BACKGROUND: The stimulus-atrial (SA) interval minus ventriculoatrial (VA) interval (SA-VA) difference represents a simple diagnostic maneuver to distinguish between atrioventricular nodal reentrant tachycardia (AVNRT) and orthodromic reentrant tachycardia (ORT) during electrophysiology study. However, its usefulness has largely been studied in selected patient subgroups. The purpose of this study was to evaluate the performance of the SA-VA difference against commonly used diagnostic maneuvers in a large cohort of consecutive patients.
METHODS: Consecutive patients with inducible supraventric

A 21-year-old woman had paroxysmal wide QRS tachycardia with a left bundle branch block configuration and a retrograde conducted P wave just behind the QRS complex. An electrophysiological study revealed antidromic atrioventricular tachycardia involving an atrioventricular connection with decremental conduction as the anterograde limb and normal atrioventricular node as the retrograde limb. During constant pacing from the high right atrium (HRA) at the cycle length (CL) of 600 ms, the QRS configurations were not identical to those during the wide QRS tachycardia or constant pacing at the CL of

BACKGROUND: Catheter ablation (CA) is a curative therapy for atrioventricular reentrant tachycardia caused by accessory pathways (APs), but precise AP localization with conventional mapping can be challenging. Although omnipolar technology near field (OTNF) provides novel frequency-based analysis, its clinical utility for AP ablation remains unproven.
OBJECTIVE: We aimed to characterize the spatial relationship between high-frequency areas identified by OTNF-derived peak frequency (PF) maps and successful AP ablation sites.
METHODS: This multicenter retrospective study analyzed OTNF data fro

PURPOSE: Ablation of the persistent left superior vena cava (PLSVC) with a left accessory pathway is more difficult than ablation in the case of an ordinary left accessory pathway. We report a case of successful ablation of the PLSVC with a left double accessory pathway. We also conducted a systematic review of the literature to summarize the procedural precautions and technical points for patients with PLSVC with a left accessory pathway.
OBSERVATION: A 48-year-old man was admitted with recurrent episodes of tachycardia. Electrocardiography revealed preexcitation. Preoperative echocardiograp

The present case describes a dilated cardiomyopathy associated with both antidromic and orthodromic atrio-ventricular reentrant tachycardias supported by multiple right accessory pathways. Both right accessory pathways were successfully eliminated by catheter ablation and the patient progressively recovered during the follow up. The following etiologies might be involved: 1) primitive dilated cardiomyopathy (or post-inflammatory); 2) septal dyssinchrony due to ventricular pre-excitation; 3) tachycardiomyopathy.

Wolff-Parkinson-White syndrome is a congenital cardiac pre-excitation syndrome that is effectively treated by ablating the accessory pathway. However, accessory pathways located in the posteroseptal region can sometimes be challenging. In this paper, we present the successful ablation of the epicardial posteroseptal accessory pathway through the middle cardiac vein in a 13-year-old girl with a coronary sinus diverticulum and Wolff-Parkinson-White syndrome, after unsuccessful ablation attempts at different locations. If the ablation procedure fails, the possibility of the posteroseptal pathway

OBJECTIVES: The purpose of this study was to evaluate long-term safety and efficacy of catheter ablation of accessory atrioventricular pathways (AP) in a pediatric cohort.
BACKGROUND: Radiofrequency catheter ablation of accessory AP is the recommended treatment for patients with atrioventricular re-entrant tachycardia. Data on long-term results ≥1 year after AP ablation in pediatric patients is sparse.
METHODS: A total of 296 patients <18 years of age who had undergone radiofrequency-AP ablation between October 2002 and June 2015 were included into the study. Follow-up was >1 year in all pat

BACKGROUND: Atrial flutter (AFl) accounts for up to one third of all fetal tachyarrhythmias and can result in premature delivery, hydrops, and fetal death in 10% of cases; however, the electrophysiology of AFl in utero is virtually unstudied.
METHODS AND RESULTS: In this observational study, we reviewed 19 fetal magnetocardiography studies from 16 fetuses: 15 fetuses (21-38 weeks' gestation) referred with an echocardiographic diagnosis of AFl and 1 fetus (20 weeks' gestation) referred with a diagnosis of tachycardia that was shown by fetal magnetocardiography to have transient AFl in addition

BACKGROUND: Differentiation between atrioventricular nodal re-entrant tachycardia (AVNRT), atrioventricular re-entrant tachycardia (AVRT), and atrial tachycardia (AT) is often challenging during electrophysiology studies. This study compared the sensitivity and specificity of identifying anterograde His bundle activation during entrainment with commonly used right ventricular (RV) pacing maneuvers to differentiate between these types of supraventricular tachycardia (SVT).
METHODS: Out of 112 consecutive patients with SVT, 90 (36 males [40%], age 37 ± 16 years) were prospectively studied. Afte

Mahaim fibers that exhibit decremental atrioventricular (AV) node-like conduction properties are unusual, comprising < 3% of accessory pathways. They are involved in antidromic AV reciprocating (Mahaim) tachycardia with a left bundle branch block morphology. There is a frequent association of Mahaim pathways with Ebstein's anomaly, additional AV accessory pathways (Kent) and dual AV nodal pathway conduction. The majority of these pathways are long right atriofascicular pathways capable of only anterograde conduction. Radiofrequency (RF) ablation of right atriofascicular pathways guided by a di

Interventional cardiorhythmology was born with the invention of electrical catheter ablation of the common atrioventricular bundle of His as a palliative treatment of supraventricular arrhythmias refractory to medicinal treatment. This method is now used as a curative treatment. In Wolff-Parkinson-White syndrome, all accessory pathways, whatever their location, can be destroyed with a very high success rate (96 p. 100) and very low morbidity and mortality rates. Reentrant nodal tachycardias can also be treated by catheter ablation with, however, a low risk of atrioventricular block which, for

An 8 year-old boy had extensive electrophysiological evaluation of his recurrent supraventricular tachycardias. His ECG never showed delta waves but intracardiac stimulation and recording disclosed the following (1) eccentric retrograde atrial activation; (2) increased cycle length and retrograde conduction time following the development of right bundle-branch block; (3) constant retrograde conduction time for increasingly premature ventricular stimuli; (4) atrial captures by ventricular stimuli when the atrioventricular-His pathways were refractory; and (5) no delta waves upon stimulation of

BACKGROUND AND PURPOSE: Despite available criteria, diagnosis of the mechanisms of wide complex tachycardia is often incorrect. We aimed in this study to identify reasons for misdiagnoses and the value and limitations of clinical and surface electrocardiographic criteria.
DATA IDENTIFICATION: The analyzed data of 150 consecutive patients with wide QRS tachycardia from this study and a literature search of key papers in English since 1960 on clinical and surface electrocardiographic criteria form the basis of this report. The final correct diagnosis was made with intracardiac electrograms.

The purpose of this study was to evaluate the value of esophageal programmed stimulation in children and teenagers with normal sinus rhythm ECG and normal noninvasive studies, having palpitations and syncope, and no documented tachycardias. Paroxysmal tachycardias are frequent in children and are often related to accessory connection. These tachycardias are sometimes difficult to prove. Transesophageal atrial pacing was performed at rest and during infusion of isoproterenol in 31 children or adolescents aged 9-19 years (16 +/- 3 years) with normal sinus rhythm ECG and suspected or documented e

BACKGROUND: Mitral isthmus block (MIB) complicating radiofrequency ablation (RFA) of orthodromic reciprocating tachycardia (ORT) using left - sided accessory pathways (APs) is poorly understood.
METHODS: Two cases and a systematic review of the literature of patients (pts) who developed MIB complicating left - sided ORT RFA is presented.
RESULTS: Among 27 pts (34 ± 12 years old, 54% female, 68% concealed AP), 15 (56%) had ≥ 1 failed RFA procedure. One RF lesion caused MIB in 6 (22%) (≤ 3 lesions in 11 (41%)). MIB caused switch from eccentric to pseudo-concentric atrial activation (23/27 (85%

Danondisease is a rare genetic disorder with an X-linked dominant inheritance affecting both skeletal and cardiac muscle. Its characteristic cardiac phenotype consists on a severe, non-obstructive and concentric hypertrophic cardiomyopathy (HCM) usually associated with a Wolff-Parkinson-White (WPW) type preexcitation pattern. Whether this corresponds to the presence of an AV or another type of accessory pathways, such as fasciculoventricular pathways (FVP) remains controversial in the literature. However, we describe the case of a teenager with Danon disease and preexcitation who develops a fi

An accessory pathway (AP) can be apparent during sinus rhythm if it depolarizes part of the ventricles ahead of the normal wave front from the conduction system. An AP can generate an anatomic circuit able to sustain a macroreentrant atrioventricular reentrant tachycardia. This arrhythmia can engage the normal conducting system in an antegrade direction or retrogradely, generating, respectively, a narrow or a wide complex tachycardia. The combined use of a standard electrocardiogram and an esophageal recording-pacing can be particularly useful in the first approach to patients with Wolff-Parki
