Partial Ossicular Reconstruction With a Novel Ball Joint Prosthesis: The mCLIP ARC Partial Prosthesis.

OBJECTIVE: Middle ear surgery involves reconstruction of the ossicular chain, predominately using rigid implants. New middle ear prostheses strive to mimic the physiologic micromovements of the ossicular chain and prevent dislocation, protrusion, and preloading of the annular ligament due to pressure fluctuations. METHODS: Thirty-five patients were included in a monocentric, prospective observational study. Patients received tympanoplasty with ossicular reconstruction using the mCLIP ARC partial prosthesis. This titanium prosthesis is equipped with a clip mechanism for coupling at the stapes

Reducing Radiation Dose for High-resolution Flat-panel CT Imaging of Superior Semicircular Canal Dehiscence.

OBJECTIVE: High-resolution flat panel computed tomography (FPCT) is useful for the evaluation of temporal bone pathologies. While radiation exposure remains a concern, efforts have been devoted to reduce dose, while maintaining image quality. We hypothesize that removing the antiscatter grid (ASG) results in decreased radiation exposure, while maintaining diagnostic image quality for the evaluation of superior semicircular canal dehiscence (SSCD). METHODS: Ten patients with clinical suspicion for SSCD participated in this prospective study. Two sequential collimated 20-second FPCT acquisition

Wideband energy reflectance measurements of ossicular chain discontinuity and repair in human temporal bone.

OBJECTIVE: Recent studies have suggested the potential of wideband energy reflectance (ER) for the assessment of middle ear disorders. The purpose of this study was to examine the use of wideband ER in the evaluation of an ossicular discontinuity and its repair in human cadaver temporal bones. DESIGN: ER measurements at ambient pressure were made on five human cadaver ears in three conditions: (1) baseline with intact ossicular chain, (2) with the ossicular chain cut using an argon laser, and (3) with the ossicular chain repaired. Laser Doppler vibrometry measurements of stapes footplate velo

Indication for surgery in otosclerotic patients with unilateral hearing loss.

OBJECTIVE: The aim of this study is to evaluate postoperative spatial speech discrimination in noise in a group of otosclerotic patients with unilateral hearing loss in a 6-month follow-up. Moreover, an additional objective is to verify if our routine criteria (air-bone gap >30 dB at 250-2,000 Hz) for surgical indication in such patients can be acceptable. STUDY DESIGN: Prospective evaluation in 20 patients divided into 2 groups: unilateral otosclerosis and bilateral otosclerosis already successfully operated on 1 side and planned for stapedotomy on the contralateral ear. Audiological evaluation of patients with otosclerosis.

Even though the diagnosis of otosclerosis is confirmed definitively during surgery, preoperative diagnosis and determination of the indication for surgery are made based on audiological evaluation. Audiological tests should firmly establish a conductive component to hearing loss. The measurement of pure-tone air and bone conduction thresholds has limitations that prevent an accurate diagnosis based solely upon these test results. Such limitations include general variability of threshold measurements, individual variations of tests in the bone conduction mode, and complex interactions between c

Clinical, experimental, and theoretical investigations of the effect of superior semicircular canal dehiscence on hearing mechanisms.

HYPOTHESIS: A superior semicircular canal dehiscence affects hearing by introducing a third window into the inner ear that 1) lowers cochlear input impedance, 2) shunts air-conducted sound away from the cochlea resulting in conductive hearing loss, and 3) improves bone-conduction thresholds by increasing the difference in impedance between the vestibule and the round window. BACKGROUND: Superior semicircular canal dehiscence has been linked to a "conductive" hearing loss characterized by a decrease in the sensitivity to air-conducted sound and hypersensitivity to bone-conducted sound. METHOD

Experience with a new pure gold piston in stapedotomy for cases of otosclerosis.

This study reports the evaluation of the hearing results after 62 implantations of a pure gold (99.9%) piston in cases of otosclerosis. A retrospective analysis of the pre- and postsurgery audiological results of these patients was carried out by microcomputer. This study shows that the use of a pure gold piston ((K-piston) with a diameter of 0.4 mm gives good results in cases of stapedotomy for otosclerosis. The hearing results show a closure of the pure-tone average (PTA) air-bone gap to within 10 dB was 70.9% (44/62) and to within 20 dB in 95.1% (59/62). A residual airbone-gap of > 20 dB wa

Three-dimensional CT-imaging of the ossicular chain.

In eight patients, with a known air-conduction hearing loss, three-dimensional images of the ossicular chain were constructed based on heavily overlapping high resolution CT-slices, obtained from a spiral CT data set. High quality three-dimensional images could be obtained in all eight patients. Supplementary information was derived from these three-dimensional images in three patients; in one, this supplementary information had therapeutic importance. The slight image quality loss in spiral-CT of the temporal bone, compared with conventional CT, is in some cases largely compensated by the pos

[Initial experiences with the dentin-ossicle prosthesis].

The results of 73 implantations performed from 1.7.1986-30.6.1988 are reported in a first follow-up study. 82% reached an air-bone gap of less than 30 dB, 55% of less than 20 dB. In comparison with other materials recently reported the functional results are a little less favourable due to the high percentage of revision surgery in our patients. Only few complications had to be noticed: no single extrusion was observed, but in one case a nearly complete resorption of the DOP was seen. Two DOPs, removed by revision surgery, were examined histologically. Beside resorptive processes, bony changes
