Delaying partial stapedectomy for otosclerosis: effects on long-term hearing outcomes following surgery.

The purpose of this study is to evaluate hearing outcomes following partial stapedectomy according to age at the time of surgery and evaluate functional hearing at ≥ 10 years' follow-up, and to assess the effect of delay in surgery on overall functional hearing outcomes. Data were collected from patients operated upon for otosclerosis by a single surgeon from 1990 to 1999. Pure-tone air (PTA), pure-tone bone (PTB), air-bone gap (ABG), air-conduction at 8 kHz (AC 8), bone-conduction at 4 kHz (BC4) and speech discrimination score (SDS) were compared between patients <45 and ≥ 45 years old, as we

An audit of 'dead ear' after ear surgery.

INTRODUCTION: 'Dead ear' is a rare and serious complication of ear surgery. This paper presents an audit of this complication. METHOD: Over 6 years, data for all 617 middle-ear operations performed under the care of a single consultant were recorded for the International Otology Audit. All cases of dead ear were identified and assessed. RESULTS: A post-operative dead ear occurred in 6 cases (approximately 1 per cent). No cases of post-operative dead ear occurred following the 83 otosclerosis operations and the 62 children's procedures. Amongst 187 adult patients undergoing mastoid surgery fo

Surgical and medical treatment of hearing loss in mixed otosclerosis.

OBJECTIVES: We assessed the effect of using preoperative sodium fluoride (NaF) on the difficulty of working with the footplate during stapedectomy and its effect on the postsurgical hearing gain in patients with mixed otosclerosis (i.e., otosclerosis and/or otospongiosis). METHODS: There were 2 groups of patients with mixed otosclerosis. The experimental group was made up of patients with an active focus of the disease; they received NaF for 6 months and then underwent stapedectomy. The control group was made up of patients with an inactive focus of the disease; they underwent stapedectomy wi

Hearing loss with stapedotomy and treated otitis media.

OBJECTIVE: To determine if treated Pseudomonas aeruginosa otitis media (PA OM) increases the risk of sensorineural hearing loss (SNHL) with stapedotomy. STUDY DESIGN AND SETTING: PA OM was induced in 28 guinea pigs. Half of the animals underwent unilateral stapedotomy before antibiotic therapy (untreated), and half underwent stapedotomy after treatment (treated). Control animals underwent saline injections followed by stapedotomy. Electrocochleographic thresholds were measured before and after stapedotomy. Temporal bones were analyzed histomorphologically. RESULTS: Less SNHL was seen after s

Effect of cochlear reserve on postoperative outcome in otosclerosis.

Patients with elevated bone conduction (BC) thresholds are not considered a good candidate for otosclerosis surgery. Sometimes, it might be difficult to decide to operate these patients considering relatively poor cochlear function. However, viewpoints may vary among otologists. This study was undertaken to compare hearing outcome following otosclerosis surgery in patients who had bone conduction (BC) thresholds >or= 30 dB, and to investigate whether BC thresholds >30 dB has a negative impact on hearing outcome. Medical records of 111 patients who had undergone otosclerosis surgery were review

Audit of stapedectomy in the north west of England for 1996 and an analysis of the criteria used to describe success.

A questionnaire was used to collect information related to stapedectomy from all the NHS Trusts in the North West region. The data collected included the identity and rank of the surgeon, the age and sex of the patients, the side of the operation, the audiometric values of the air conduction audiogram preoperatively and postoperatively, and an account of any complications. The results were analysed and compared to national and international standards. The criteria for analysis of success in surgery for conductive hearing loss were reviewed.

Prospective study of resident-performed stapedectomy.

Because of the diminishing number of patients diagnosed with otosclerosis, the adequacy of residency training in stapedectomy techniques remains controversial. A prospective study, conducted from 1986 to 1991, assessed whether or not surgical outcome obtained by residents, under close supervision by otologic faculty, could be improved using a single stapedectomy technique. Comparison of pre- and postoperative hearing results (pure-tone averages of 500, 1000, and 2000 Hz) from 49 cases revealed closure of the air-bone gap to within 10 dB in 68 percent of procedures. Complications included failu

Perilymphatic fistula: a New Hampshire experience.

Thirty-five patients with perilymphatic fistulas (PLFs) are presented. Of this group of 35 patients (39 ears), 4 patients did not have fistulas that could be observed with certainty but were presumed to have fistulas by virtue of their excellent response to surgical repair. Our case reports provide examples of the great variety and possible classifications of presentations and symptom complexes that lead one to suspect the diagnosis of perilymphatic fistula. Comments on diagnostic and therapeutic modalities and on postoperative care and counseling are included. The age range of patients in our

Relationship between short-term and mid-term hearing outcomes after stapedotomy in patients with otosclerosis: an investigation.

BACKGROUND: Although stapedotomy is effective for patients with clinical otosclerosis, the time of hearing stabilization has not yet been consistent. OBJECTIVE: To investigate the relationships between post-operative follow-up times, hearing outcomes, and threshold shift after stapedotomy. MATERIALS AND METHODS: Fifty-five patients with clinical otosclerosis that underwent stapedotomy were retrospectively studied. Pure tone audiometry tests were conducted within the first month (short-term) and within 1 year (mid-term) postoperatively. Data were analyzed for two rounds of audiometry tests at

Hearing outcomes following primary malleostapedial rotation ossiculoplasty in patients undergoing modified radical mastoidectomy.

INTRODUCTION: Treatment of cholesteatoma consists of either excision or exteriorisation of disease. Approaches have traditionally included a radical or modified radical mastoidectomy and combined approach tympanoplasty. Hearing thresholds following a modified radical mastoidectomy alone have been reported as poor. We assessed hearing outcomes in patients undergoing a primary malleostapedial reconstruction combined with their open cavity surgery. METHODS: All patients undergoing open cavity mastoidectomy with primary malleostapedial rotation ossiculoplasty between 2009 and 2013 were identified

Patients' lives following stapedectomy complications.

Nowadays, it is widely accepted that patients must be informed about the risks associated with any type of surgical procedure. Usually, the information provided consists of quoting a list of risks and their probability of occurrence based on data from the literature, and, more appropriately, the figures reflecting the surgeon's personal experience. As a rule, such data are sufficient to hold up in court in the event of a lawsuit, but may be insufficient to help patients make the most appropriate choice. We report two cases that presented complications following stapedotomy. Both cases had a se

Vibrant soundbridge middle ear implant in otosclerosis: technique - indication.

With our growing experience with the Vibrant Soundbridge (VSB) middle ear implant, the question emerged of its indication in mixed hearing loss due to advanced otosclerosis. We describe the VSB implantation technique in primary otosclerosis performed together with a stapedotomy piston procedure. Hearing results under headphone and free-field conditions show that the stapedotomy piston procedure closes the air-bone gap as expected and that the VSB provides comparable gain to that usually recorded for pure sensorineural hearing loss. The gains of the two procedures add up. These results open the
