Anesthesia Exposure in the Young Child and Long-term Cognition: An Integrated Review.

Millions of children every year undergo seemingly safe general anesthetics for surgical procedures and imaging studies. Anesthetic agents have been shown to cause detrimental effects on brain cell survival and cognitive function in animals. As a result, the safety of general anesthetics in children is an active field of investigation. The objective of this review is to evaluate the human research on anesthesia neurotoxicity in the young child. Three databases were searched for studies on anesthesia exposure in infants and children. Positive clinical outcomes in several studies showed no differ

On Dr Dick Climie and Dr Jack Thomas, and the genesis of chemical-clinical pharmacology in Australian anaesthesia research.

Pharmacokinetic and pharmacodynamic research is regularly reported in most contemporary anaesthesia-oriented journals. This sub-specialty area of pharmacology grew rapidly from the 1960s as various essential concepts and tools-laboratory analysis of drug/metabolite concentrations in biofluids, physiological signal collection, and methods for analysing/presenting relevant pharmacokinetic and pharmacodynamic data-started coming together. For Australia, such research began in Sydney in the mid-1960s with collaboration between anaesthetist Dr C.R. Climie (1923-2013) at the Royal Hospital for Women

Anesthetic management of a child with Hunter's syndrome.

Hunter's syndrome is a member of a group of recessively inherited metabolic disorders termed mucopolysaccharidoses, caused by deficiency of lysosomal enzymes required for degradation of mucopolysaccharides or glycosaminoglycans, leading to accumulation of partially degraded glycosaminoglycans in various tissues. This leads to various anatomical abnormalities and systemic involvement, posing a challenge to an anesthetist. We present the anesthetic management of a 4-year old child with Hunter's disease with anticipated difficult airway, who presented for adenotonsillectomy and repair of umbilica

Surgery and Anesthesia in Patients with Pulmonary Hypertension.

Pulmonary hypertension is characterized by right ventricular impairment and a reduced ability to compensate for hemodynamic insults. Consequently, surgery can be challenging but is increasingly considered in view of available specific therapies and improved longer term survival. Optimal management requires a multidisciplinary patient-centered approach involving surgeons, anesthetists, pulmonary hypertension clinicians, and intensivists. The optimal pathway involves risk:benefit assessment for the proposed operation, optimization of pulmonary hypertension and any comorbidities, the appropriate

Neurally Mediated Syncope During Cesarean Delivery: A Case Report.

A 28-year-old woman with preeclampsia at 32 weeks of gestation underwent a cesarean delivery under spinal anesthesia. Administration of nitroglycerin at 200 μg to relax uterine smooth muscles and the application of fundal pressure led to severe bradycardia and loss of consciousness, followed by cardiac arrest. Delivery was completed immediately and recovery was achieved 10 seconds later following cardiopulmonary resuscitation. Neurally mediated syncope was considered the cause of cardiac arrest. Anesthetists should be aware of the potential risk during cesarean delivery following the administr

A multidisciplinary approach to the management of temporomandibular joint ankylosis in a sickle-cell anemia patient in a resource-limited setting.

This report describes the multidisciplinary management of a 35-year-old female sickle-cell anemia patient who had unilateral bony ankylosis of the left temporomandibular joint secondary to septic arthritis. She was managed by a team comprising of maxillofacial surgeons, anesthetists, otorhinolaryngologist, and hematologist. Unilateral left interpositional arthroplasty and ipsilateral coronoidectomy through a postrami approach were done and followed by aggressive jaw physiotherapy in the postsurgical period. No perioperative morbidity was encountered. Mouth opening of 3.5 cm was achieved and ma

Anesthetic management of children with craniofacial anomalies.

Children with craniofacial anomalies, in particular those with facial clefts and the associated syndromes of Treacher Collins, Pierre Robin, and Klippel-Fiel and craniosynostosis and its related syndromes, present some of the most hazardous and difficult challenges that anesthetists may encounter within the entire practice of pediatric anesthesia. Through this review of the embryological development of the above-listed anomalies, the specific concerns of the airway and coexisting congenital defects, the reader will develop a better understanding of each anomaly in order to create a safe anesth

Prevention of occupational transmission of bloodborne diseases in clinical nurse anesthesia practice.

This article discusses the most commonly recognized bloodborne infectious diseases with which the nurse anesthetist comes into contact during the provision of clinical care: hepatitis B virus, hepatitis C virus, and the human immunodeficiency virus, along with the potential percutaneous and mucocutaneous routes of exposure. The prevention of occupational exposure to these pathogens is discussed by applying standard or universal precaution measures to anesthesia clinical practice. Treatment measures for accidental exposure to bloodborne pathogens is also discussed.

BACKGROUND AND AIMS: When there is an immediate threat to maternal or fetal life, it is recommended to deliver within 30 min of the decision to have favorable perinatal outcomes. However, there is no data on the delivery intervals for Category-I emergency cesarean section in Bhutan. The study evaluated the decision to delivery interval (DDI) and its effect on perinatal and maternal outcomes in Category-I emergency cesarean section. METHODS: A retrospective cross-sectional study was conducted at the Phuentsholing General Hospital, Bhutan, from January 1, 2020 to December 31, 2020. Mothers who

Barriers and enablers to the implementation of perioperative hypothermia prevention practices from the perspectives of the multidisciplinary team: a qualitative study using the Theoretical Domains Framework.

Purpose: Inadvertent perioperative hypothermia is a significant problem for surgical patients globally, and is associated with many detrimental side-effects. Despite the availability of rigorously developed international evidence-based guidelines for prevention, a high incidence of this complication persists. This qualitative study aims to identify and examine the domains which act as barriers and enablers to perioperative hypothermia prevention practices, from the perspectives of the key healthcare professionals involved with perioperative temperature management. Methods: A qualitative study

Specially trained registered nurses can safely manage epidural analgesia infusion in laboring patients.

PURPOSE: To discover evidence for defining the registered nurse's (RN's) role in the management of epidural analgesia in the labor and delivery setting. DESIGN: The Labor Epidural Nurse Safety (LENS) study consisted of two parts. The first part was a 10-year retrospective review of the outcomes of 2,568 laboring women for whom epidural catheters had been placed and verified by an anesthesiologist or certified registered nurse anesthetist, then continuous epidural infusion initiated, and basal rate or patient-controlled epidural analgesia (PCEA) dose increased, if needed, within specified para

Life is in the Blood: Experiential Learning in Surgical Missions.

Many healthcare facilities in low-income countries (LICs) lack on-site blood banks, creating unfamiliar barriers to U.S. healthcare workers on mission trips. Abdominal hysterectomies are frequently performed on surgical missions and may require transfusion. In this case study, a patient received warm fresh whole blood (WFWB) from student registered nurse anesthetists and a registered nurse, post-hysterectomy in Guatemala. This case highlights preparedness for critical needs, the value of WFWB, and the educational and personal values gained on international missions. Clinical implications of WF

Anesthetic Management of Patients Undergoing Electroconvulsive Therapy.

Electroconvulsive Therapy (ECT) is an effective treatment for mood disorders, particularly treatment-resistant depression, and several psychiatric illnesses. Anesthetic management of patients undergoing ECT requires an understanding of ECT, the physiologic changes induced by electrical stimulus, the anesthetic agents used, and the potential complications associated with the procedure. Numerous medical and surgical conditions can impact procedural approach and anesthetic management. ECT is challenging due to its typically off-site location, fast-paced general anesthesia with bag-mask ventilatio

Classification and clinical evaluation.

Conjoined twins represent a great challenge for most pediatric specialists including pediatric surgeons, anesthetists, neonatologists, urologists, neurosurgeons, and orthopedic surgeons. This anomaly can be classified according to the type of twins׳ fusion. Various organs can be fused making the separation difficult. Conjoined twins are usually diagnosed antenatally by ultrasound. Detailed fetal echocardiography is necessary to counsel the parents during pregnancy. Postnatally, the majority of the conjoined twins can be thoroughly investigated using various imaging techniques. This allows care

Aortic stenosis: A review.

The prevalence of aortic stenosis has been increasing in recent decades, and nurse anesthetists are more likely to encounter this problem as the population ages. Fortunately, the widespread use of echocardiography has expanded our understanding of valvular heart disease. The purpose of this course is to evaluate the current literature regarding aortic stenosis with a focus on anesthetic management. The 2 most common causes of aortic stenosis are calcific tricuspid disease and congenital bicuspid valves. An inflammatory, atherosclerotic disease process also has been identified in aortic stenosi

[Predictive value of the allergologic profile in anesthesia and resuscitation].

The allergic history to anaesthetics in general and especially to synthetic myorelaxants, poses few etiological diagnostic problems in practice, because of on one hand the excellent collaboration between Anesthetists and Allergologists in the CHU at Angers and on the other because of the great sensitivity of skin testing. Why then have a predictive history for myorelaxants? The results of a study at the CHU at Angers on 15 patients showed sensitivity to anaesthetics in general (myorelaxants 86%, Diprivan) and/or an antibiotic, latex were very encouraging.
