Retrospective Analysis of Bicanalicular Lacrimal Silicone Tube Intubation in Patients with Congenital Nasolacrimal Duct Obstruction: A Long-term Follow-up Study.

Congenital nasolacrimal duct obstruction (CNLDO) is the most common cause of epiphora in the first year of life. In addition to conservative therapy, there are several invasive methods available. The aim of this retrospective study was to conduct a long-term follow-up of bicanalicular lacrimal duct intubation as the primary intervention in a large cohort of patients with CNLDO. The electronic medical records of 487 patients with CNLDO who underwent bicanalicular tube intubation were reviewed. To determine the long-term outcomes, a telephone interview was conducted. A total of 328 eyes of 235 p

The National Institute for Health and Clinical Excellence, and otolaryngology: review of the evidence.

BACKGROUND: The adoption of evidence-based practice is fundamental to good medical care; it ensures that intervention is clinically effective and safe. In a world of limited healthcare resources, consideration of cost-effectiveness must, unfortunately, restrict clinicians' choice. The National Institute for Health and Clinical Excellence has, for over 10 years, developed guidance to achieve a national consensus on best practice. OBJECTIVES: This review describes the Institute's methodology, examines guidance relevant to otolaryngology and presents more recent research to update the evidence.

[Fusobacterium nucleatum isolated from a patient presenting lachrymal canaliculitis].

A 68-year-old woman presented with a painless inflammation of the right superior eyelid that had started several weeks before. The clinical diagnosis concluded in canaliculitis and the solid concretions were surgically extracted from the superior canalicula. The anaerobic bacteria Fusobacterium nucleatum sp. nucleatum was isolated. Signs dramatically regressed two weeks after surgery followed by one course of oral amoxicillin and clavulanic acid associated with topical tobramycin. The clinical signs had disappeared two months later.

An orbital abscess secondary to acute dacryocystitis.

An orbital abscess is an ophthalmic surgical emergency that is typically caused by the spread of bacteria from adjacent structures, such as the sinuses, eyelids, or teeth. Although acute dacryocystitis is commonly associated with preseptal cellulitis, it rarely causes orbital infection. Infection of the lacrimal sac will typically localize in the preseptal space because the lacrimal sac lies anterior to the orbital septum. To the authors' knowledge, this is the first report of an intraconal abscess secondary to acute dacryocystitis. The key points in the surgical management of this entity are

[Early intubation of lacrimal duct in traumatic and operative lower eyelid loss and medial canthal region defects, with simultaneous reconstruction dressed by application of self constructed battlefield ophthalmic dressing].

The treatment of acquired, imperfect drainage of tears through the lacrimal duct system in the situation of medial canthal region defect (operative or traumatic), is an important part of clinical ophthalmology and reconstructive surgery. The multidisciplinary approach to the reconstruction of all soft tissues with use of early prosthetic reconstruction and lower canalicular intubation by use of silicone tube or simple nylon thread, is the theme of the paper. The lacrimal duct repair should be performed on primarily, as well as coverage or reconstruction of skin defect. The authors compared the

Lacrimal surgery at St John, Jerusalem.

Lacrimal surgery forms a prominent component of a general ophthalmic surgeon's workload at the St. John Ophthalmic Hospital, Jerusalem. The hospital caters for the Palestinian community of East Jerusalem, the West Bank and Gaza, the combined population totaling approximately 2.2 million. The area is under-developed economically and, in addition, the ongoing political conflict, the Intifada, makes the provision of health care difficult. Nonetheless the St. John Ophthalmic Hospital provides a level of surgical care which is notable for the fact that it is consistent and high. Advanced, obstructi

Endoscopic Denker's Approach for the Treatment of Extensive Sinonasal Tumors: Our Experience.

OBJECTIVES: Surgical management of large sinonasal tumors, spreading to the pterygopalatine, and infratemporal fossae have always been a challenge for the ENT surgeon and the multidisciplinary team. METHODS: Endoscopic Denker's approach allows complete exposure of the anterior, inferior, and lateral recesses of the maxillary sinus, without necessitating a gingivobuccal or a transseptal incision. Moreover, endoscopic Denker's approach facilitates direct, straight entry to the pterygopalatine and infratemporal fossae. RESULTS: Twenty-two patients with extensive sinonasal pathology underwent re
