Hyperphosphatemic tumoral calcinosis caused by FGF23 compound heterozygous mutations: what are the therapeutic options for a better control of phosphatemia?

BACKGROUND: Hyperphosphatemic familial tumoral calcinosis (HFTC) is a rare autosomal recessive disease caused by mutations in genes encoding FGF23 or its regulators, and leading to functional deficiency or resistance to fibroblast growth factor 23 (FGF23). Subsequent biochemical features include hyperphosphatemia due to increased renal phosphate reabsorption, and increased or inappropriately normal 1,25-dihydroxyvitamin D (1,25-D) levels. CASE-DIAGNOSIS/TREATMENT: A 15-year-old girl was referred for a 1.2-kg-calcified mass of the thigh, with hyperphosphatemia (2.8 mmol/L); vascular impairment

Severe vascular calcification and tumoral calcinosis in a family with hyperphosphatemia: a fibroblast growth factor 23 mutation identified by exome sequencing.

BACKGROUND: Tumoral calcinosis is an autosomal recessive disorder characterized by ectopic calcification and hyperphosphatemia. METHODS: We describe a family with tumoral calcinosis requiring amputations. The predominant metabolic anomaly identified in three affected family members was hyperphosphatemia. Biochemical and phenotypic analysis of 13 kindred members, together with exome analysis of 6 members, was performed. RESULTS: We identified a novel Q67K mutation in fibroblast growth factor 23 (FGF23), segregating with a null (deletion) allele on the other FGF23 homologue in three affected m

Diseases associated with hidranitis suppurativa: part 2 of a series on hidradenitis.

Hidradenitis suppurativa (HS), a pathologic follicular disease, impacts patients' lives profoundly and usually occurs in isolation. The diseases with the strongest association are obesity, depression, and pain. HS is associated with many diseases including acne conglobata (AC), dissecting cellulitis, pilonidal cysts, and obesity. Pyoderma fistulans sinifica (fox den disease) appears to be the same entity as Hurley Stage 2 of 3 HS. The rate of acne vulgaris in HS patients mirrors unaffected controls. The most common, albeit still uncommon, association is with seronegative, haplotype unlinked ar

[Treatment of spinal fractures complicating ankylosing spondylitis].

OBJECTIVE: To study the treatment of spinal fractures in ankylosing spondylitis. METHODS: Nineteen cases of spinal fractures complicating ankylosing spondylitis admitted in our hospital were studied retrospectively. RESULTS: All of 19 cases were up to the diagnosis standards of ankylosing spondylitis. Eleven patients had cervical fracture and 8 had thoracolumbar fracture. Of the patients with cervical fracture, fractures occurred at C(5 - 7) in 9 patients. Of the patients with thoracolumbar injury, stress fractures were seen in 7 patients and all of seven fractures occurred at T(10)-L(2). Si

[Vertebral manifestation of chronic recurrent multifocal osteomyelitis (CRMO)].

Chronic recurrent multifocal osteomyelitis (CRMO) is a systemic osteo-articular disease that is characterized by a sterile, primarily chronic osteomyelitis with various distribution patterns of the individual lesions. In this article, we describe the "axial type" with predominant involvement of the spine, which represents 13 of our 41 CRMO cases of different age groups. The important element of its diagnosis is the typical lympho-plasmacellular spondylitis that can be detected and staged by scintigraphy, MRI and conventional radiography. Potentially affected are all vertebrae from the mid-cerv

Subadult health and disease in prehistoric Tonga, Polynesia.

This article focuses on the differential diagnosis of pathologic lesions recorded on the limbs and crania of 17 subadults from two pre-European burial mounds in Tonga, western Polynesia. All affected subadults were between the ages of 6 months and 3 years at death. The lesions described consist primarily of subperiosteal new bone deposition on the limbs and endocranial surface. However, the presence of cribra orbitalia in a number of individuals indicates concurrent iron-deficiency anaemia. A differential diagnosis of haematogenous osteomyelitis, congenital syphilis, yaws, scurvy, hypervitamin

Connective tissue and joint disease in diabetes mellitus.

Connective tissue is ubiquitous and subject to alterations that result in changes in the extracellular matrix of vessels and tissues leading to the long-term complications of diabetes. This article reviews only those abnormalities of interstitial connective tissue involving skeleton, joints, skin, and periarticular tissues. Abnormalities in the skin and periarticular tissues result in syndromes limiting joint movement, including limited joint mobility, Dupuytren disease, flexor tenosynovitis, carpal tunnel syndrome, stiff-hand syndrome, and shoulder-hand reflex dystrophy. Of these, only limite

Application of musculoskeletal ultrasound in endemic skeletal fluorosis: a comparative study of water-borne and brick-tea-borne subtypes.

BACKGROUND: Endemic skeletal fluorosis is a chronic disorder caused by prolonged excessive fluoride intake and is characterized by skeletal hyperostosis, periarticular soft-tissue degeneration, and joint dysfunction. Digital radiography (DR) is the conventional diagnostic modality, but it has limited sensitivity for early soft-tissue changes and provides little dynamic information. Musculoskeletal ultrasound (MSKUS) may address these limitations, yet comparative evidence across major fluorosis subtypes remains limited. OBJECTIVE: To characterize MSKUS findings in endemic skeletal fluorosis an

DOTATATE-PET in custom spheno-orbital implant design for meningiomas with orbital involvement: a case series.

OBJECTIVE: Hyperostosing spheno-orbital meningiomas (SOMs) infiltrate bone and cause bone overgrowth leading to progressive visual and cosmetic deformities. DOTA0-Tyr3-octreotate (DOTATATE) is a somatostatin receptor-targeted PET ligand that enables highly sensitive identification of meningiomas. This ligand is useful in defining the extent of bone infiltration by the meningioma, which can be difficult to detect on standard CT and MRI. In this study, the authors aimed to provide a pragmatic technical framework for integrating molecular imaging, navigation, and patient-specific reconstruction i

Treatment of Severe Palmoplantar Pustular Psoriasis With Bimekizumab.

IMPORTANCE: Palmoplantar pustulosis (PPP) and palmoplantar plaque psoriasis with pustules remain challenging to treat. Studies suggest that an interleukin 17 or interleukin 36 loop acts synergistically in these diseases to induce palmoplantar pustules. OBJECTIVE: To assess the effectiveness of bimekizumab in treating PPP and palmoplantar plaque psoriasis with pustules. DESIGN, SETTING, AND PARTICIPANTS: This case series involved 21 adults with PPP (11 patients) or palmoplantar plaque psoriasis with pustules (10 patients) treated at 1 of 7 tertiary dermatological centers in France from Septem

Loss of the disease-associated glycosyltransferase Galnt3 alters Muc10 glycosylation and the composition of the oral microbiome.

The importance of the microbiome in health and its disruption in disease is continuing to be elucidated. However, the multitude of host and environmental factors that influence the microbiome are still largely unknown. Here, we examined UDP-GalNAc:polypeptide N-acetylgalactosaminyltransferase 3 (Galnt3)-deficient mice, which serve as a model for the disease hyperphosphatemic familial tumoral calcinosis (HFTC). In HFTC, loss of GALNT3 activity in the bone is thought to lead to altered glycosylation of the phosphate-regulating hormone fibroblast growth factor 23 (FGF23), resulting in hyperphosph

Low-level mesodermal somatic mutation mosaicism: late-onset craniofacial and cervical spinal hyperostoses.

Craniofacial and cervical spinal hyperostoses are rarely seen in the absence of other abnormalities. Only seven patients with isolated cranial hyperostoses have been reported, and only a single patient with both calvarial and cervical vertebral hyperostoses. We report on an adult with late-onset right-sided asymmetrical hyperostoses of the cranium, mandible, and cervical vertebrae in the absence of an AKT1 mutation. At presentation, the patient displayed neither generalized overgrowth nor dysregulated adipose tissue. Standard polymerase chain reaction and Sanger sequencing of DNA extracted fro

The spectrum of orofacial manifestations in renal osteodystrophy: diagnostic challenges of an uncommon presentation.

Renal osteodystrophy refers to a spectrum of bone diseases caused by pathologic alterations in the metabolism of calcium, phosphate, and bone in the context of end-stage renal disease and secondary hyperparathyroidism. Radiographic alterations affecting the jaw and facial skeleton are common and among the earliest signs of renal bone disease. Renal osteodystrophy also shares clinical, histologic, and radiologic similarities with several benign fibro-osseous conditions affecting the craniofacial region, and its recognition is critical to prevention, choice of therapy, and overall prognosis. Vascularised fibula graft inlaid in a massive bone allograft: considerations on the bio-mechanical behaviour of the combined graft in segmental bone reconstructions after sarcoma resection.

Vascularised fibula transfer (VFT) is a trustworthy procedure in the reconstruction of skeletal defects secondary to several pathological conditions. Over the last 20 years, progress in diagnosis in skeletal oncology and in adjuvant therapy has increased the possibility of applying limb-saving procedures in the treatment of bone sarcoma. In this context, VFT appeared to be a valuable reconstructive tool following the wide segmental resection of the long bones of the limbs. The vascularized fibula allows for fast bone fusion. It also demonstrates a tendency of progressive hypertrophy and, in th

Resectability of spheno-orbital meningiomas: surgical outcome in 93 cases and a proposed clinically relevant anatomical classification.

OBJECTIVE: Resection is the foundation of spheno-orbital meningioma (SOM) treatment, but the confluence of critical neurovascular structures traversing the cranium and orbit and the unforgiving nature of orbital injury make resection of the tumor's orbital components challenging. Large series detailing surgical outcomes are rarely reported, and a surgical framework is needed. This study examined the feasibility of resection of SOMs with an involved orbit. The authors classified SOMs based on the relative orbital involvement to determine tumor resectability, with the goals of improving proptosi

Anthropological and palaeopathological analysis of the human remains from three "Tombs of the Nobles" of the necropolis of Thebes-west, upper Egypt.

During several recent excavation campaigns at the necropolis of Sheikh-Abd-el-Gurna, Thebes-West, Upper Egypt, we investigated the human remains of three "Tombs of the Nobles" totalling at least 273 individuals. The investigation covered the human material (skeletons and mummy residues) from the tombs TT-84, TT-85 and TT-95. These tombs had been built in the New Kingdom (approx. 1500-1000 B.C.) and used until the Late period (up to 330 BC). All samples were analyzed macroscopically, isolated findings were further investigated by endoscopic and radiological techniques. The at least 273 individu
