Ilizarov fixator combined with an intramedullary nail for tibial nonunions with bone loss: is it effective?

BACKGROUND: Treatment of tibial nonunion with bone loss is extremely difficult. A variety of techniques have been described, but each has shortcomings, in particular prolonged external fixation time as well as serious complications such as nonunion and infection. Accordingly, we developed a technique that seeks to reduce these complications by using a circular external fixator in addition to an intramedullary nail to achieve union, limb lengthening, and stability of the regenerated segment. DESCRIPTION OF TECHNIQUE: First, the pseudoarthrosis area is resected, and acute compression is continu

Single-level fixation of flexion distraction injuries.

Flexion distraction injuries of the thoracic and lumbar spine can be stabilized with a short construct spanning one motion segment. This surgical technique has not been well accepted because of the paucity of published outcomes of patients treated in this manner. The current study is a cohort of patients who underwent a standardized posterior open reduction and single-level fixation for this injury pattern. Independent observation prospectively followed the cohort for a minimum of 20 months with functional and radiologic outcomes determined. A significant (p < 0.0001) correction of deformity w

Efficiency of ligamentotaxis using PLL for thoracic and lumbar burst fractures in the load-sharing classification.

The use of pedicle screws for short-segment implants has been known to be dangerous in patients who score a 7 or higher on McCormack's classification. The efficiency of ligamentotaxis of the posterior longitudinal ligament (PLL) and short-segment implants and fusion in relation to McCormack's classification has not been proven. The purpose of this study was to compare the clinical and radiological results of indirect decompression using PLL ligamentotaxis between patients with a high- (score of 7 or higher) or low-grade (score of 6 or less) fracture. Eighteen patients (19 levels) in the low-gr

Femoral bone transport by a monolateral external fixator with or without the use of intramedullary nail: a single-department retrospective study.

BACKGROUNDS: Treatment for bone defect remains a challenge for orthopedists. Bone transport gives an effective alternative, which can be performed with an external fixator alone or combined with an intramedullary nail. Each has its advantages and disadvantages. We present a retrospective study to find out the optimal choice by evaluating the outcomes of treatment for femoral bone defect with two methods. METHODS: Two groups of patients, the monolateral external fixator alone (group A, n = 13) and the monolateral external fixator combined with intramedullary nail (group B, n = 15), were compar

Gradual distraction fronto-orbital advancement with 'floating forehead' for patients with syndromic craniosynostosis.

Eleven patients with syndromic craniosynostosis were treated with gradual distraction fronto-orbital advancement using "floating forehead." The frontal and supraorbital area was cut and remolded. Bony orbits were widened in three patients. Frontal bone was let floating on the dura and fixed loosely with absorbable threads to remolded supraorbital bone. A pair of distracters with hinge plates (A.V.D. system, Bear Medic Corp, Tokyo, Japan) was fixed between the temporal area and supraorbital bone. Distraction was begun 5 to 7 days after the surgery, and 1.8 to 3.2 cm advancement was obtained. Di

INTRODUCTION: Otopalatodigital syndrome type II is a rare X-linked recessive disorder with generalized skeletal dysplasia and hearing anomalies. Its features include conductive hearing loss, unusual facies, cleft palate, micrognathia, and overlapping flexed fingers and toes. It is a more lethal variant of otopalatodigital syndrome type I. There are many consistently reported craniofacial and dental findings; however, no case has been published in dental literature. CASE REPORT: We report a case of otopalatodigital syndrome type II with micrognathia, cleft of the soft palate, and partial anodo

[Treatment concepts and results in non-infected post-traumatic pseudarthroses of the femur and tibia].

AIM OF INVESTIGATION: Different methods of internal and external fixation are used to treat aseptic posttraumatic nonunion of the femur and tibia. The advantages and disadvantages of the different methods will be demonstrated by analysing the clinical course and the outcome of our patients. Utilizing these data, a therapeutic concept tailored to the individual situation is recommended. METHODS: Depending on the form of reaction we distinguish between vital and non-vital nonunions. The classification is made according to the clinical course, x-ray-findings and in special cases the results of s

Oblique Lateral Closing-Wedge Osteotomy for Cubitus Varus in Skeletally Immature Patients.

BACKGROUND: We perform an oblique lateral closing-wedge osteotomy of the distal end of the humerus to correct cubitus varus deformity in children. This deformity is often the consequence of undertreatment, malreduction, or malunion of supracondylar humeral fractures1. Although standard arcs of motion may be altered, cosmesis was traditionally considered a primary surgical indication. However, uncorrected cubitus varus leads to posterolateral rotatory instability of the elbow (PLRI)2, lateral condylar fractures3, snapping medial triceps, and ulnar nerve instability4. Effect of zoledronic acid in an L6-L7 rabbit spine fusion model.

Previous studies have shown that zoledronic acid administration can increase mineral content and strength in distraction osteogenesis. Of the few studies that have examined the use of bisphosphonates in spinal arthrodesis, none have assessed the effect of single dose treatment. The objective of this study was to evaluate the feasibility of enhancing spinal fusion rate using single dose zoledronic acid (ZA) to increase fusion-mass size and mineral density. Forty-eight New Zealand white rabbits underwent an L6-L7 intertransverse process fusion. The L6-L7 model is more challenging than the more c

Treatment of hypertrophic distal tibia nonunion and early malunion with callus distraction.

BACKGROUND: Hypertrophic nonunions and early malunions of pilon and distal tibia fractures result in complex, challenging to treat deformities. Callus distraction histiogenesis is an option for their management, allowing for the simultaneous correction of multiplanar deformity and limb length discrepancy. METHODS: A single-surgeon, retrospective case series was performed. Eight patients (6 males and 2 females) who were treated with callus distraction from 1991 to 2011 were reviewed. Six of 8 patients had varus deformities (range, 8-19 degrees) and 2 patients had valgus deformities (both 16 de

Early midfacial distraction for syndromic craniosynostotic patients with obstructive sleep apnoea.

Syndromic craniosynostosis is known to be associated with various obstructive respiratory disorders, including sleep apnoea. We performed early midfacial distraction in 11 syndromic craniosynostotic patients with obstructive respiratory apnoea and obtained good results. There were four patients with Crouzon syndrome, three patients with Pfeiffer syndrome and four patients with Apert syndrome. Their ages ranged from 7 months to 3 years, 9 months (mean: 2 years, 5 months). Midfacial distraction was performed using an internal or external device to improve obstructive respiratory disorders, such

Multidisciplinary approach to a nonsyndromic oligodontia patient using advanced surgical techniques.

Treatment of a woman suffering from oligodontia and multiple diastemata with insufficient alveolar ridges in both the maxilla and mandible is described in this clinical report. The total number of missing teeth was 11 (excluding the wisdom teeth). The treatment strategy consisted of an interdisciplinary team approach of orthodontic, surgical, and prosthodontic phases. Sinus floor augmentation, alveolar ridge augmentation via vertical alveolar distraction, and lateral augmentation with ramus graft procedures were performed after fixed orthodontic treatment and prior to dental implant placement.

Mapping the Mandibular Lingula in Pierre Robin Sequence: A Guide to the Inverted-L Osteotomy.

BACKGROUND AND PURPOSE: The inverted-L osteotomy for mandibular distraction in Pierre Robin sequence (PRS) is a useful technique for avoiding injury to the tooth root and inferior alveolar nerve. Identification of the lingula is understudied and may decrease iatrogenic complications. This study aims to map the position of the lingula in the micrognathic mandible and compare the location of the lingula in relative normal mandible. METHODS: This is a retrospective cohort study of symptomatic PRS patients. Three-dimensional CT scans were reviewed and the relative lingula position described. RES
