Effects of flumazenil on cerebral blood flow and oxygen consumption after midazolam anaesthesia for craniotomy.

Cerebral blood flow (CBF) and cerebral metabolic rate for oxygen (CMRO2) were measured by a modification of the Kety-Schmidt technique using i.v. xenon-133 in 20 patients undergoing craniotomy for supratentorial cerebral tumours. Anaesthesia was induced and maintained with midazolam, fentanyl and nitrous oxide. Pancuronium was given for neuromuscular block. The lungs were ventilated to normocapnia. The first flow measurements were performed approximately 1 h after induction of anaesthesia. At the end of operation the patients were allocated to two groups. Ten patients were given flumazenil 0.0

A double blind randomized placebo controlled pilot study of single-dose preoperative modafinil for functional recovery after general anesthesia in patients with obstructive sleep apnea.

BACKGROUND: We theorized that modafinil, an atypical psychomotor stimulant, utilized to improve daytime somnolence in patients with obstructive sleep apnea, would improve functional recovery after general anesthesia by improving time to extubation, post-anesthesia care unit (PACU) length of stay and subjective recovery after general anesthesia. METHODS: A double blind, randomized, placebo-controlled pilot study was performed. 102 patients with the diagnosis of obstructive sleep apnea (OSA) were randomized to receive either 200 mg of modafinil or placebo before general anesthesia. The trial wa

Comparison of the effects of different general anesthesia methods on the quality of early postoperative recovery in patients undergoing metabolic bariatric surgery: A randomized controlled trial.

The optimal choice of general anesthesia technique for patients with obesity remains the subject of debate. At present, the anesthesia methods widely applied in clinical settings mainly consist of propofol-based intravenous anesthesia and sevoflurane-based inhalation anesthesia. For patients undergoing metabolic bariatric surgery, there is limited evaluation data concerning the overall recovery effect, and the conclusions are varied. This study aims to explore the difference between intravenous anesthesia and inhalation anesthesia in terms of the quality of early postoperative recovery in pati

Retrobulbar block fails to prevent an increase in serum cortisol concentration on emergence from anaesthesia after cataract surgery.

We have studied 30 elderly patients undergoing cataract surgery, allocated randomly to receive general anesthesia, local anaesthesia by retrobulbar block or general anaesthesia combined with retrobulbar block given after induction. Retrobulbar block alone prevented the increases in circulating cortisol and glucose values which occurred in those patients receiving general anaesthesia alone. Retrobulbar block given after induction of general anaesthesia, however, suppressed the cortisol and glucose response during surgery, but did not prevent a marked increase in cortisol concentrations during t

Anaesthesia for extracorporeal shock wave lithotripsy.

The anaesthetic considerations of patients presenting for extracorporeal shock wave lithotripsy are described. Regional anaesthesia with sedation may be preferable to general anaesthesia for patients undergoing this form of therapy. If regional anaesthesia is contra-indicated, general anaesthesia using controlled ventilation with muscle relaxation, supplemented with a narcotic and a low concentration of volatile anaesthetic has been found to be a suitable alternative. The additional epidural preparation time has to be balanced against the benefits of easier patient transfer, especially during

Development and validation of a predictive model for emergence agitation after general anesthesia in total knee arthroplasty patients.

This study aims to identify independent risk factors for postoperative agitation during the postoperative recovery period in patients undergoing total knee arthroplasty (TKA) under general anesthesia and construct and validate a predictive nomogram model. We conducted a retrospective analysis of clinical data from 957 patients who underwent primary unilateral TKA under general anesthesia between January 2021 and December 2024. Independent risk factors were identified using univariate and multivariate logistic regression analysis. A predictive nomogram was constructed based on these variables.

Association between postoperative delirium in the postanaesthesia care unit and subsequent ward delirium: A retrospective cohort study.

BACKGROUND: The ageing surgical population underscores the need for routine postoperative delirium (POD) screening, particularly in the immediate postoperative period. The postanaesthesia care unit (PACU) presents a critical window for early detection and intervention, yet the clinical significance of PACU-diagnosed POD (PACU-POD) remains underexplored. OBJECTIVES: To examine whether PACU-POD is associated with subsequent ward-POD and other adverse postoperative outcomes. DESIGN: Retrospective cohort study. SETTING: Tertiary academic hospital, single-centre, conducted between 2020 and 2022.

Cognitive Recovery by Decade in Healthy 40- to 80-Year-Old Volunteers After Anesthesia Without Surgery.

BACKGROUND: Postoperative delirium and postoperative cognitive dysfunction are the most common complications for older surgical patients. General anesthesia may contribute to the development of these conditions, but there are little data on the association of age with cognitive recovery from anesthesia in the absence of surgery or underlying medical condition. METHODS: We performed a single-center cohort study of healthy adult volunteers 40 to 80 years old (N = 71, mean age 58.5 years, and 44% women) with no underlying cognitive dysfunction. Volunteers underwent cognitive testing before and a

Epidural anesthesia accelerates the recovery of postischemic bowel motility in the rat.

BACKGROUND: Intestinal ischemia is associated with derangement of gastrointestinal motility. Uncontrolled clinical observations that bupivacaine injected into the epidural space causes faster recovery of bowel motility after various abdominal operations led us to assess the hypothesis that epidural anesthesia can hasten the recovery of gastrointestinal motility in the immediate postischemic period. METHODS: Gut motility studies were performed in rats in which epidural anesthesia and intestinal ischemia could be initiated without the need to provoke surgical trauma. Epidural lidocaine was comp

General versus general anaesthesia combined with caudal block in laparoscopic-assisted Soave pull-through of Hirschsprung disease: a retrospective study.

BACKGROUND: Caudal block is one of the most preferred regional anesthesia for sub-umbilical region surgeries in the pediatric population. However, few studies are available on caudal block performed in laparoscopic-assisted Soave pull-through of Hirschsprung disease (HD). We aimed to compare general anesthesia (GA) and general anesthesia combined with caudal block (GA + CA) in laparoscopic-assisted Soave pull-through of HD. METHODS: A retrospective review was performed in children with HD operated in our hospital between 2017 and 2020. Patients were divided into the GA and GA + CA group. The
