The locked-in syndrome: a syndrome looking for a therapy.

The locked-in syndrome (LIS) is a very severe condition caused by a primary vascular or traumatic injury to the brainstem, normally corresponding to a ventral pons lesion due to an obstruction of the basilar artery, and characterized by upper motor neuron quadriplegia, paralysis of lower cranial nerves, bilateral paresis of horizontal gaze and anarthria, and with preserved consciousness. Patients who have suffered this pontine lesion generally have preserved vertical eye movements and movement of the eyelids (blinking), this being their only means of responding to the outside world. Case report on refractory epistaxis successfully treated with a novel taping therapy: A new approach.

RATIONALE: Epistaxis is a very common nose disease that often requires hospital admission. Chemical therapies, surgery, and nasal packing have mostly been used for treatment; however, it remains prevalent, and noninvasive treatments using meridian and acupuncture theory have rarely been studied. Here we report two chronic cases that were successfully treated with novel taping therapy. PATIENT CONCERNS: First, a 62-year-old male patient with right-side epistaxis, which appeared almost every morning and had lasted for6 months visited. Second, a 9-year-old female patient with both-side epistaxis

Novel multimodal sensing and machine learning strategies to classify cognitive workload in laparoscopic surgery.

BACKGROUND: Surgeons can experience elevated cognitive workload (CWL) during surgery due to various factors including operative technicalities and the environmental demands of the operating theatre. This can result in poorer outcomes and have a detrimental effect on surgeon well-being. The objective measurement of CWL provides a potential solution to facilitate classification of workload levels, however results are variable when physiological measures are used in isolation. The aim of this study is to develop and propose a multimodal machine learning (ML) approach to classify CWL levels using

Dry eye sensitizes cool cells to capsaicin-induced changes in activity via TRPV1.

Corneal cool cells are sensitive to the ocular fluid status of the corneal surface and may be responsible for the regulation of basal tear production. Previously, we have shown that dry eye, induced by lacrimal gland excision (LGE) in rats, sensitized corneal cool cells to the transient receptor potential melastatin 8 (TRPM8) agonist menthol and to cool stimulation. In the present study, we examined the effect of dry eye on the sensitivity of cool cells to the transient receptor potential vanilloid 1 (TRPV1) agonist capsaicin. Single-unit recordings in the trigeminal ganglion were performed 7-

The Immunosuppressant Macrolide Tacrolimus Activates Cold-Sensing TRPM8 Channels.

TRPM8 is a polymodal, nonselective cation channel activated by cold temperature and cooling agents that plays a critical role in the detection of environmental cold. We found that TRPM8 is a pharmacological target of tacrolimus (FK506), a macrolide immunosuppressant with several clinical uses, including the treatment of organ rejection following transplants, treatment of atopic dermatitis, and dry eye disease. Tacrolimus is an inhibitor of the phosphatase calcineurin, an action shared with cyclosporine. Tacrolimus activates TRPM8 channels in different species, including humans, and sensitizes

Occipital lobe epilepsy: electroclinical manifestations, electrocorticography, cortical stimulation and outcome in 42 patients treated between 1930 and 1991. Surgery of occipital lobe epilepsy.

Our study documents the clinical and electrographic findings in 42 patients with medically refractory occipital lobe epilepsy, who underwent surgery at the Montreal Neurological Institute between 1930 and 1991, and the evolving manner in which those patients were studied by successive generations of investigators. In more than two-thirds of the patients the clinical manifestations indicated the occipital onset of the seizures. Seventy-three percent experienced visual aurae, of which elementary hallucinations were the most common and 12 also had ictal blindness. Other occipital manifestations i

TalkingEyes: Pluralistic Speech-Driven 3D Eye Gaze Animation.

Although significant progress has been made in the field of speech-driven 3D facial animation recently, the speech-driven animation of an indispensable facial component, eye gaze, has been overlooked by recent research. This is primarily due to the weak correlation between speech and eye gaze, as well as the scarcity of audio-gaze data, making it very challenging to generate 3D eye gaze motion from speech alone. In this paper, we propose a novel data-driven method which can generate diverse 3D eye gaze motions in harmony with the speech. To achieve this, we firstly construct an audio-gaze data

INTRODUCTION: Facial diplegia is an uncommon regional variant of Guillain-Barre syndrome (GBS). It may initially resemble acute cerebrovascular disease when facial weakness appears unilateral or central in pattern. CASE PRESENTATION: A 48-year-old Sri Lankan man with untreated hypertension, obesity, and newly recognized diabetes presented approximately 18 h after onset of slurred speech and persistent left facial weakness. Initial examination suggested a left central facial palsy, although incomplete left eye closure was also present. Non-contrast computed tomography and same-day brain magnet

Corneal Protection in Facial Synkinesis.

BACKGROUND: Corneal protection is the first priority in acute facial paralysis. Orbicularis oculi paralysis causes paralytic lagophthalmos, ectropion, impaired blink, and exposure keratopathy. However, many patients progress to synkinesis, in which muscle reactivation may improve corneal coverage. This study aimed to characterize the longitudinal trajectory of corneal status as patients transition from acute facial paralysis to synkinesis. METHODS: A retrospective cohort study of synkinetic patients evaluated by ophthalmology between 2009 and 2025 was conducted. Included were patients with at

Identifying diagnostic biomarkers in functional motor disorders through multimodal behavioral, neurophysiological, and imaging assessment using explainable machine learning.

BACKGROUND: Functional motor disorders (FMDs) represent a frequent and disabling neurological condition. The lack of reliable diagnostic biomarkers and their heterogeneity might affect diagnosis. We identified multimodal biomarkers distinguishing FMDs from healthy controls (HCs) using machine-learning approaches. METHODS: In this multicenter cross-sectional study, consecutive adults with a clinically established FMDs diagnosis (n = 75, 74.7% female; mean age 44.20 ± 12.92) and age- and sex-matched HCs (n = 75; 58.6% female; 48.42 ± 11.67) were recruited. Treating bacterial keratitis during sleep through placing hydrogel beads with sustained tobramycin/baicalin delivery capability at the palpebra inferior.

Simultaneously restricting bacterial proliferation while mitigating inflammatory reactions and excessive reactive oxygen species (ROS)-mediated corneal damage during sleep represents a crucial therapeutic strategy for managing bacterial keratitis (BK). In this study, we developed micron-scale PCBT hydrogel beads for nocturnal BK treatment by integrating two classical ophthalmic drugs: tobramycin (TOB) as the antibacterial component and baicalin (BA) as the anti-inflammatory/antioxidant agent. The formation of the hydrogel beads was facilitated by the synergistic interplay between: (i) TOB solu

Unseen Consequences: The Expanding Burden of Iatrogenic Dry Eye Disease in Surgical and Cosmetic Practice.

Iatrogenic dry eye disease (DED) has emerged as a significant but overlooked complication in both ophthalmic surgery and aesthetic medicine. Characterized by disruption of tear film homeostasis, ocular surface inflammation, and neurosensory dysfunction, this condition may arise acutely or chronically following medical interventions. The Tear Film and Ocular Surface Society Dry Eye Workshop II and III reports have highlighted iatrogenic DED as a distinct clinical entity, underscoring its multifactorial etiology and the need for tailored risk mitigation strategies. Refractive procedures such as

Species-specific tear proteomic signatures in humans and rabbits: A DIA mass spectrometry comparative study.

PURPOSE: Rabbits exhibit an exceptionally low blink frequency and a markedly prolonged tear breakup time compared with humans. These physiological differences suggest the presence of specialized tear-stabilizing mechanisms that may be reflected in the molecular composition of rabbit tear fluid. This exploratory study compared human and rabbit tear proteomes to identify shared and species-associated differences potentially relevant to tear film stability and ocular surface homeostasis. METHODS: Basal tears were collected from 10 healthy human subjects and 10 healthy New Zealand White rabbits u
