Traumatic carotid artery dissection without head or neck trauma: a case report following thoracoabdominal crush injury.

A previously healthy 41-year-old woman was admitted after being crushed beneath a forklift from the chest to the lower extremities, presenting with traumatic asphyxia. Initial evaluation revealed no apparent trauma to the head or neck, but multiple fractures from the chest to the lower extremities were diagnosed. The patient underwent emergent thoracoabdominal surgery with subsequent clinical improvement. However, 4 days after the accident, anisocoria developed, and neuroimaging confirmed ischemic stroke due to traumatic carotid artery dissection. Anticoagulation and lipid-lowering therapy wer

Varix Rupture Due to High-Flow Parasagittal Sinus Dural Arteriovenous Fistula: A Case Report and Literature Review.

Parasagittal sinus dural arteriovenous fistulas (DAVFs) are rare and aggressive vascular malformations with a high risk of intracranial hemorrhage (ICH). We present the case of a 75-year-old man with altered consciousness, anisocoria, and left hemiparesis due to a large subcortical ICH. Imaging revealed a ruptured varix associated with a parasagittal sinus DAVF (Cognard type IV, Borden type III). The patient underwent emergency hematoma evacuation, followed by successful transarterial embolization via the contralateral middle meningeal artery (MMA) using N-butyl-2-cyanoacrylate. Despite comple

Management of Cerebral Herniation Secondary to Lead Encephalopathy: A Case Report.

BACKGROUND: Adult lead encephalopathy is a rare but critical condition to recognize in modern healthcare settings. Few reports have described the medical and neurosurgical management of severe adult lead encephalopathy. CASE PRESENTATION: A 22 year old woman presented with severe headache, anemia, vomiting, 40-lb weight loss, and constipation. At the time of presentation, she had extensive colonic radiopaque material and a serum lead concentration of 87 mcg/dl (normal <10). She rapidly developed anisocoria requiring emergent ventriculostomy insertion. Following CSF diversion, ICP mitigation,

Transient ipsilateral mydriasis following carotid artery stenting.

BACKGROUND AND IMPORTANCE: One of the most feared and devastating complications of carotid revascularization procedures is hyperperfusion hemorrhage. The acute onset of an ipsilateral mydriatic pupil following carotid endarterectomy (CEA) or carotid artery stenting (CAS) should prompt immediate neurosurgical evaluation to rule out hyperperfusion injury. CLINICAL PRESENTATION: We describe a case of benign, transient ipsilateral mydriasis following CAS. After undergoing right common and internal carotid artery (ICA) angioplasty and stenting with distal embolic protection, the patient developed

The role of apraclonidine in Horner’s syndrome – A case report.

INTRODUCTION: Horner’s syndrome is an interruption of the sympathetic nervous system at any point along its course between the hypothalamus and the orbit. Horner’s syndrome is classically presented as an ipsilateral miosis, subtle ptosis, and facial anhidrosis. Pharmacologic testing is very useful in the diagnosis of Horner’s syndrome as it could help to localize the lesioned neuron in the sympathetic pathway, suggesting an etiology. CASE OUTLINE: We present a case report of a 41-year-old woman who reported right eyelid drooping immediately after operation of sympathetic chain schwannoma. We

Primary histiocytic sarcoma in the brain with renal metastasis causing internal ophthalmoparesis and external ophthalmoplegia in a Maine Coon cat.

CASE SUMMARY: An 11-year-old neutered male Maine Coon cat was presented for investigation of anisocoria and depression. Neurological examination was consistent with a lesion at the level of the middle cranial fossa, and biochemistry was indicative of moderate renal functional impairment. MRI of the brain identified an extra-axial mass lesion at the level of the middle cranial fossa, T2-weighted hyperintense and strongly homogeneously contrast enhancing with dural tail. The cat was euthanased after 6 weeks of palliative treatment with corticosteroids. Histopathology and immunohistochemistry of

Acquired Intermittent Pediatric Horner Syndrome due to Neuroblastoma.

A 3-month-old male developed intermittent left upper eyelid ptosis at the age of 1 month that was gradually increasing in frequency and duration. Examination revealed anisocoria and left upper and lower eyelid ptosis, consistent with a left Horner syndrome. Imaging showed a mass in the left superior posterior mediastinum, which was resected, and pathology was consistent with neuroblastoma. Eight months thereafter, the patient underwent left upper eyelid ptosis repair. Cases of infantile acquired Horner syndrome due to neuroblastoma are rare. To the authors' knowledge, there has only been one c

Ophthalmologic complications after administration of local anesthesia in dentistry: a systematic review.

OBJECTIVE: The aim of this review was to investigate the association between the occurrence of ocular adverse events and dental local anesthesia, the most plausible anatomic mechanisms, and the measures that offer patients a restitutio ad integrum. STUDY DESIGN: This systematic review adopted a structured protocol to access available publications and followed the PRISMA statement. RESULTS: Eighty-nine cases of patients experiencing ocular adverse events after administration of dental local anesthesia have been reported in the literature. Most of the complications manifested as double vision.

Visual impairment in stroke patients--a review.

Approximately 30% of all stroke patients suffer from post-stroke visual impairment. Hemianopia is the most common symptom, but also neglect, diplopia, reduced visual acuity, ptosis, anisocoria, and nystagmus are frequent. Partial or complete recovery of visual disorders can occur, but many patients suffer permanent disability. This disability is often less evident than impairment of motor and speech functions, but is negatively correlated with rehabilitation outcome and can lead to a significant reduction in day-to-day functioning. To be visually impaired after stroke reduces quality of life a

[Emergency unilateral mydriasis].

UNLABELLED: UNEQUAL PUPILS: Or anisocoria is frequent and can be physiological, benign or life threatening. VARIATIONS IN THE PUPIL: Constriction depends on the parasympathetic system and dilatation on the sympathetic system. IN PARASYMPATHETIC DISORDERS: Peripheral or central neurological causes can be distinguished, among which acute cerebral lesions with compression of the trunk are the most dramatic. Differential diagnosis can be made by studying the local causes, which may represent ophthalmologic emergencies (acute glaucoma or lesions of the retina). AMONG THE SYMPATHETIC DISORDERS: Clau

Sinus arrhythmia and pupil size in Chiari I malformation: evidence of autonomic dysfunction.

In a prospective study on latent autonomic dysfunction in the Chiari type I malformation, 15 patients were examined for degree of sinus arrhythmia in the supine and the sitting positions with a deep breathing test, and for pupil size and function with electronic pupillometry after sympathicomimetic eyedrop stimulation. The 5 patients with brainstem signs had a marked disturbance of sinus arrhythmia, which was more pronounced in the supine than in the sitting position. The clinical signs suggested a dysfunction of the reflex centers in the medulla rather than of the lower cranial nerves. The pa

[Comas in the emergency room of a central hospital].

The aim of the present prospective work was to study the etiologic diagnosis and prognosis of the comatose patients for whom a neurologic examination is requested. It included 148 consecutive cases admitted to the emergency room of a general hospital. Coma was caused by supratentorial lesions in 38%, subtentorial lesions in 10%, diffuse or metabolic brain dysfunction in 49%, and psychiatric disorder in 1% of the patients. CT scan was the most valuable ancillary exam, modifying the initial etiologic diagnosis in 42% of the cases on whom it was performed. Seventy percent of the patients died. Co

[A case of absence of the left internal carotid artery associated with aneurysm of the anterior communicating artery].

A 67-year-old female was admitted to our hospital because of unconsciousness after severe headache on April 7, 1986. On admission, the patient was semicomatose with anisocoria and absence of light reflex. Computed tomography showed subarachnoidal hemorrhage with intraventricular clots. Right carotid angiogram revealed that the left anterior cerebral artery was filled from the right side through the anterior communicating artery and a saccular aneurysm in anterior communicating artery. On left-side carotid angiogram, neither the left internal carotid artery nor the intracranial arteries were vi

Acute cerebral ischemia after intracranial bleeding in unilateral moyamoya disease: case report.

A 31-year-old male presenting with intracranial hemorrhage manifesting as deep coma and anisocoria underwent immediate emergency surgery. Three-dimensional computed tomography (CT) angiography revealed stenosis of the right middle cerebral artery (MCA) and perfusion CT immediately after the surgery suggested severe hypoperfusion in the right MCA territory. Postoperative angiography demonstrated right unilateral moyamoya disease. We predicted that brain edema and intracranial pressure (ICP) elevation occurring after the hemorrhage might result in cerebral infarction. Hyperosmotic drugs were con

Oculomotor nerve palsy following coronary artery bypass graft surgery: can pituitary apoplexy complicate the post-operative course of cardiac surgery?

Oculomotor nerve palsy, due to pituitary apoplexy, has been previously reported in the literature. However, the association with coronary artery bypass graft surgery (CABG) is rarely investigated. This article reports a case of pituitary apoplexy presenting with oculomotor nerve palsy following CABG. A 65-year-old male, known to have ischemic heart disease, diabetes mellitus and hypertension, presented with ptosis, diplopia and anisocoria that developed after 1 day of CABG. Radiological imaging demonstrated a pituitary adenoma with acute/subacute hemorrhage causing mild mass effect on the cave

[Fatal Acute Brain Swelling after Clipping of Unruptured Anterior Communicating Aneurysm: A Case Report].

There have been few reports about acute brain swelling caused by unruptured aneurysm surgery. We report a case of fatal acute brain swelling after clipping of an unruptured anterior communicating artery aneurysm following lung cancer surgery. A 65-year-old man underwent aneurysm neck clipping for an unruptured anterior communicating artery aneurysm by interhemispheric approach 35 days after curative resection of lung cancer. Immediately after the operation, his recovery from anesthesia was prolonged and anisocoria developed. Therefore, we performed emergent reoperation and encountered acute br

Isolated unilateral neurogenic blepharoptosis secondary to eyelid trauma.

PURPOSE: To describe eyelid trauma as a cause of isolated neurogenic blepharoptosis. DESIGN: Observational case series. METHODS: Three previously healthy patients (two male and one female; aged 29 to 39 years) were evaluated for unilateral blepharoptosis following forceful anterior displacement of the upper eyelid. Two injuries occurred during domestic altercations and one while the patient was playing basketball. RESULTS: All three patients had complete unilateral upper eyelid ptosis with no levator palpebrae superioris function, consistent with loss of innervation. Additional findings inc

Negative MRI versus real disease.

A 76-year-old diabetic woman presented with progressive binocular vertical diplopia and right eye pain. Examination revealed a pupil-involving partial right third cranial nerve palsy, with development of anisocoria over the course of several hours. MRI of the brain showed no mass lesion. MRA, even with retrospective review of the images, failed to clearly identify a 1 cm right posterior communicating artery aneurysm detected by subsequent conventional cerebral angiography. While MRA has been reported to be highly sensitive in cerebral aneurysm detection at some centers, other investigators hav

A prospective study to identify high-yield criteria associated with acute intracranial computed tomography findings in head-injured patients.

We conducted a multicenter, prospective study of head-injured patients to identify high-yield clinical criteria for acute intracranial injuries. Emergency patients with a history of blunt head trauma occurring within 2 weeks and who underwent nonenhanced cranial computed tomography (CT) were entered onto the study during a 12-month period. Of the 264 patients, 32 (12%) had abnormal CT findings. Nine high-yield variables were associated with abnormal CT findings: alcohol use before injury, antegrade amnesia, prolonged loss of consciousness, anisocoria and/or fixed and dilated pupils, abnormal B

The role of the physical examination in the evaluation of headache.

The population of patients suffering with headaches is vast and underserved. The most critical element in headache evaluation is the history. The targeted history not only differentiates primary from secondary headaches but also provides a realistic list of conditions associated with secondary headache. Several of these conditions do present with specific physical findings, such as papilledema, Horner's syndrome, or CN palsy. The targeted physical examination of the patient with headache takes less than 3 minutes. The ability simply to recognize but a few straightforward clinical findings dire
