Orthognathic surgery for the complete rehabilitation of Moebius patients: principles, timing and our experience.

Moebius syndrome is a rare disorder found in approximately 1/100,000 neonates and the treatment of facial palsy is now well established worldwide and consists of free-muscle transplants reinnervated with motor nerves. Dentofacial deformities are often detected in Moebius patients, and different degrees of micrognathia are often present, particularly in patients with complete expressions of Moebius syndrome. However only two published reports have described the surgical treatment of such anomalies in these patients; in both cases, the suggested approach consisted of orthognathic surgery followe

Randomized controlled trial on physical therapy for TMJ closed lock.

This study evaluated the one-year effect of physical therapy on pain and mandibular dysfunction associated with anterior disc displacement without reduction of the temporomandibular joint (closed lock). Forty-nine individuals were randomly assigned to either a physical therapy group [n = 23, mean age (SD) 34.7 (14.0) yrs] or a control group [n = 26, mean age 38.5 (15.1) yrs]. At baseline and after 3, 6, 12, 26, and 52 wks, pain and mandibular function were evaluated. All patients received extensive information about avoiding parafunctions and oral habits on all evaluation days. The physical th

Facial movement before and after masseteric-facial nerves anastomosis: a three-dimensional optoelectronic pilot study.

To quantify the effects of facial palsy reanimation, 14 patients aged 17-66 years were analysed. All patients had unilateral facial paralysis, and were candidates for surgical masseteric to facial nerve anastomosis. Two patient groups were measured: seven patients were waiting for surgery, the other seven patients had already been submitted to surgery, and had regained facial mimicry. Each patient performed three facial animations: brow raise; free smile; lip purse. These were recorded using an optoelectronic motion analyser. The three-dimensional coordinates of facial landmarks were obtained,

A clinical, MRI, and EMG analysis comparing the efficacy of twin blocks and flat occlusal splints in the management of disc displacements with reduction.

Internal derangement of the temporomandibular joint (TMJ) is characterized by an abnormal disc-condyle relationship. Of all the various treatment modalities used in the management of disc displacements, flat occlusal and anterior repositioning splints are the most commonly used. Myofunctional appliances (such as bite-jumping appliances) are also advocated to treat anterior disc displacements. The present study compares the efficacy of twin blocks with conventional flat occlusal splints in patients with anterior disc displacement with reduction using clinical examination, MRI, and EMG. The samp

A machine learning approach to predict treatment response in myofascial pain patients receiving masseter trigger point injections.

BACKGROUND: Trigger point injection (TPI) therapy is widely used for masseter myofascial pain syndrome (MPS), yet outcomes vary substantially. Individualized prediction tools are lacking, often leading to trial-and-error treatment selection. OBJECTIVE: To develop, externally validate, and generalize ensemble machine learning models for predicting composite treatment success following masseter TPI, and to deploy a web-based clinical decision support system (CDSS). METHODS: This multicenter study included 1,181 patients with DC/TMD‑diagnosed masseter MPS treated with one of six injectable moda

Spectral components in electromyograms from four regions of the human masseter, in natural dentate and edentulous subjects with removable prostheses and implants.

OBJECTIVE: To compare the frequency or spectral components between different regions of the superficial masseter in young natural dentate and total edentulous older adults rehabilitated with removable prostheses and fixed-implant support. A secondary objective was to compare these components between the three groups. DESIGN: 21 young natural dentate and 28 edentulous (14 with removable prostheses and 14 with fixed-implant support) were assessed. High-density surface electromyography (sEMG) was recorded in four portions of the superficial masseter during submaximal isometric bites. Spectral co

Dual Coaptation of Facial Nerve Using Masseteric Branch of Trigeminal Nerve for Iatrogenic Facial Palsy: Preliminary Reports.

OBJECTIVES: Immediate facial nerve substitution or graft technique has been used for the repair of facial nerve defects occurring as a result of tumour dissection. However, some patients report unsatisfactory outcomes, such as difficulty in maintaining resting or smiling symmetry, due to persistent flaccid facial palsy. Here we evaluated the functional outcomes of transferring the masseteric branch of the trigeminal nerve to the facial nerve adjunct to facial nerve graft. METHODS: We reviewed the medical records of seven patients who underwent facial reanimation surgery between 2014 and 2016.

Oblique osteotomy and coronoidectomy in extreme prognathism of Apert syndrome.

We report a case of oblique osteotomy and coronoidectomy for correction of extreme prognathism in Apert syndrome.A 16-year-old girl presented with a prognathic mandible. A prototype model was formed using computed tomography. After a mock bilateral oblique osteotomy of the mandible, the distal segment did not overlap the proximal segment, and it caused bony impingement between the coronoid process and the subcondyle of the proximal segment. An oblique osteotomy of the mandible was done through a submandibular approach, and bilateral coronoidectomy followed. After surgery, mandibular parameters

Progression of preexisting trigeminalgia to Tolose-Hunt-like syndrome. The importance of neuroimaging for early differential diagnosis.

Recurrent unbearable, paroxysmal, unilateral facial pain in the distribution of one or more branches of the trigeminal nerve often provoked by sensory stimuli is typical for idiopathic trigeminal neuralgia. The less frequent localization in the area of ophthalmic branch (5%) is particularly controversial and should be distinguished from pathological lesions in the brainstem and middle and posterior cranial fossa and from diseases of the orbit and eye. This case study presents a 79-year-old woman with typical clinical features of 1st division trigeminalgia without any neurological loss and with

Peripheral neuromuscular manifestations in systemic sclerosis (scleroderma).

Systemic sclerosis (scleroderma) is thought to be the least likely of the collagen vascular disorders to cause nervous system damage. We evaluated the peripheral neuromuscular manifestations in 32 patients with scleroderma. A clinically defined peripheral nervous system (PNS) lesion was manifest in 5 of 32 patients (16%), including 2 patients with trigeminal neuropathy and single cases of polyneuropathy, brachial plexopathy, and lumbosacral radiculopathy. Neurophysiological studies suggested subclinical PNS involvement in 6 additional patients (3 with distal axonal polyneuropathy, 1 with proba

Biomechanical analysis comparing natural and alloplastic temporomandibular joint replacement using a finite element model.

PURPOSE: Prosthetic materials and bone present quite different mechanical properties. Consequently, mandible reconstruction with metallic materials (or a mandible condyle implant) modifies the physiologic behavior of the mandible (stress, strain patterns, and condyle displacements). The changing of bone strain distribution results in an adaptation of the temporomandibular joint, including articular contacts. MATERIALS AND METHODS: Using a validated finite element model, the natural mandible strains and condyle displacements were evaluated. Modifications of strains and displacements were then

Long term follow up after transorbital penetrating injury: A case report.

PURPOSE: Traumatic brain injury is the leading cause of mortality and disability among young individuals. Unfortunately, there are few publications concerning long term follow up of patients with these types of injuries. We present a case of trans-orbital penetrating brain injury with an 18 year follow up. OBSERVATIONS: A 43-year-old, previously healthy, male was accidently impaled on a fencing foil resulting in a penetrating brain injury. Initial symptoms included diplopia, ophthalmoparesis, a non-reactive pupil, decreased visual acuity, decreased sensation across the cheek, dysphagia and dy

Passive and active immunization models of MuSK-Ab positive myasthenia: electrophysiological evidence for pre and postsynaptic defects.

Antibodies directed against the post-synaptic neuromuscular junction protein, muscle specific kinase (MuSK) are found in a small proportion of generalized myasthenia gravis (MuSK-MG) patients. MuSK is a receptor tyrosine kinase which is essential for clustering of the acetylcholine receptors (AChRs) at the neuromuscular junction, but the mechanisms by which MuSK antibodies (MuSK-Abs) affect neuromuscular transmission are not clear. Experimental models of MuSK-MG have been described but there have been no detailed electrophysiological studies and no comparisons between the MuSK-MG and the typic

Recurrent Ameloblastoma of the Mandibular Ramus Resected Using a Modified High Perimandibular Approach: A Case Report.

Ameloblastoma typically lacks a well-defined capsule and exhibits locally invasive growth, making them prone to recurrence. On the other hand, a subtype known as the unicystic type is generally well circumscribed and less invasive compared with conventional solid/multicystic variants, but recurrence may occur in rare cases. Therefore, after tumor removal, it is essential to resect or curette the adjacent bone surface that may have been infiltrated. The high perimandibular approach is a surgical technique originally developed for treating condylar fractures. It involves a 4-5 cm skin incision p

Does combining oro-facial manual therapy with bruxism neuroscience education affect pain and function in cases of awake bruxism? A pilot study.

BACKGROUND: Although awake bruxism is associated with temporomandibular disorder (TMD) as well as head and neck pain, the effects of physical therapy and bruxism education to address these factors have not been investigated. OBJECTIVE: The aim of this study was to evaluate the effects of oro-facial manual therapy and bruxism neuroscience education (BNE) on awake bruxism over a 3-week period with an open-ended follow-up questionnaire after 3 months. METHODS: Subjects (n = 28) were randomly allocated to one of two groups, an intervention group and a control group. Data regarding disability, fu

Chocolate intake and muscle pain sensation: A randomized experimental study.

BACKGROUND: Chocolate, as a cocoa-derived product rich in flavanols, has been used for medical and anti-inflammatory purposes. Therefore, the aim of this study was to investigate if the ingestion of different percentages of cocoa products affects the experimentally induced pain caused by intramuscular hypertonic saline injections in the masseter muscle of healthy men and women. METHODS: This experimental randomized, double-blind, and controlled study included 15 young, healthy, and pain-free men and 15 age-matched women and involved three visits with at least a 1-week washout. Pain was induce

Tooth eruption results from bone remodelling driven by bite forces sensed by soft tissue dental follicles: a finite element analysis.

Intermittent tongue, lip and cheek forces influence precise tooth position, so we here examine the possibility that tissue remodelling driven by functional bite-force-induced jaw-strain accounts for tooth eruption. Notably, although a separate true 'eruptive force' is widely assumed, there is little direct evidence for such a force. We constructed a three dimensional finite element model from axial computerized tomography of an 8 year old child mandible containing 12 erupted and 8 unerupted teeth. Tissues modelled included: cortical bone, cancellous bone, soft tissue dental follicle, periodont
