Adalimumab as steroid-sparing treatment of inflammatory-stage thyroid eye disease.

PURPOSE: Steroids are often used as medical therapy for active thyroid eye disease (TED). While high-dose steroids have been shown to be effective in reducing the severity of TED symptoms, the side effects of steroids can be severe. As the pathogenesis of TED is thought to involve the upregulation of proinflammatory cytokines, including tumor necrosis factor-α (TNF-α), it has been postulated that anti-TNF agents may be used as steroid-sparing agents in the treatment of TED. This retrospective study was conducted to examine the efficacy of adalimumab, a subcutaneously administered TNF-α antagon

Breast cancer in a male with thyroid-associated orbitopathy and subclinical hyperthyroidism: a rare occurrence.

We present a case of a biochemically euthyroid male with active moderately severe thyroid associated orbitopathy who was coincidentally found to have breast carcinoma. The relationship between thyroid disease and breast carcinoma is discussed. The authors recommend that an enquiry of any breast lump or abnormality should be undertaken in all patients with thyroid disease.

Pure eye muscle involvement in endocrine orbitopathy.

We present a long-term follow-up examination concerning patients with isolated extra-ocular muscle involvement in thyroid-related orbitopathy. Within the previous 13 years we observed 7 patients with endocrine orbitopathy and marked myopathy of the extra-ocular muscles. Five of these patients had no detectable proptosis, 2 of them showed a minimal unilateral proptosis. Five patients showed elevated thyroid-stimulating hormone (TSH) receptor auto-antibodies and 6 patients a marked swelling of the extra-ocular eye muscles on CT or MRI scans. One patient had elevated antibodies against thyroid pe

The role of TRPV1 in corneal wound healing under a thyroxine-induced TAO-like condition.

Thyroid-associated ophthalmopathy (TAO) is an autoimmune orbital disorder associated with proptosis, ocular surface exposure, and corneal complications that may impair corneal repair. Transient receptor potential vanilloid 1 (TRPV1) is a calcium-permeable stress sensor involved in epithelial repair, inflammation, and stromal remodeling; however, its role in corneal wound healing under TAO-like ocular surface conditions remains unclear. In this study, a thyroxine (T4)-induced TAO-like condition was established in rats, followed by standardized alkali corneal injury to evaluate healing responses

CD40 Enhances Sphingolipids in Orbital Fibroblasts: Potential Role of Sphingosine-1-Phosphate in Inflammatory T-Cell Migration in Graves' Orbitopathy.

PURPOSE: Graves' orbitopathy (GO) is an autoimmune orbital disorder associated with Graves' disease caused by thyrotropin receptor autoantibodies. Orbital fibroblasts (OFs) and CD40 play a key role in disease pathogenesis. The bioactive lipid sphingosine-1-phosphate (S1P) has been implicated in promoting adipogenesis, fibrosis, and inflammation in OFs. We investigated the role of CD40 signaling in inducing S1P activity in orbital inflammation. METHODS: OFs and T cells were derived from GO patients and healthy control (Ctl) persons. S1P abundance in orbital tissues was evaluated by immunofluor

Orbit and orbital apex.

Pathology affecting the orbit and orbital apex is diverse and heterogeneous. Many of the differential pathologies require management in a multidisciplinary team involving both otolaryngology and ophthalmology. This article discusses the differential pathologies. Emphasis has been placed on Graves orbitopathy, traumatic optic neuropathy, and the indications for decompression in each. The differential diagnosis for a lesion within the orbit and orbital apex is diverse. The presentation, investigation, and appropriate management of these conditions is discussed with emphasis on traumatic optic ne

Teprotumumab Divergently Alters Fibrocyte Gene Expression: Implications for Thyroid-associated Ophthalmopathy.

CONTEXT: Teprotumumab, an IGF-I receptor (IGF-IR) inhibitor, is effective in thyroid-associated ophthalmopathy (TAO). The drug can modulate induction by TSH of IL-6 and IL-8 in CD34+ fibrocytes and their putative derivatives, CD34+ orbital fibroblasts (CD34+ OF). Fibrocytes express multiple thyroid autoantigens and cytokines implicated in TAO, which are downregulated by Slit2. Inflammation and disordered hyaluronan (HA) accumulation occur in TAO. Whether teprotumumab alters these processes directly in fibrocytes/CD34+ OF remains uncertain. OBJECTIVE: Determine teprotumumab effects on expressi
