Identification and Characterization of Oral Injury in Suspected Child Abuse Cases: One Health System's Experience.

OBJECTIVES: Accurately differentiating inflicted from accidental injury in infants and toddlers is critical. Many studies have documented characteristics of inflicted bruises, fractures, and head injuries facilitating the development of clinical tools. There are few studies characterizing inflicted oral injuries, and no clinical tools exist. This study identified characteristics that differentiated inflicted from accidental oral injuries in children younger than 24 months. METHODS: Retrospective review using International Classification of Diseases, Ninth Revision billing codes and an interna

Genital injuries in prepubertal girls from inline skating accidents.

We report 2 cases where accidents from inline skating resulted in significant genital injury to female children. When genital trauma occurs in children, it frequently raises the concern that sexual abuse might have caused the injury. Historical information provided by the child and others must be compared with the injuries to understand whether an injury may be the result of abuse. We characterize the nature of the genital injuries of our cases as being caused by an accidental "rapid-splits" type mechanism while children were using inline skates. We conclude that the reported accidental mechan

[Gynecologic injuries, management].

Accidental trauma to the female perineum is relatively rare and occurs most often in the 4- to 12-year-old age group. Vulva and vaginal trauma are the result of straddle injuries, accidental penetration, intercourse, sexual abuse and motor vehicle accidents. Injuries to the genitalia require typical surgical repair and, in association with anogenital or urogenital injuries, a multidisciplinary approach.

Contact sports and injury.

A year long survey of sports injuries in the accident and emergency department of St. Luke's Hospital, Kilkenny revealed 864 injuries, with hurling and soccer accounting for 41% and 23% respectively. In hurling, lacerations to face and scalp, and fractures of the wrist and hand, accounted for a large number of the injuries seen, while in soccer bruising and sprains of the lower limbs accounted for the majority of the injuries. We make some recommendations for avoidance of these injuries. We show clearly that non-accidental or deliberately inflicted injury forms a significant proportion of spor

Single stab injuries.

Determining the manner of death in cases involving multiple stab injuries from a knife is generally straightforward. The medico-legal investigation of a stabbing death caused by a single stab injury from a knife comprises a smaller but potentially more problematic subset of forensic cases. We reviewed our institute's experience with single stab injuries and endeavored to identify features identified at the post-mortem examination which may aid in the differentiation between cases of homicide, suicide and accidental death. The single stab injury was to the left chest in the majority of deaths f

Rare retinal haemorrhages in translational accidental head trauma in children.

PURPOSE: The characteristic findings in accidental head injury consist of linear skull fracture, epidural haematoma, localized subdural haematoma, or cortical contusion because of a linear or translational impact force. Retinal haemorrhages have been found, although uncommon, in accidental head trauma. METHODS: We performed a retrospective study of 24 consecutive cases of children with severe head injuries caused by falls. Inclusion criteria were skull fractures and/or intracranial haemorrhages documented by computerized tomography. All patients underwent a careful ophthalmic examination incl

A proclaimed accidental fall of an infant-an experimental case reconstruction study.

The differentiation between a non-accidental injury and injuries resulting from accidents, such as falls to the ground or onto various objects, is a challenge not only for forensic experts but also for all clinicians caring for children. The forensic-biomechanical analysis of accidents in infants aims at the assessment of the loading characteristics based on the reconstruction of the particular incident. It includes an evidence-based analytical comparison between the actual injuries presented and the injuries that should be expected as a result of the proclaimed accident. With the help of kine

Hair straightener burns in children: An experimental study originating from 2 cases.

Burns in children are a topic of fundamental importance in the context of the differential diagnosis between accidental events and abuse. In this context, the present study arose from two cases of children with second-degree burns (in case 1 on the leg, in case 2 on the foot) caused by contact with the hot surfaces of a hair straightener, which were reported as accidental events but trough the complete medico-legal investigation later turned out to be abusive burns. With an experimental set on pig skin and by simulating the two anatomical districts (leg and foot), using the same type of hair s

Accidental puncture or laceration in colorectal surgery: a quality indicator or a complexity measure?

BACKGROUND: Accidental puncture or laceration during a surgical procedure is a patient safety indicator that is publicly reported and will factor into the Centers for Medicare and Medicaid's pay-for-performance plan. Accidental puncture or laceration includes serosal tear, enterotomy, and injury to the ureter, bladder, spleen, or blood vessels. OBJECTIVE: This study aimed to identify risk factors and assess surgical outcomes related to accidental puncture or laceration. DESIGN: This is a retrospective study. SETTINGS: This study was conducted in a single-hospital department of colorectal su

Healing of genital injuries.

Child sexual abuse as well as accidental trauma may cause acute injuries in the anogenital area. Most data on residual findings following genital trauma come from longitudinal studies of children who have been sexually assaulted, undergone surgical procedures, or experienced accidental trauma. Like injuries in other part parts of the body, such injuries undergo a predictable pattern of healing. Most superficial injuries heal without any residual evidence. Deeper injuries, as well as those that become infected or experience repeated disruption, may produce permanent changes. While the presence

Death due to severe blood loss following an accidental lesion to the femoral vessels.

Deaths from sharp force injuries are a significant cause of violent death. While homicides and suicides caused by sharp force wounds are common, accidental injuries are less often described in forensic literature, so their actual incidence or prevalence is not known. This paper presents an accidental death due to massive blood loss which resulted from lesions to the left femoral vessels by glass fragments. The body was found in a pool of blood surrounded by hundreds of glass fragments. Autopsy revealed a near-total transection of the superficial femoral artery, and two wounds of the superficia

[Oral injuries in cases of child abuse].

Child abuse is more common than previously supposed, and non-accidental injuries may also present in the oral cavity. Results of physical abuse are most commonly diagnosed as such, but injuries from sexual abuse and results of neglect may be observed in the mouth. Bruises are the most common types of non-accidental injuries, the lips and frena being specially exposed, through burns may be cause by over-hot food or caustic liquids. The teeth may fracture, be avulsed, displaced or discoloured as a result of non-accidental injury. Sexually transmitted diseases in children's mouths are strong indi

[Craniocerebral trauma in childhood].

This article describes typical head injuries in infants and children. In comparison with adults there are distinct differences in the etiology of trauma and in the kind of reaction of the skull and brain. In infants and children there are three different types of trauma: birth trauma, accidental and non-accidental injury. The typical injuries in these three groups are described.

Acute Computer Tomography Findings in Pediatric Accidental Head Trauma-Review.

Head trauma in paediatric patients is a worldwide and constant issue. It is the number one cause for childhood mortality and morbidity. Children of all ages are susceptible to sustaining head trauma and the anatomical characteristics of the region put them in a high-risk category for developing severe traumatic brain injuries. Boys are more frequently victims of accidental head traumas, and their injuries are more severe than those encountered in girls. The mechanisms of the trauma are a determining factor for the types of lesions we find. The traumatic injuries fall into two categories, prima

Perineal impalements in children: distinguishing accident from abuse.

UNLABELLED: Perineal impalements are uncommon and potentially life-threatening injuries. Medical providers must evaluate the risk of child abuse in all cases of genital or anal trauma. Determination of abuse depends on medical assessment of the mechanism of injury and statements by the child and witnesses, and may require collaboration with police or child protection agencies for scene investigation. OBJECTIVE: To describe circumstances, medical findings, and child protection issues raised in accidental pediatric perineal impalement injuries. METHODS: Retrospective case series selected from

Penetrating Head Injury With a Motor Bike Key.

Traumatic head injuries can be penetrating and non-penetrating. Penetrating head injuries are usually caused by low to high velocity, either with sharp or blunt objects. Penetrating objects may include knives, nails, forks, scissors, pencils, and even keys. Patients with penetrating brain injuries are mostly involved in road traffic accidents, assault injuries, accidental injuries, or even suicide attempts. This case report focuses on an accidental penetrating brain injury due to a bike key to a 21-year-old male near the left orbital wall. The patient underwent left lateral wall orbitotomy wit

Bilateral Parietal Skull Fractures in Infants Attributable to Accidental Falls.

INTRODUCTION: Multiple skull fractures, including bilateral parietal skull fractures (BPSFs) in infants are considered to be suspicious for abusive head trauma (AHT). The aim of this report is to describe a series of BPSF cases in infants which occurred due to accidental falls. METHODS: We searched our neuroradiology database for BPSF in infants (<1 year old) diagnosed between 2006 and 2019; we reviewed initial presentation, mechanisms of injury, clinical course, head imaging, skeletal survey X-rays, ophthalmology, social work and child abuse physicians (CAP) assessments, and long-term follow

Extra-axial haemorrhages in young children with skull fractures: abuse or accident?

OBJECTIVE: Infant and toddler subdural haemorrhages (SDH) are often considered indicative of abuse or major trauma. However, accidental impact events, such as falls, cause contact extra-axial haemorrhages (EAHs). The current study sought to determine frequency and clinical behaviour of EAHs with infant and toddler accidental and abusive skull fractures. PATIENTS AND METHODS: Children aged <4 years with accidental skull fractures and abusive fractures identified by CT at two paediatric tertiary care centres. Clinical data were abstracted by child abuse paediatricians and images were reviewed b

The true cost of child abuse at a level 1 pediatric trauma center.

BACKGROUND: Child physical abuse (CPA) is a significant cause of morbidity and mortality. Children who sustain CPA consume significant healthcare resources. We hypothesized that the costs to care for children who sustain for children with CPA-type injuries are greater than the costs to care for children who sustain accidental injuries. METHODS: All confirmed CPA patients between the ages of 0 and 19 years old, who were admitted to a level 1 pediatric trauma center between January 2010 and September 2018, were retrospectively reviewed. We compared outcomes, including mortality, length of stay
