Cardiac Arrest in Special Circumstances-Recent Advances in Resuscitation.

BACKGROUND: Cardiopulmonary resuscitation (CPR) in special circumstances includes the emergency intervention for special causes, special environments, and special patients. Special causes cover the potential reversible causes of cardiac arrest that must be identified or excluded during any resuscitation act. The special environments section includes recommendations for the treatment of cardiac arrest occurring in specific locations: cardiac surgery, catheterization laboratory, dialysis unit, dental surgery, commercial airplanes or air ambulances, playing field, difficult environment (eg, drown

Bone Cement Implantation Syndrome Causing Intraoperative Cardiac Arrest in a Nonagenarian: A Case Report.

Bone cement implantation syndrome (BCIS) is a rare but potentially fatal complication of cemented orthopedic procedures, most commonly hip hemiarthroplasty in elderly patients. It is characterized by acute cardiovascular and respiratory compromise that can progress to cardiac arrest. We present the case of a 90-year-old woman with long-standing hypertension who sustained a left intertrochanteric femoral fracture following a fall. She was scheduled for cemented hemiarthroplasty and underwent spinal anesthesia. Approximately five minutes after insertion of polymethylmethacrylate bone cement, she

Fatal Neonatal Cardiac Rhabdomyoma Presenting With Pulmonary Hypoplasia: An Autopsy Case Report From Jamaica.

Cardiac rhabdomyomas are the most common primary cardiac tumors in infants and are frequently associated with tuberous sclerosis complex. Although typically benign with a tendency for spontaneous regression, large lesions may result in significant morbidity and mortality. We report the case of a preterm female neonate in whom a large cardiac mass was detected on third-trimester ultrasound, accompanied by pleural effusion, lung hypoplasia, and ascites. At birth, the infant presented with severe respiratory distress, bradycardia, and hypoxia requiring ventilatory support, but despite initial sta

Cardiac Rehabilitation in Patients After Coronary Artery Bypass Grafting: Core Components and Long-Term Follow-Up.

Cardiac rehabilitation is strongly recommended in secondary cardiovascular prevention. In patients after coronary artery bypass grafting, this intervention is suggested to reduce mortality, morbidity, and disability. In addition, rehabilitation programs improve quality of life and cardiorespiratory fitness. Modern cardiac rehabilitation programs include structured exercise training, education, nutritional counseling, psychosocial support, and management of cardiovascular risk factors, each tailored to the specific needs of post-coronary artery bypass grafting patients who often face a high bur

Cardiac laceration and pericardial tamponade due to cardiopulmonary resuscitation after myocardial infarction.

Complications of cardiopulmonary resuscitation (CPR), such as rib fractures and pneumothorax, are not uncommon. The authors report the case of a 69-year-old woman who underwent surgery for a perforated duodenal ulcer. Eighteen hours postoperatively she sustained a cardiac arrest; vigorous resuscitation efforts, using advanced cardiac life-support procedures, failed. At autopsy, she had 350 mL of fresh blood in her pericardial sac, which had caused cardiac tamponade. Three ribs were fractured at the left sternal border. Directly underneath the fractured ribs were a 0.4-cm laceration of the peri

Comparison of hemodynamic effects and resuscitation outcomes between automatic simultaneous sterno-thoracic cardiopulmonary resuscitation device and LUCAS in a swine model of cardiac arrest.

INTRODUCTION: Mechanical cardiopulmonary resuscitation (CPR) devices are widely used to rescue patients from cardiac arrest. This study aimed to compare hemodynamic effects and resuscitation outcomes between a motor-driven, automatic simultaneous sterno-thoracic cardiopulmonary resuscitation device and the Lund University cardiac arrest system (LUCAS). MATERIAL AND METHODS: After 2 minutes of electrically induced ventricular fibrillation (VF), Yorkshire pigs (weight 35-60 kg) received CPR with an automatic simultaneous sterno-thoracic CPR device (X-CPR group, n = 13) or the Lund University ca

Procedural circulatory support-enabled intracoronary cell delivery in advanced dilated cardiomyopathy.

Advanced heart failure due to dilated cardiomyopathy (DCM) has limited treatment options. Although intracoronary autologous cell therapy may improve cardiac function, its use in patients with severe left ventricular dysfunction is limited by procedural risk. This exploratory study investigated whether temporary mechanical circulatory support with an Impella device could enable safer therapy delivery. Twenty patients with DCM and a left ventricular ejection fraction (LVEF) of ≤35% received granulocyte colony-stimulating factor pretreatment followed by Impella-supported intracoronary infusion of

Echocardiography for extracorporeal membrane oxygenation.

Extracorporeal membrane oxygenation (ECMO) provides advanced cardiopulmonary life support for patients in cardiac and/or respiratory failure. Echocardiography provides essential diagnostic and anatomic information prior to ECMO initiation, allows for safe and efficient ECMO cannula positioning, guides optimization of flow, provides a modality for rapid troubleshooting and patient evaluation, and facilitates decision-making for eventual weaning of ECMO support. Currently, guidelines for echocardiographic assessment in this clinical context are lacking. In this review, we provide an overview of

[Long QT syndrome and anaesthesia].

The long QT syndrome (LQTS) is a rare, congenital or acquired disease, which may lead to fatal cardiac arrhythmias (torsade de pointes, TdP). In all LQTS subtypes, TdPs are caused by disturbances in cardiac ion channels. Diagnosis is made using clinical, anamnestic and electrocardiographic data. Triggers of TdPs are numerous and should be avoided perioperatively. Sufficient sedation and preoperative correction of electrolyte imbalances are essential. Volatile anaesthetics and antagonists of muscle relaxants should be avoided and high doses of local anaesthetics are not recommended to date. Pro

Successful prehospital extracorporeal cardiopulmonary resuscitation in pediatric cardiac arrest - a case report.

INTRODUCTION: Cardiac arrest in children necessitates a systematic approach, including Basic Life Support, Advanced Life Support and post-resuscitation care. Extracorporeal Cardiopulmonary Resuscitation offers a viable option for refractory cardiac arrest by providing hemodynamic support through extracorporeal circulation. While prehospital E-CPR in pediatric patients remains rare, two cases were reported in the Netherlands in 2023, demonstrating feasibility despite poor outcomes. We present the first documented case of pediatric prehospital E-CPR in Germany, conducted as part of the Klinikum

Immediate defibrillation or defibrillation after cardiopulmonary resuscitation.

OBJECTIVES: This study aimed to determine whether short cardiopulmonary resuscitation (CPR) by emergency medical services before defibrillation (CPR first) has a better outcome than immediate defibrillation followed by CPR (shock first) in patients with ventricular fibrillation/pulseless ventricular tachycardia (VF/pulseless VT) out-of-hospital cardiac arrest. METHODS: We analyzed a national database between 2006 and 2008, and included patients aged 18 years or more who had witnessed cardiac arrests and whose first recorded rhythm was VF/pulseless VT. Those study subjects were divided into fi

Cannabis-Induced Cardiac Arrest in a Young Adult: A Case Report.

BACKGROUND Tetrahydrocannabinol (THC), the psychoactive compound in cannabis, is increasingly used recreationally, especially among young adults. Although often perceived as harmless, emerging evidence links THC to serious cardiovascular complications, including QTc prolongation, ventricular arrhythmias, and sudden cardiac arrest, even in individuals without underlying heart disease. CASE REPORT A 26-year-old woman with a history of chronic cannabis use was brought to the Emergency Department (ED) after having a sudden cardiac arrest. Her initial rhythm was ventricular fibrillation, and return

Bradycardia, renal failure, atrioventricular nodal blockade, shock, and hyperkalemia: An important syndrome to recognize.

BRASH syndrome is a syndrome characterized by bradycardia, renal failure, usage of atrioventricular (AV) nodal blocker, shock, and hyperkalemia (BRASH). It is more common among patients with multiple comorbidities such as cardiac disease, kidney dysfunction, and hypertension requiring AV nodal blockers. Cardiac conduction abnormalities are frequently caused by severe hyperkalemia. However, it may also occur in mild-to-moderate hyperkalemia with concomitant use of AV nodal blockers due to the synergistic effects between these two factors in the presence of renal insufficiency. It is essential f

Cardiac arrest and successful resuscitation in a patient with Tako-Tsubo Cardiomyopathy.

Tako-Tsubo Cardiomyopathy (TTC) is described as transient left ventricular (LV) dysfunction without coronary artery stenoses. Typically the onset of TTC-syndrome is following emotional or physical stress. As an acute cardiac syndrome it is mimicking ST-elevation myocardial infarction. In this case we report from a 73-year old woman presenting with cardiac arrest after a long-distance bus-tour, and ongoing resuscitation to our ICU. Advanced life support with temporary ventricular pacing was continued until onset of stable spontaneous circulation. Left ventricular angiography revealed normal cor

Effects of Left Ventricular Versus Traditional Chest Compressions in a Traumatic Pulseless Electrical Activity Model.

BACKGROUND: Prehospital cardiopulmonary resuscitation has commonly been considered ineffective in traumatic cardiopulmonary arrest because traditional chest compressions do not produce substantial cardiac output. However, recent evidence suggests that chest compressions located over the left ventricle (LV) produce greater hemodynamics when compared to traditional compressions. We hypothesized that chest compressions located directly over the LV would result in an increase in return of spontaneous circulation (ROSC) and hemodynamic variables, when compared to traditional chest compressions, in
