Association of Maternal Risk Factors With First-Trimester Missed Abortion: A Cross-Sectional Study.

Background First-trimester‍‌‍‍‌‍‌‍‍‌ missed abortion, that is, the death of the embryo or fetus without expulsion, is still the leading cause of pregnancy loss in the early stages, both worldwide and in India. The incidence of this event is a cry of demographic, metabolic, obstetric, and lifestyle-related maternal factors, with the respective patterns showing considerable differences from one region to another. Localized evidence is very important for the early warning of at-risk pregnancies. This research describes the distribution of maternal risk factors for the first-trimester missed

Dexmedetomidine-Based Anesthesia for Pediatric Adenotonsillectomy: A Retrospective Observational Case Series.

Background Tonsillectomy is a surgical procedure primarily aimed at the removal of the tonsils, which are lymphoid tissues located at the back of the throat. This procedure is commonly indicated for conditions such as recurrent tonsillitis, chronic tonsillitis, or obstructive sleep-disordered breathing resulting from enlarged tonsils, which is often conducted under general anesthesia. The purpose of this research is to assess the safety and efficacy of dexmedetomidine-based anesthesia with minimal opioid supplementation in children undergoing adenotonsillectomy. Methods We studied pediatric pa

The effect of intraoperative methadone on postoperative opioid requirements in children undergoing orchiopexy: A randomized clinical trial.

BACKGROUND: Children undergoing outpatient surgery are at risk of inadequate postoperative pain control. Methadone has a long duration of action and an intraoperative dose may provide stable analgesia throughout the postoperative period. Intraoperative methadone has been shown to improve pain control in adolescents but its use for postoperative pain in pediatric patients undergoing outpatient surgery has not been studied before. Therefore, we conducted a double-blind randomized placebo-controlled trial to investigate the effects of a single dose of intraoperative methadone in children aged les

Safety and efficacy of esketamine for postoperative analgesia in pediatric patients with hypospadias.

OBJECTIVE: To explore the safety and efficacy of the combination of continuous intravenous infusion of esketamine with sacral block for postoperative analgesia in pediatric patients undergoing surgery for hypospadias. METHODS: Pediatric patients (n = 77) undergoing surgery for hypospadias were randomized into two groups: a hydromorphone group (H group, initial dose, 0.02 mg/kg; maintenance dose, 0.01 mg/kg/h) or an esketamine group (E group, initial dose, 0.3 mg/kg; maintenance dose 0.15 mg/kg/h). Caudal epidural block involved injection of 0.2% ropivacaine 1 ml/kg through the sacral hiatus.

Middle Ear Condition at the Time of Pediatric Myringotomy Tube Placement: Pain Associations Following Intraoperative Fentanyl/Ketorolac and Seasonal Variation.

BACKGROUND: Ketorolac-refractory pain behavior following bilateral myringotomy and pressure equalization tube placement (BMT) is associated with the absence of middle ear fluid. Intraoperative fentanyl/ketorolac affords more reliable pain control than ketorolac alone. We hypothesized that middle ear condition would correlate with postoperative pain despite such combination therapy. We further sought to demonstrate seasonal variation in ear condition and its influence on pain. METHODS: We conducted a single-institution retrospective cohort study of healthy children (9 months-7 years), who unde

A Rare Case of Pontocerebellar Hypoplasia Type 1B With Literature Review.

A full-term female newborn was transferred to our neonatal intensive care unit (NICU) on day two of life for hypotonia. Physical examination was significant for overriding sutures, displaced small anterior fontanelle, axial hypotonia, extremity hypertonia, and slow deep tendon reflexes. She was also noted to have stridor with crying but had unlabored breathing without oxygen requirements and a normal heart examination. A brain magnetic resonance imaging (MRI) showed a large cisterna magna and cerebellar hypoplasia with the majority of the cerebellar vermis present, suggesting a possible Dandy-

Association of a Simplified Finnegan Neonatal Abstinence Scoring Tool With the Need for Pharmacologic Treatment for Neonatal Abstinence Syndrome.

IMPORTANCE: Observer-rated scales, such as the Finnegan Neonatal Abstinence Scoring Tool (FNAST), are used to quantify the severity of neonatal abstinence syndrome (NAS) and guide pharmacologic therapy. The FNAST, a comprehensive 21-item assessment tool, was developed for research and subsequently integrated into clinical practice; a simpler tool, designed to account for clinically meaningful outcomes, is urgently needed to standardize assessment. OBJECTIVES: To identify FNAST items independently associated with the decision to use pharmacologic therapy and to simplify the FNAST while minimiz

Organ donation - "attitude and awareness among undergraduates and postgraduates of North-East India".

BACKGROUND: Organ donation is defined as "When a person allows an organ of theirs to be removed, legally, either by consent while the donor is alive or after death with the assent of the next of the kin." Common transplantations after organ donations include kidney, heart, liver, pancreas, intestines, lungs, bones, bone marrow, skin, and cornea. Although some organs such as kidney and tissues like part of the liver, pancreas, lungs, and intestines can be donated while the donor is alive, most of the donations occur only after the donor's death. In India, the legislative foundation for brain de

A randomized study of local anesthesia for pain control during intra-articular corticosteroid injection in children with arthritis.

BACKGROUND: Intra-articular corticosteroid injections (IACI) are routinely used by pediatric rheumatologists in the treatment of chronic arthritis. Frequently, topical anesthetics are used to control procedural pain, but their relative efficacy has not been reported. In this study, we evaluated the level of pain associated with different anesthetic methods, Numby® 900 Iontophoretic Drug Delivery System, or EMLA® cream, with or without subcutaneous buffered lidocaine (SQBL), during IACI of the knee in children with arthritis. METHODS: We conducted a prospective study of patients, ages 4 to 21

Greater monoamine oxidase a binding in perimenopausal age as measured with carbon 11-labeled harmine positron emission tomography.

IMPORTANCE: Perimenopause is a period of high risk for mood disorders, and it has been proposed that perimenopause is also a window of risk for processes linked to later dementia. However, in human perimenopause, the neurobiological changes implicated in the genesis of mood disorders or dementia have not been identified. Monoamine oxidase A (MAO-A) is an important brain enzyme that creates oxidative stress, influences apoptosis, and metabolizes monoamines. After declines in estrogen level, MAO-A density may be elevated for a month or longer, and repeated declines in estrogen level occur with g

Comparison of sacral erector spinae plane block and caudal epidural block for postoperative analgesia in pediatric hypospadias surgery: a prospective observational study.

BACKGROUND: Effective postoperative analgesia is essential in pediatric urogenital surgery to reduce stress responses and opioid exposure. Caudal epidural block is widely used for hypospadias repair; however, its duration may be limited and it is associated with neuraxial-related concerns. Sacral erector spinae plane (ESP) block has recently emerged as a potential alternative regional technique. This study aimed to compare the analgesic efficacy of sacral ESP block and caudal epidural block in children undergoing hypospadias surgery. METHODS: This prospective, comparative observational study

Vein of Galen Malformation with Pial Arteriovenous Fistula in a 12-month-old Child.

Pial arteriovenous fistulas are extremely rare vascular malformations of the brain (accounting for approximately 1.6% of vascular malformations of the brain) and are classified as Galenic and non-Galenic pial fistulas. We present a rare association of a mural-type vein of Galen malformation with a pial fistula involving the precentral branch of the right middle cerebral artery (MCA) in a 12-month-old infant. A 12-month-old preterm male infant, born vaginally with a delayed cry (birth weight 1.2 kg), presented with progressive macrocephaly, recurrent upper respiratory tract infections, developm

Pediatric simple triage score: A simplified approach for triaging pediatric patients with fever in the emergency department.

INTRODUCTION: The initial 24-h period following admission to a hospital holds profound significance for pediatric patients, representing a critical window where proactive interventions can substantially influence outcomes. We devised a simple triage system, pediatric simple triage score (PSTS), to see whether rapid triage of sick pediatric patients with fever can be done using the new triage system in the emergency department (ED) to predict hospital admission. METHODS: This was a prospective observational study, conducted at the department of emergency medicine of a tertiary care teaching ho
