SMART (self- monitoring analysis and reporting technology) and sensor based technology applications in trauma and orthopaedic surgery.

BACKGROUND: Innovations in implant designs and computer technology have led to the development of smart implants and prostheses in the field of orthopedics and trauma. Sensor-guided devices enable close monitoring of physical, chemical and biological environment around the implants, which has been purported to meliorate the intra-operative precision and post-operative surveillance of patients. OBJECTIVE: We evaluate the current applications of sensor-based technology in the management of patients with a spectrum of musculoskeletal conditions. MATERIAL AND METHODS: A thorough search of litera

Comparative Efficacy of Obstructive Sleep Apnea Patients Undergoing Multilevel Surgery Followed by Upper Airway Stimulation Versus Isolated Upper Airway Stimulation.

PURPOSE: Upper Airway Stimulation (UAS) of the hypoglossal nerve is a rapidly growing management option for patients with obstructive sleep apnea (OSA). Our study compares the treatment efficacy of UAS between those who were initially ineligible for UAS but subsequently met eligibility after multilevel surgery versus those who underwent isolated UAS for the treatment of moderate to severe OSA. METHODS: The investigators implemented a retrospective single-center cohort study of patients aged 18+ years who presented for surgical evaluation of OSA from 2016-2019 and underwent UAS implantation. Bone stiffness in children: part II. Objectives criteria for children to assess healing during leg lengthening.

BACKGROUND: The decision when to remove the frame after limb lengthening through standard distraction osteogenesis remains a challenge. Multiple studies have attempted to find objective criteria to assess bone healing after fracture or bone lengthening. However, there is a paucity of such data for the pediatric population. The purpose of this study was to correlate data obtained after dual-energy x-ray absorptiometry (DXA) measurement and bending stiffness in children to find an end-point value for the safe removal of an external fixation device. METHODS: We investigated 16 consecutive childr

Mandibular reconstruction in Goldenhar syndrome using temporalis muscle osteofascial flap.

Goldenhar syndrome is a well-known developmental anomaly of the maxillofacial skeleton and hemifacial soft tissue. Maxillofacial anomalies of that syndrome can be managed by a variety of means such as inlay- or onlay-applied nonvascularized bone grafts, vascularized osteocutaneous/osseous flaps, or distraction osteogenesis. Vascularized full-thickness calvarial bone grafting is an important option for mandibular reconstruction for cases in which, for one reason or another, other techniques are not available, not applicable, or have failed. A mandibular defect of a 6-year-old boy presenting wit

Risk-Stratified Predictive Analysis of Docking Site Outcomes in Lower Extremity Bone Transport: Identifying High-Risk and Low-Risk Zones for Large Segmental Defect Management.

Background: Reconstruction of limbs with extensive bone loss often requires complex surgical procedures, which can be technically demanding, time-consuming, and physically and psychologically burdensome for patients. Historically, the lack of alternatives for large bone defects often led to primary amputation. Modern musculoskeletal practice allows for reconstruction using autologous or allogeneic bone grafts, or through more complex procedures such as the Masquelet technique or distraction osteogenesis. However, these methods share a common challenge: the need for a docking site procedure in

Ankle arthrodesis following bone transport as a definitive limb salvage strategy after multiple failed microsurgical reconstructions for infected tibial pilon fracture nonunion: a case report.

BACKGROUND: Reconstruction of large segmental tibial bone defects remains one of the most challenging problems in orthopedic trauma surgery, particularly when complicated by infection and soft tissue compromise. Various techniques, including vascularized fibular grafting, induced membrane formation, and bone transport, have been described for limb salvage in such complex situations. Bone transport based on distraction osteogenesis enables biological reconstruction of bone defects while simultaneously improving the local biological environment. Treatment of grade 3B open tibia fracture by segmental resection and bone transport: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Open tibial fractures represent the most prevalent type of open long bone fracture, constituting 13.7 % of all open fractures, typically resulting from road traffic accidents and falls from a standing position. The AO Trauma Foundation has developed comprehensive treatment procedures that encompass wound irrigation and debridement, fracture stabilization, and either delayed primary wound closure or early flap coverage. Managing Gustilo IIIB tibial fractures in adults poses problems due to elevated complication rates, increased infection risk, and prolonged union ti

Ridge augmentation with titanium mesh: A case report.

UNLABELLED: Insufficient bone volume for dental implant placement in the maxillary anterior segment is a constant challenge in oral surgery. Several techniques have been suggested to reconstruct deficient alveolar ridges and to facilitate dental implant placement. These techniques include bone splitting osteotomy, distraction osteogenesis, inlay and onlay bone grafting. Guided bone regeneration (GBR) is also a promising alternative that increases the bone volume by the use of a subperiosteal barrier. AIM: The aim of this case was to demonstrate that the use of rigid titanium occlusive barrier

[Treatment of gunshot fractures of the lower extremity: Part 2: Procedures for secondary reconstruction and treatment results].

BACKGROUND: Gunshot wounds of the lower extremities are always serious injuries, especially in cases in which bone is affected. Contamination and extensive tissue damage can be life-threatening for the patient and severely affect the function of the extremity. Contamination and local infections with multidrug resistant pathogens are regularly encountered particularly in casualties evacuated from crisis regions. Treatment of this special type of injury, which differs in the form and extent from conventional high-energy trauma of the lower extremities, usually requires lengthy and extensive ther

Fast and early mandibular osteodistraction (FEMOD) in severe Pierre Robin Sequence.

Pierre Robin Sequence (PRS) is a congenital abnormality characterized by mandibular hypoplasia, glossoptosis and often secondary palate cleft. It may be an isolated or part of a most complicated syndrome. The genetic syndrome that most frequently co-occurs is Stickler syndrome characterized by skeletal abnormalities, joint pain, congenital myopia and retinal detachment. The authors describe their fast and early mandibular osteodistraction (FEMOD) protocol in severe cases of PRS airway obstruction.

Patient characteristics as predictors of clinical outcome of distraction in treatment of severe ankle osteoarthritis.

Osteoarthritis (OA) is a slowly progressive joint disease. Joint distraction can be a treatment of choice in case of severe OA. Prediction of failure will facilitate implementation of joint distraction in clinical practice. Patients with severe ankle OA, who underwent joint distraction were included. Survival analysis was performed over 12 years (n = 25 after 12 years). Regression analyses were used to predict failures and clinical benefit at 2 years after joint distraction (n = 111). Survival analysis showed that 44% of the patients failed, 17% within 2 years and 37% within 5 years after join

Reconstruction of large mandibular and surrounding soft-tissue defects using distraction with bone transport.

Reconstruction of large bone and soft-tissue defects of the inferior third of the face is possible using various surgical techniques. Patients who require these procedures need to be in good general health, may have sequelae linked to donor sites, and require several interventions to achieve good aesthetic and functional results. The aim of this study was to report outcomes in patients with large mandibular and soft-tissue defects treated using osteogenic distraction with bone transport. Between 2001 and 2008, 14 patients had distraction with bone transport. Most patients were men (92.1%). The
