive loads is considered a factor in the development of hypercapnic respiratory failure, especially during exacerbation in patients with chronic obstructive pulmonary disease. We analyzed the perception of resistive loads in 10 asthmatics who had suffered two or more respiratory arrests during an acute exacerbation. A group of eight asthmatics (same mean age and basal bronchial obstruction) who had never

Nasopharyngeal airways in Pierre Robin Syndrome.

Nasopharyngeal airways have been assessed in the management of infants with severe Pierre Robin syndrome. In 12 such infants the positioning and subsequent maintenance of these tubes were found to be important in ensuring adequate relief of the airway obstruction. In five infants measurements of lung mechanics demonstrated the benefits of NP tube placement and confirmed the observed improvements in cyanotic episodes, heart failure, electrocardiograms, and arterial gas tensions. In a retrospective survey of 40 infants with PRS, failure to thrive was found to be significantly correlated with the

Acute effects of deep diaphragmatic breathing in COPD patients with chronic respiratory insufficiency.

This study investigated the impact of deep diaphragmatic breathing (DB) on blood gases, breathing pattern, pulmonary mechanics and dyspnoea in severe hypercapnic chronic obstructive pulmonary disease (COPD) patients recovering from an acute exacerbation. Transcutaneous partial pressure of carbon dioxide (Ptc,CO2) and oxygen (Ptc,O2) and arterial oxygen saturation (Sa,O2), were continuously monitored in 25 COPD patients with chronic hypercapnia, during natural breathing and DB. In eight of these patients, breathing pattern and minute ventilation (V'E) were also assessed by means of a respirator

Upper airway mechanics.

This review discusses the pathophysiological aspects of sleep-disordered breathing, with focus on upper airway mechanics in obstructive and central sleep apnoea, Cheyne-Stokes respiration and obesity hypoventilation syndrome. These disorders constitute the end points of a spectrum with distinct yet interrelated mechanisms that lead to substantial pathology, i.e. increased upper airway collapsibility, control of breathing instability, increased work of breathing, disturbed ventilatory system mechanics and neurohormonal changes. Concepts are changing. Although sleep apnoea is considered more and

Periodic breathing at high altitude.

Periodic breathing is often associated with heart disease or stroke, and commonly Cheyne-Stokes breathing has a period of about a minute. Periodic breathing also commonly occurs in healthy subjects at high altitude, and here the periods may be much shorter, of the order of 15-20 s. In this paper we study such periodic breathing using the classical model of Grodins et al. (1967, J. Appl. Physiol. 22, 260-276), together with a prescription for the dependence of ventilation on the blood CO2 concentration, modulated by the reduced oxygen pressure (the 'Oxford fan'). The model focusses on the fast
