A comparative study between bone transport technique using Ilizarov/LRS fixator and induced membrane (Masquelet) technique in management of bone defects in the long bones of lower limb.

INTRODUCTION: In earlier times due to difficulty in managing segmental long bone defects, amputation was the preferred treatment. Nonunion with bone loss of long bones is a challenging problem, requiring serious attention. Post-traumatic segmental bone defects can have severe long-term ill impact on patient's lives. Reconstruction is more difficult and functional outcome is usually less satisfactory compared to bony outcome. Distraction osteogenesis and induced membrane technique are the techniques that can be used. AIMS AND OBJECTIVES: To find out and compare clinical, radiological, and func

Mandibular distraction to correct severe non-isolated mandibular hypoplasia: The role of drug-induced sleep endoscopy (DISE) in decision making.

OBJECTIVES: In patients with mandibular hypoplasia, mandibular distraction osteogenesis (MDO) aims to relieve tongue-based airway obstruction. Drug-induced sleep endoscopy (DISE) provides a dynamic assessment of the upper airway and visualizes anatomical site and cause of airway obstruction. The aim of this study was to evaluate the effect of MDO on tongue-based airway obstruction found by DISE within a non-isolated patient population with severe upper airway obstruction (UAO). Furthermore, we aimed to assess the additional value of DISE in clinical decision making by correlating DISE findings

Monofocal bone transport technique for bone defects greater than 5 cm in tibia: our experience in a case series of 24 patients.

BACKGROUND: As the tibial bone defect increases in size, the problems in treatment also increase. The treatment may be problematic but different treatment approaches can be used. Among these approaches, distraction osteogenesis is a method an orthopedic surgeon with limited conditions can use although it has a longer treatment period. In our case series, we evaluated current treatment approaches. METHOD: Retrospective study based on patient records and radiographs. We evaluated our cases with tibial bone defects Type B and greater than 5 cm. Twenty four cases were operated between 1995 and 20

Le Fort II midfacial distraction combined with orthognathic surgery in the treatment of nasomaxillary hypoplasia.

Patients with nasomaxillary hypoplasia have severe facial concavity and compromised skeletal class III malocclusion. Its treatment is still a challenge to surgeons. Our aim was to evaluate the combination of midfacial distraction and orthognathic surgery in the treatment of nasomaxillary hypoplasia. Four patients with nasomaxillary hypoplasia were enrolled in this study. After Le Fort II osteotomy, the rotational distraction of nasomaxillary complex was performed to rehabilitate facial convexity. Then bilateral sagittal split ramus osteotomy with or without Le Fort I osteotomy was used to corr

Posterior vault distraction with midface distraction without osteotomy as a first stage for syndromic craniosynostosis.

Syndromic craniosynostosis (SC) may cause intracranial hypertension, exorbitism, midfacial hypoplasia associated with airway obstruction, and class III malocclusion. Current treatment strategies for SC involve expansion of the cranial vault and orbits, and midfacial advancement to relieve symptoms and sequelae of obstructive sleep apnea. We present a case of a 12-week-old female patient with Saethre-Chotzen syndrome who presented with bicoronal craniosynostosis, severe turribrachycephaly, midface hypoplasia, and a history of obstructive sleep apnea (apnea-hypopnea index = 14). Implant-prosthetic partial-arch restoration in a patient with ectodermal dysplasia characterized by oligodontia and localized bone deficiency: a case report.

Ectodermal dysplasia (ED) is a genetic disorder that is often associated with anomalies of teeth such as hypodontia and oligodontia. Individuals affected by ED have abnormalities of the epithelial tissue of the hair, nails, teeth, or sweat glands. Implant-prosthetic rehabilitation is often necessary because the severity of disease may limit other treatment options. This is a report of a 30-year-old man affected by ED who exhibited severe hypodontia and atrophy of the jaws. Radiographic and clinical evaluations showed inadequate quantity of bone for immediate implant rehabilitation. Osteogeneti

Development of a three-dimensional treatment planning system based on computed tomographic data.

Current surgical treatment planning systems predict three-dimensional (3D) corrections from two-dimensional (2D) data and are inadequate for complex movements. In this paper, we present a 3D planning system based on computed tomographic (CT) data. A three-dimensional CT scan of the craniofacial skeleton forms the database. Software developed in the Harvard Surgical Planning Laboratory was modified for the craniofacial skeleton. Reproducible skeletal landmarks are identified for superimposition. A 'cutting tool' is used to segment the mandible and segments are moved to their predicted positions

Internal craniofacial distraction with biodegradable devices: early stabilization and protected bone regeneration.

Internal craniofacial distraction has several advantages over distraction with external devices. The scars may be less conspicuous; the devices are buried and therefore easier for patients to tolerate; and the devices are more rigidly secured to bone, preventing uncontrolled rotation of the advancing segments. There are several types of internal devices. The Modular Internal Distraction System developed by one of the authors (S.R.C.) and Stryker-Leibinger, Inc. has had widespread use for midface distraction, but requires a substantial surgical procedure for device removal. This is justified in

Management of low lumbar fractures by dorsal decompression, fusion, and lumbosacral laminar distraction fixation.

OBJECT: The authors conducted a study to assess the anatomical appropriateness of using the S-2 dorsal neuroforamina as a hook fixation point, and they present the results of their clinical experience of using a nonscrew alternative for the surgical management of low lumbar (L-4 or L-5) burst fractures. METHODS: The technique used involves lumbar laminar fixation, rod contouring (to preserve lordosis), S- sublaminar wire fixation, S-2 dorsal neuroforaminal hook fixation, cross-fixation, and distraction. Because the S-2 dorsal neuroforamina was used as a unique fixation point, anatomical data

Implant Rehabilitation in Grafted Alveolar Clefts: Clinical and Radiographic Outcomes.

Objective: Implant rehabilitation in grafted alveolar clefts requires a complex staged reconstructive approach, and a clinically important yet underexplored question is which patients require additional pre-implant block regrafting after primary grafting has already been completed. This single-centre retrospective cohort study aimed to evaluate whether a history of maxillary advancement is associated with a reduced likelihood of requiring pre-implant block regrafting (defined here as Re-graft 1), and to describe medium-term implant survival outcomes in a cleft implant cohort. Methods: Forty-tw

Complications and Salvage Management in Ilizarov Reconstruction After Wide Excision for Distal Femoral Giant Cell Tumour: A Retrospective Series of 10 Cases.

Background Wide excision for Campanacci Grade III giant cell tumours (GCT) of the distal femur results in major segmental defects. Where endoprosthetic reconstruction is limited, Ilizarov distraction osteogenesis provides a biologic limb salvage option, though with frequent mechanical and infectious complications. Objectives The objective of this study was to describe clinical outcomes, complication profiles, management strategies, and salvage interventions in 10 consecutive cases of wide resection followed by Ilizarov reconstruction for distal femoral GCT in young adults. Total Hip Replacement and Femoral Nail Lengthening for Hip Dysplasia and Limb Length Discrepancy: A Literature Review.

Developmental dysplasia of the hip (DDH) is a serious condition resulting in inadequate acetabular development, distorted bone configuration, and substantially altered hip biomechanics. An extensive leg length discrepancy (LLD) is commonly encountered in such cases, making a total hip arthroplasty (THA) procedure extremely challenging. Although good results in terms of patients' satisfaction, implant survival rates and overall improved quality of life have been reported, complication rates are considerably higher than primary THA procedures performed for idiopathic osteoarthritis. Non-invasive Ventilation and CPAP Failure in Children and Indications for Invasive Ventilation.

Non-invasive ventilation (NIV) and continuous positive airway pressure (CPAP) are effective treatments for children with severe sleep disordered breathing (SBD). However, some patients may present too severe SDB that do not respond to NIV/CPAP or insufficient compliance to treatment. A careful revaluation of the interface and of ventilator settings should be performed before considering alternative treatments. In patients with obstructive sleep apnea (OSA), alternatives to CPAP/NIV rely on the underlying disease. Ear-nose-throat (ENT) surgery such as adeno-tonsillectomy (AT), turbinectomy or s

Comparative study of the results of compound tibial shaft fractures treated by Ilizarov ring fixators and limb reconstruction system fixators.

PURPOSE: Ilizarov ring fixator and limb reconstruction system (LRS) fixators have been used in the management of complex tibial fractures with severe soft tissue injuries, compound tibial fractures, and infected tibial nonunion for which conventional internal fixation cannot be contemplated. Fracture union and distraction osteogenesis can be done simultaneously with these external fixators, allowing early weight bearing. Several previous studies have shown almost equal results of rail and ring fixators for the compound tibial shaft fractures. Thus we performed a prospective study to evaluate t

Microvascular free flap reconstruction in pediatric lower extremity trauma: a 10-year review.

BACKGROUND: Limb-threatening wounds of the adult distal lower extremity pose a challenge to the microvascular surgeon and are the subject of a sizable body of literature. The microsurgical management of these injuries in the pediatric population has some unique features but has not previously been the subject of a separate study. METHODS: A review was performed of all pediatric patients (<18 years of age) who received lower extremity free flaps for trauma at the University of Kentucky Medical Center between 1992 and 2002. Charts were reviewed and patients were contacted via telephone and give
