Synchronous splenectomy during cholecystectomy for hereditary spherocytosis: is it really necessary?

BACKGROUND/PURPOSE: Expert guidelines recommend performing synchronous splenectomy in patients with mild hereditary spherocytosis (HS) and symptoms of gallstone disease. This recommendation has not been widely explored in the literature. The aim of this study is to determine if our data support expert opinion and if different practice patterns should exist. METHODS: This is an IRB-approved retrospective study. All HS patients under 18 years of age who underwent cholecystectomy for symptomatic gallstones at a single institution between 1981 and 2009 were identified. Patients who underwent chol

A pseudoepidermoid cyst arising from exuberant squamous metaplasia of the gallbladder.

Squamous lesions of the gallbladder are uncommon. We report a case of a pseudoepidermoid cyst in a middle-aged woman arising on a background of chronic cholecystitis and cholelithiasis, which induced exuberant squamous metaplasia of the entire gallbladder mucosa, clinically mimicking a gallbladder tumor. To our knowledge, this is the first such reported case in the English literature.

Serum Procalcitonin as an Indicator of Disease Severity and Clinical Deterioration in Hospitalized Patients With Acute Cholangitis.

BACKGROUND: Acute cholangitis (AC) results from bile stasis and subsequent infection in the biliary tract, requiring urgent evaluation to determine the timing of biliary decompression. Procalcitonin (PCT) and lactic acid (LAC) may be reliable indicators of disease severity to suggest early biliary decompression. We seek to assess the utility of PCT and LAC to predict severe AC and adverse in-hospital outcomes such as ICU transfer, vasopressor use, or mortality. METHODS: This is a retrospective observational study of patients admitted with AC to a tertiary hospital in the United States between

Uncovering variation in cholecystitis treatment: protocol and statistical analysis plan for a nationwide observational study - the Dutch Cholecystitis Snapshot Study (Dutch CHESS).

INTRODUCTION: Cholecystitis is a highly prevalent disease that imposes a substantial burden on the healthcare system. Despite strong underlying evidence, guideline adherence in the treatment of cholecystitis remains low. Moreover, important gaps in knowledge persist that must be addressed to optimise existing guidelines. The primary aim is to assess the nationwide variation in cholecystitis treatment and identify opportunities to improve guideline adherence. Secondary aims include determining the best cystic duct closure method; the best model to predict concomitant choledocholithiasis; the op

[Needle-knife sphincterotomy for biliary access: a prospective study].

OBJECTIVE: Needle-knife sphincterotomy (NKS) is used to achieve biliary access when routine cannulation methods have been unsuccessful. The aim of this study was to analyze the results of NNS in patients with an inaccessible common bile duct (difficult cannulation) using standard techniques in endoscopic retrograde cholangiopancreatography and the factors associated with the success of this procedure and complications. MATERIAL AND METHODS: We performed a prospective study of 72 patients who underwent NKS performed by the same endoscopist (J.E.) because of difficult cannulation of the common

Overview of therapeutic modalities for the treatment of gallstone diseases.

The management of gallstone diseases has been revolutionized in less than 2 years by the advent of laparoscopic cholecystectomy (LC). However, the rapid adoption of LC has occurred without comparative randomized trials with other available therapies. Thus, the evaluation of LC versus other therapies can only be based on case series. The criteria used for this evaluation are clinical effectiveness, cost-effectiveness, and the patient's level of acceptance and satisfaction with the procedure. The techniques of both LC and open cholecystectomy (OC) have the advantage over other approaches, such a

Incidental gallbladder cancer at cholecystectomy.

BACKGROUND: Gallbladder tumours rank fifth in the world among gastrointestinal system tumours. Coincidental gallbladder tumours are diagnosed during cholecystectomies, or by examining the cholecystectomy material. AIMS: In this study, we aimed to evaluate the incidence of gallbladder cancer among patients undergoing cholecystectomies due to gallbladder disease. STUDY DESIGN: Retrospective study METHODS: The files of 341 patients who had undergone routine cholecystectomy operations between January 2013 and March 2016 were reviewed, and their pathology results were recorded. Those patients wit

Use of the self-assembling peptide (PuraStat™) for the management and prevention of ERCP-related bleeding: a multicenter prospective cohort study.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) remains a cornerstone therapeutic intervention for pancreatobiliary disease but is associated with serious adverse events. Bleeding related to endoscopic sphincterotomy or periampullary manipulation may be difficult to control because of anatomic constraints and patient-related risk factors. PuraStat™ is a transparent self-assembling peptide (SAP) hydrogel that promotes hemostasis and healing. Data specific to the efficacy of SAP in ERCP-related bleeding remains limited. METHODS: We performed a multicenter, prospective, observa

A four-year analysis of the predictors of overnight stay in day-case laparoscopic cholecystectomy and the prevention strategies within two centers in the UK: a double-centric retrospective observational study.

BACKGROUND: Laparoscopic cholecystectomy (LC) is the mainstay treatment of benign gallbladder disease, most commonly gallstones. Although the recent guidelines of British Association of Day-Surgery (BADS) and World Society of Emergency Surgery (WSES) recommend same-day discharge (i.e. day-case surgery), there is still a considerable proportion of cases that get admitted unexpectedly overnight. The primary aim of this study is to determine the predictors of overnight stay and consequently develop prevention strategies. MATERIALS AND METHODS: This was a retrospective study involving 913 patient

Incidental Detection of Post-cholecystectomy Lemmel Syndrome With Burr Hole Plating and Ventriculoperitoneal Shunt: A Cadaveric Case Report of Quadruple Pathology and Clinico-Pathophysiological Correlation.

During surgical anatomy dissection on a cadaveric subject for educational purposes in a medical university, we encountered a rare syndrome and evidence of a constellation of three past surgeries, which provided us with an intellectually rewarding tapestry of clinical and academic findings sufficient to enrich the overall learning experience. During her lifetime, the elderly female subject had undergone a cholecystectomy, presumably for calculous cholecystitis. The dissection also revealed what was putatively diagnosed as type III Lemmel syndrome, characterized by a large periampullary duodenal

Imaging Findings of Mirizzi Syndrome Associated With Acalculous Cholecystitis: A Case Report.

Right upper quadrant pain is one of the most frequent reasons for consultations in the emergency room. Gallbladder pathology is among the most common etiologies and can include cholecystitis, cholelithiasis, choledocholithiasis, and cholangitis, among others. Mirizzi syndrome is a complication that manifests as hepatobiliary dysfunction due to a gallstone causing extrinsic compression of the common bile duct. However, acute cholecystitis can externally obstruct the common bile duct and mimic Mirizzi syndrome in the absence of a causative gallstone. The relevance of this syndrome lies in its ti

Safety and Efficacy of Endoscopic Retrograde Cholangio-Pancreatography in Patients of Liver Cirrhosis: A Case-Control Study.

Background With the improvement in noninvasive diagnostic imaging modalities, Endoscopic Retrograde Cholangio-Pancreatography (ERCP) has evolved into a primarily therapeutic procedure. Besides being efficacious and one of the most commonly done procedures, ERCP is also associated with a high risk of complications. However, there is a lack of studies analyzing the safety and success of ERCP in patients with liver cirrhosis. We retrospectively evaluated the outcome of ERCP in patients with cirrhosis of the liver compared to non-cirrhotic patients using the database from our institute. Methods Pa

A descriptive study of abdominal complications in patients with mild COVID-19 presenting to the emergency department: a single-center experience in Japan during the omicron variant phase.

BACKGROUND: COVID-19 is widely known to induce a variety of extrapulmonary manifestations. Gastrointestinal symptoms have been identified as the most common extra-pulmonary manifestations of COVID-19, with an incidence reported to range from 3 to 61%. Although previous reports have addressed abdominal complications with COVID-19, these have not been adequately elucidated for the omicron variant. The aim of our study was to clarify the diagnosis of concomitant abdominal diseases in patients with mild COVID-19 who presented to hospital with abdominal symptoms during the sixth and seventh waves o
