[Hypertrophic obstructive cardiomyopathy: surgical treatment and results].

The natural history of hypertrophic obstructive cardiomyopathy is characterized, in particular, in younger patients by sudden cardiac death, in the majority of patients by progressive congestive heart failure [14, 26, 27, 33-36]. With the aid of beta-adrenergic receptor blockers, calcium channel blockers and antiarrhythmic agents the clinical outlook for most of the patients can be improved [17, 19, 23, 28]. Patients who do not respond to medical treatment can be regarded as possible candidates for surgical revision of the left ventricular outflow tract. In consideration of surgical treatment,

[Arrhythmias in hypertrophic cardiomyopathy. Significance and therapeutic consequences].

The natural history of the patient with hypertrophic cardiomyopathy is characterized by slow progression of symptoms and left ventricular hypertrophy; severe functional limitation is unusual and encountered only in about 20%. Most of the deaths are sudden and occur irrespective of the functional status of the patient and there are no routine electrocardiographic or hemodynamic measurements indicative of poor prognosis. Sudden death may be associated with syncope and young age at diagnosis as well as with a family history of sudden death. The mechanisms underlying the cause of sudden death have

Laparoscopic nipple invagination combined extravesical (NICE) reimplantation technique in the management of primary obstructed megaureter.

AIMS: Villanueva challenged Paquin's 5:1 tunnel length and showed in a computer simulation model that UVJ competence was more sensitive to a 2-mm protrusion of ureteric orifice into the bladder compared to an increase in the intravesical tunnel. Thompson later successfully applied the Shanfield technique of invaginating the spatulated primary obstructed megaureter (POM) laparoscopically, causing a nipple antireflux mechanism. In this study we have reported the outcomes of our modification: Nipple Invagination Combined Extravesical (NICE) reimplantation, in the management of POM. METHODS: Pati

Treatment for swallowing difficulties (dysphagia) in chronic muscle disease.

BACKGROUND: The management of dysphagia (difficulty in swallowing), a common complication of long-term progressive muscle disease in children and adults, is currently unclear. OBJECTIVES: Our objective was to determine the most appropriate intervention for dysphagia in people with chronic, untreatable, non-inflammatory muscle disease. SEARCH STRATEGY: We searched the Cochrane Neuromuscular Disease Group trials register, Cochrane Central Register of Controlled Trials (Cochrane Library Issue 2, 2003), MEDLINE (from January 1966 to Week 2 June 2003), EMBASE (from January 1980 to Week 2 June 200

Personal experiences and algorithm of endoscopically assisted subperiosteal face lift in orientals for 5 years.

Orientals are anatomically distinct from Caucasians and are characterized by a thick dermis, a Mongoloid slant of the palpebral fissure, a relatively prominent zygoma and mandible angle, and a relatively flat nose. Given these characteristics, it was believed that the subperiosteal face lift was not suitable for Orientals. However, at our institution, endoscopically assisted subperiosteal face lifts were performed from May of 1994 to October of 1998 on 236 patients; variable pitfalls, as well as satisfying results, were reported. Patient ages ranged from 29 to 66 years (mean age, 55.2 years),

Functional Popliteal Artery Entrapment Syndrome.

INTRODUCTION: Popliteal artery entrapment is an uncommon syndrome, caused by extrinsic compression of the popliteal artery by muscular or tendinous structures. It occurs mainly in young individuals, with no atherosclerostic risk factors, and a mean age of presentation of 20 to 40 years, and has higher prevalence in males (83% of patients). Clinical presentation depends on the degree of arterial lesion, the most common being intermittent claudication, with critical ischemia being frequent. Occasionally, it may present as acute ischaemia. METHODS: A 40-year-old female patient, physical educatio

The use of the internet in data assimilation in rare diseases.

The incidence and prevalence of achalasia is 0.6/100,000 people per year and 1/10,000, respectively. It is difficult then for one center to accumulate a large cohort of patients. One study (Gut 33:1011,1992) described the presenting symptoms in only 38 patients. To approach this problem differently, we used the internet to access a larger patient population. Using search engines at Google and Yahoo, Inc., we identified achalasia support groups. We examined the most populated support group (YSG) and assembled the exchanged messages into 4 categories: support, symptoms, treatment, and diagnosis.

Selective peripheral denervation for refractory cervical dystonia: a single-center retrospective analysis of 160 cases.

OBJECTIVE: Cervical dystonia (CD) is the most common focal dystonia, characterized by abnormal head postures and pain, often refractory to botulinum toxin therapy. Selective peripheral denervation (SPD) remains a key surgical option, but long-term outcomes in large cohorts are underreported. The purpose of this study was to evaluate the long-term outcomes of SPD in patients with botulinum toxin-refractory CD, focusing on symptom severity, pain, and head posture improvement. METHODS: This retrospective study included 160 adults (mean ± SD age 54 ± 12.0 years, 46% male) with refractory CD who u

Management Strategies for Zenker's Diverticulum: A Comprehensive Review.

Zenker's diverticulum (ZD) is an esophageal condition that results in an outpouching of the mucosal layer through a weakened area in the hypopharyngeal wall. This condition can cause symptoms like dysphagia, regurgitation, and aspiration, impacting patients' quality of life. Historically, open surgery was the primary treatment. Although effective, this method is associated with longer recovery times and risks such as infections, nerve damage, and prolonged hospitalization. Rigid endoscopic stapling emerged as a less invasive alternative, offering high success rates for patients with favorable

Clinical presentation and disease course of patients with esophagogastric junction outflow obstruction.

Esophagogastric junction outflow obstruction, characterized by preserved peristalsis in conjunction with an elevated integrated relaxation pressure, can result from specific anatomic variants or may represent achalasia in evolution. There is limited information on the clinical significance of this diagnosis. The aim of this study is to describe the clinical characteristics and outcomes in our cohort of patients with esophagogastric junction outflow obstruction.Consecutive adult patients who had undergone high-resolution esophageal manometry between February 2013 and November 2015 with a diagno

Survival After Myectomy for Obstructive Hypertrophic Cardiomyopathy: What Causes Late Mortality?

BACKGROUND: Survival of patients with obstructive hypertrophic cardiomyopathy who undergo septal myectomy appears to be excellent and, in some series, is similar to that of matched populations. In this study, we investigated causes of late death in a large cohort of surgical patients. METHODS: Between January 1961 and October 2017, 2,956 patients underwent surgery for hypertrophic cardiomyopathy. We reviewed medical records of these patients. Vital status and causes of mortality were verified using 2 methods: LexisNexis Accurint, a commercially available resource, and the National Death Index

Upper esophageal sphincter abnormalities: frequent finding on high-resolution esophageal manometry and associated with poorer treatment response in achalasia.

BACKGROUND: Abnormalities of the upper esophageal sphincter (UES) on high-resolution esophageal manometry (HREM) have been observed in both symptomatic and asymptomatic individuals and are often interpreted as incidental findings of unclear clinical significance. AIMS: Our primary aims were: (1) to assess the frequency of UES abnormalities in consecutive patients referred for HREM studies; and (2) to characterize the demographics, clinical symptoms, and manometric profiles associated with UES abnormalities as compared with those with normal UES function. MATERIALS AND METHODS: We performed a

Symptoms and clinical outcomes of gastric electrical stimulation in treatment-resistant gastroparesis: a systematic review across short- and long-term follow-up.

BACKGROUND: Gastroparesis is a chronic disorder characterized by delayed gastric emptying without mechanical obstruction, significantly impacting patient quality of life. Management of refractory gastroparesis remains challenging, with both gastric electrical stimulation (GES) and prokinetic medications used as treatment options. This systematic review synthesizes the available evidence on GES clinical outcomes, the role of continued prokinetic therapy after GES, and indirect comparative data relevant to symptom management in adult patients with refractory gastroparesis. METHODS: A comprehens

Gastroparesis: A Review.

IMPORTANCE: Gastroparesis is a condition of delayed gastric emptying in the absence of gastric outlet obstruction. Based on a 2018 US administrative health insurance claims database study, prevalence of definite gastroparesis (with documented delayed gastric emptying) was 21.5 per 100 000 persons. OBSERVATIONS: Gastroparesis, which is caused by reduced contractions of the distal stomach or abnormal pyloric relaxation, is more common among females than males (ratio, 2:1-4:1) and typically causes nausea, vomiting, early satiety, bloating, and abdominal pain or discomfort. In a large US epidemio

Congenital esophageal stenosis associated with esophageal atresia: new concepts.

Congenital esophageal stenosis (CES) is suspected by a fixed intrinsic narrowing of the esophagus that affects the normal swallowing mechanism. The diagnosis is only confirmed by histopathologic picture, which may show fibromuscular disease (FMD) or tracheobronchial remnants (TBR). The latter involves ciliated pseudo stratified columnar epithelium, seromucous glands or cartilage each alone or in combination. The aim of this study is to document the usefulness of histologic picture of surgical specimens obtained from the lower esophageal pouch (LEP) during primary repair in detecting cases of C

Can't Stomach It: A Dilated Tale of Achalasia.

Achalasia is a common esophageal motility disorder defined by impaired relaxation of the gastroesophageal junction (GEJ). Complications can occur secondary to achalasia, such as dysphagia, weight loss, chest pain, aspiration, and esophageal rupture. This report discusses a case of massive esophageal dilatation, also known as megaesophagus, in the setting of achalasia. This case highlights the complications secondary to achalasia, such as aspiration, pneumatic cysts, weight loss, dysphagia, and esophageal dilatation, and describes the multidisciplinary approach needed to manage patients with se

Repair of interrupted aortic arch: a ten-year experience.

Eighty-two consecutive patients with interrupted aortic arch were referred to our institution between 1985 and 1995. Three died before any attempt at operation and 79 underwent surgical repair. Median age at operation was 9 days (range 1 day to 6 years) and median weight was 3.0 kg (range 1.8 to 20 kg). All but one were in severe congestive heart failure and 31.5% had oliguria or anuria. Preoperative pH varied between 6.8 and 7.4 (median 7.3). Sixty-nine received prostaglandin E1 infusion and 54 received mechanical ventilation. Aggressive preoperative ressucitation was necessary in 43 cases. P
