Iatrogenic facial nerve injury during otologic surgery.

Perhaps the most devastating complication in otologic surgery is that of inadvertent injury to the facial nerve. A review of 22 patients who had sustained an iatrogenic facial nerve injury was conducted. Although the most common procedure being performed during the injury was mastoidectomy (55%), a surprising number of patients had injury during tympanoplasty (14%) or during removal of exostoses (14%). The most common area of injury to the facial nerve in this series was in the tympanic segment. In 79% of the patients, the facial nerve injury was not detected at the time of surgery. All patien

Hearing aids poststapedectomy: incidence and timing.

Ninety-four percent of 508 patients operated on between 1960 and 1975 have been followed-up at yearly intervals. Their status as of 1977 was assessed in order to determine their mean duration of socially adequate hearing levels and to relate the type of surgical procedure to the interval between operation and hearing aid requirement. Postoperative hearing losses due to conduction and sensorineural deficits resulted in preoperative bone/postoperative air gaps greater than 10 dB in up to 40% of large fenestra operations and in 15% of small fenestra operations at 5 years. On average, a large fene

Experience with stapedectomy in a developing country: a review of 200 cases.

Since stapedectomy was first performed in 1956, many innovations have been described. We retrospectively analyzed 200 cases of stapedectomy that had been performed with standard otologic instruments. Of the 200 patients, 54 (27.0%) had significant sensory deafness associated with footplate fixation. In most cases, the footplate was partially removed (stapedotomy group); in the remainder, the footplate was removed in its entirety or subtotally (stapedectomy group). Of the 200 patients, 163 were available for short-term follow-up. The air-bone gap was closed to within 10 dB in roughly 83% of bot

Assessment of the Oval Window-Facial Canal Corridor and Mastoid Pneumatization: A Preliminary HRCT-Based Radiological-Anatomical Framework.

Background/Objectives: The anatomical relationship between the oval window (OW) and the facial nerve (FN) is of significant importance in the context of stapes and middle ear surgery. However, the influence of mastoid pneumatization on OW morphology and FN proximity remains to be elucidated. Methods: This retrospective study included 220 radiologically normal temporal bones from 110 adults. OW width, OW height, OW niche width, FNt-OW distance, FNm-OW distance, OW-mastoid cell distance, and OW-FNm-FNt corridor area were measured on HRCT images. Mastoid pneumatization was classified using a sigm

Titanium Prostheses for Treating Posttraumatic Ossicular Chain Disruption.

BACKGROUND: Posttraumatic chain disruption may be caused by blunt head trauma, barotrauma, or a penetrating foreign body. In cases of severe damage to the incus, or its absence, a titanium prosthesis is a good option for reconstructing the ossicular chain. METHODS: A retrospective analysis was performed on 24 cases of posttraumatic ossicular chain disruption that had been treated with a titanium partial or total ossicular replacement prosthesis. Air conduction, bone conduction, and air-bone gap were measured before, 6-12 months after, and more than 2 years after the operation. Hearing thresho

Evaluation of Vibrant® Soundbridge™ positioning and results with laser doppler vibrometry and the finite element model.

The etiology of hearing loss originates from genetic factors and includes several other events including infections, working or living environment, as well as several endocrine and metabolic disorders. The Vibrant® Soundbridge™ (VSB) is an implantable hearing aid whose floating mass transducer (FMT) is attached to the long process of the incus. The device is used for pure sensorineural hearing loss with an intact middle ear. Variations in the manner of attachment may occur. Knowledge of the impact of such variations on the overall device performance may guide towards optimal transducer attachm

Study of the improvement in bone conduction threshold after stapedectomy.

INTRODUCTION AND OBJECTIVES: Bone conduction threshold depression is not always a result of inner ear and cochlear nerve pathology. In fact, middle ear pathologies may be responsible for such threshold depression, as occurs in otosclerosis. The aims of this study were to evaluate the improvement of bone conduction threshold in patients with otosclerosis that underwent stapedectomy and to study the postoperative audiological results. MATERIAL AND METHODS: This was a retrospective study on 95 patients (116 ears) diagnosed with otosclerosis having conductive or mixed hearing loss that received s
