[Clinical features and course of 5 cases with HIV encephalopathy].

Clinical features and courses of 5 cases with HIV encephalopathy were reported. The subjects were among the patients registered as HIV patients at the Nagoya Medical Center, between 1996 and 2005. There were 458 patients with HIV infection including 127 cases of AIDS. All patients suffered from severe immunological deficiency when HIV encephalopathy developed. Other opportunistic infections had also occurred in three patients. HIV encephalopathy was one of the presenting manifestations of HIV infection in four patients, and no patients had received antiretroviral therapy. HAART improved motor

Developments in HIV neuropathogenesis.

Despite the fact that neurons are rarely infected by the human immunodeficiency virus (HIV), neuronal loss is common in patients with HIV infection, likely due to the effects of viral proteins and inflammatory mediators on these cells. Despite the widespread use of highly active antiretroviral therapy (HAART), at least in developed nations, cognitive impairment and other neurological complications of HIV infection persist with devastating personal and socioeconomic consequences. Fortunately, we have made important advances in recent years in defining the molecular mechanisms by which HIV infec

Impaired performance on the object retrieval-detour test of executive function in the SIV/macaque model of AIDS.

NeuroAIDS, the neurological, motor, and cognitive impairments that occur in acquired immunodeficiency syndrome (AIDS) patients, is characterized by compromised function in frontal cortical and subcortical brain regions including impairments in motor control, reaction time, and executive functions. Executive function is a cognitive domain involving the regulation of behavior, including inhibitory control. The present study evaluated the effects of simian immunodeficiency virus (SIV) infection on the object retrieval detour (ORD) task to assess inhibitory control. The ORD task measures the abili

Patterns of neuroAIDS in Africa.

According to UNAIDS, the African population accounts for greater than half of persons infected with HIV. Nevertheless, little information exists characterizing HIV in this population. Thus, the natural history and progression of HIV in the African population is virtually undocumented and therefore, poorly understood. Information regarding virtually every aspect of the disease including microbiology, pathogenicity, virulence, and clinical manifestation is based largely on data from select and limited populations. During the HAART-era, we have seen dramatic and significant changes in patterns of

Cardiomyopathy and encephalopathy in AIDS.

HIV encephalopathy has been in the past years the most typical CNS disorder in patients with AIDS. Histologic abnormalities consist in astrocytosis, myelin pallor, infiltration by infected macrophages, resident microglia and multinucleated giant cells, generally in absence of direct infection of neurons. Mononuclear phagocytes in the brain are the main target of HIV-1 infection and the site of productive viral replication, and viral stimulation leads to the release of neurotoxic products causing neurologic damage. Subclinical cardiac abnormalities are common in HIV+ patients and several studie

The burden of care: the impact of HIV-associated dementia on caregivers.

It has been suggested that up to 15% of patients with AIDS may develop HIV-associated dementia. The syndrome may be either abrupt or insidious and is characterized by poor prognosis. Increasing cognitive impairment will necessitate the patient receiving a substantial amount of care and support in the community in addition to medical treatment and periods of hospitalization. The impact of caregiver burden is reasonably well documented in Alzheimer's disease, but there is a dearth of literature relating to caregiving and HIV-associated dementia. The current investigation is an observational stud

Relevance of glutamate levels in the CSF of patients with HIV-1-associated dementia complex.

Chronic hyperactivation of excitatory amino acid pathways in the CNS of patients infected with HIV-1 may contribute to the pathogenesis of HIV-1-associated dementia complex. However, no correlation between the concentration of glutamate in CSF (mean 3.3 micromol/L) and either HIV-1 infection or HIV-1-associated dementia complex was observed. The results clarify several important issues regarding analysis of glutamate in the CSF and the role of excitotoxins in HIV-1-associated dementia complex.

The regulation of quinolinic acid in human immunodeficiency virus-infected monocytes.

Quinolinic acid (Quin) is thought to underlie cognitive and motor dysfunctions for a variety of neurological disorders. Specifically, in human immunodeficiency virus (HIV)-associated dementia, Quin levels correlate with the degree of neurological dysfunction observed in affected individuals. Since recent data from our laboratories suggest that both HIV-1 infection and activation of brain macrophages are required for the development of neurotoxicity we examined Quin production during virus infection and immune activation. HIV-1 infection of monocytes induced low levels of Quin while lipopolysac

The clinical spectrum and pathogenesis of HIV encephalopathy, myelopathy, and peripheral neuropathy.

Human immunodeficiency virus infection commonly affects the nervous system, with the most important features being a dementing illness, a myelopathy, and a neuropathy. In the past year, important advances have been made in the epidemiology and treatment of these conditions. Moreover, a better understanding of the pathogenetic mechanisms has emerged, with particular emphasis being placed upon gp120, tumor necrosis factor, and quinolinic acid and other partly characterized neurotoxins. This review highlights these advances in the context of the clinical syndromes.

Neurogenic and psychogenic behavioral correlates of HIV infection.

In this chapter, we have suggested that neurobehavioral phenomena in HIV-infected individuals can be usefully grouped according to underlying pathogenesis. Neurogenic phenomena are those that result from direct involvement of the brain either by HIV-1 (primary neurogenic disorder) or from opportunistic infection, neoplasia, or side effects of treatment (secondary neurogenic disorder). These phenomena should be distinguished from disturbances resulting from a second pathogenetic mechanism--the psychogenic disorders. We define psychogenic disorders as those that reflect primarily psychological r

Localized cerebral proton MR spectroscopy in HIV infection and AIDS.

PURPOSE: To document differences in the cerebral proton MR spectra of patients with early and late stages of human immunodeficiency virus (HIV) infection. METHOD: We studied the relative N-acetyl-aspartate (NAA) levels by localized proton spectroscopy of the parietooccipital region of the brain in 43 HIV-seropositive patients, including 26 with an acquired immunodeficiency syndrome (AIDS)-defining diagnosis, and in eight control subjects. RESULTS: Reduced relative NAA levels were shown in those HIV-1-seropositive patients: 1) with AIDS against HIV-1-seropositive patients without AIDS (P < .0

Incidence of the WAIS-R Fuld profile in HIV-1 infection.

The incidence of a WAIS-R subtest "marker" sensitive to cholinergic dysfunction was assessed in a sample 116 homosexual males infected with HIV (Acquired Immunodeficiency Syndrome [AIDS] N = 40; AIDS Related Complex [ARC], N = 76). The incidence of positive profiles was low in the overall sample (11/116, 9%), and significantly lower than incidence rates reported for known cholinergic deficient groups (Alzheimer's disease; scopolamine). However, significantly more AIDS patients (8/40, 20%) than ARC patients (3/76, 4%) demonstrated positive profiles. These results suggest that, as a group, perso

Risk factors for HIV-associated neurocognitive impairment and frailty in aging people with HIV in Latin America: a systematic review.

BACKGROUND: As people living with HIV (PWH) in Latin America and the Caribbean (LAC) have longer life expectancy due to improved access to antiretroviral therapy (ART), complications such as HIV-associated neurocognitive impairment (NCI; including HIV-associated neurocognitive disorder [HAND]) and frailty are increasingly more prevalent. We aimed to identify the risk factors for neurocognitive impairment and frailty among aging PWH in LAC. METHODS: We conducted a systematic review following PRISMA guidelines. We searched five databases that reported on NCI, HAND or frailty in PWH in LAC. Eligi

HIV-Associated Neurocognitive Disorder: A Look into Cellular and Molecular Pathology.

Despite combined antiretroviral therapy (cART) limiting HIV replication to undetectable levels in the blood, people living with HIV continue to experience HIV-associated neurocognitive disorder (HAND). HAND is associated with neurocognitive impairment, including motor impairment, and memory loss. HIV has been detected in the brain within 8 days of estimated exposure and the mechanisms for this early entry are being actively studied. Once having entered into the central nervous system (CNS), HIV degrades the blood-brain barrier through the production of its gp120 and Tat proteins. These protein

Imaging of Brain Structural and Functional Effects in People With Human Immunodeficiency Virus.

Before the introduction of antiretroviral therapy, human immunodeficiency virus (HIV) infection was often accompanied by central nervous system (CNS) opportunistic infections and HIV encephalopathy marked by profound structural and functional alterations detectable with neuroimaging. Treatment with antiretroviral therapy nearly eliminated CNS opportunistic infections, while neuropsychiatric impairment and peripheral nerve and organ damage have persisted among virally suppressed people with HIV (PWH), suggesting ongoing brain injury. Neuroimaging research must use methods sensitive for detectin

BACKGROUND: Human Immunodeficiency Virus (HIV) infection causes a myriad of neurological complications including cognitive deficits referred to as HIV-Associated Neurocognitive Disorders (HAND). With the introduction of combination antiretroviral therapy, there has been an epidemiological shift in cognitive disorders with a decline in the more severe HIV-Associated Dementia (HAD) to an increase in the less severe HAND: Asymptomatic Neurocognitive Impairment (ANI) and HIV-associated Mild Neurocognitive Disorder (MND). Central Nervous System (CNS) involvement in HIV interferes with cognitively d

Evolving clinical phenotypes in HIV-associated neurocognitive disorders.

PURPOSE OF REVIEW: To describe the changes in the presentation of HIV-associated neurocognitive disorders (HAND) comparing the current combination antiretroviral therapy (cART) treatment era to the pre-cART era. RECENT FINDINGS: The frequency of the most severe stage of HAND, HIV-associated dementia (HAD), has decreased, but the frequencies of milder stages of HAND, asymptomatic neurocognitive impairment (ANI) and mild neurocognitive disorder, have increased. In the pre-cART era, HAD was a progressive disorder leading to death within months. With cART, HIV+ individuals with HAND frequently re
