Effects of carvedilol on the prevention of cardiotoxicity induced by anthracyclines: Design and rationale of the CARDIOTOX trial.

BACKGROUND: Patients with cancer undergoing chemotherapy with an anthracycline-based regimen are at increased risk of cardiotoxicity, predisposing to heart failure, arrhythmias and death. Whether carvedilol may confer benefit to prevent anthracycline-induced cardiotoxicity remains to be determined. DESIGN: CARDIOTOX is a double-blind, placebo controlled randomized clinical trial that plan to enroll 1,018 patients across 25 study sites in Brazil. Patients with active cancer scheduled to undergo an anthracycline-based chemotherapy regimen are eligible. Patients with prior HF or cardiomyopathy a

Upper gastrointestinal bleeding secondary to toxicity by anthracyclines, cytarabine and methotrexate in a patient with acute lymphoblastic leukemia.

A 60-year-old male patient with no medical history but acute lymphoblastic leukemia was admitted in the hematology ward for chemotherapy treatment (methotrexate, cytarabine, fludarabine and idarubicin). Approximately six days after finishing this chemotherapy, the patient began with symptoms of upper gastrointestinal bleeding (hematemesis and melena). Gastroscopy revealed erosive esophagitis, gastritis and duodenitis, as well as Forrest III ulcers in the duodenal bulb and multiple Forrest IIb ulcers in the second portion of the duodenum. Given the multiplicity of lesions, medical treatment wit

Concurrent administration of trastuzumab and anthracyclines as adjuvant regimen for HER2-positive breast cancer: a randomised controlled trial.

BACKGROUND: Epidermal growth factor receptor (EGFR) mutation status plays a critical role in guiding targeted therapy for non-small cell lung cancer (NSCLC). However, molecular testing in patients with stage IA NSCLC may be limited by insufficient tissue availability, procedural invasiveness, and resource constraints. Therefore, developing a non-invasive approach for EGFR mutation prediction is of substantial clinical interest. This study aimed to develop a computed tomography (CT) radiomics based model integrating clinical variables for non-invasive prediction of EGFR mutation status in stage

Role of NT-proBNP in detection of myocardial damage in childhood leukemia survivors treated with and without anthracyclines.

BACKGROUND: Exposure to anthracyclines (ANT) during childhood represents a high risk for development of late cardiotoxicity. Cardiotoxicity is usually detected only when clinical symptoms or progressive cardiac dysfunction have already occurred. Early detection of cardiotoxicity may lead to better therapeutic outcome. N-terminal pro-brain natriuretic peptide (NTproBNP) has been hypothesized to reflect increased left ventricular wall stress before development of echocardiographic abnormalities. The aim of this study was to detect cardiac abnormalities using plasma NTproBNP and echocardiography

Anthracyclines, mitoxantrone, radiotherapy, and granulocyte colony-stimulating factor: risk factors for leukemia and myelodysplastic syndrome after breast cancer.

PURPOSE: To determine the risk factors for acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS) after breast cancer. PATIENTS AND METHODS: We conducted a case-control study among women treated for breast cancer between 1985 and 2001 in French general hospitals, cancer centers, or clinics. We included 182 AML and MDS patients and 534 matched controls. Breast cancer characteristics, type of treatment, and family history of cancer were compared in both groups. RESULTS: The risk of AML/MDS was increased after topoisomerase-II inhibitor-based chemotherapy (P < 10-16) and was higher for

Anthracyclines added to the conditioning regimen for allogeneic bone marrow transplantation are associated with a slower haematopoietic recovery.

Factors which may influence haematopoietic recovery after allogeneic bone marrow transplantation were analysed. Forty-six evaluable patients transplanted with lymphocyte-depleted marrow for acute lymphoblastic leukaemia, acute non-lymphoblastic leukaemia, chronic myeloid leukaemia, myelodysplastic syndrome and severe aplastic anaemia were studied. The median time for platelet recovery to greater than or equal to 20 and to greater than or equal to 50 x 10(9)/l was 21 (9-72) and 26 (11-86) days respectively. The neutrophil recovery to greater than or equal to 0.5 x 10(9)/l and the leucocyte reco

Comparative in vitro activity of 4'-deoxy-4'-iododoxorubicin and other anthracyclines in the human tumor clonogenic assay.

A new halogenated anthracycline analog 4'-deoxy-4'-iododoxorubicin (IODO) was compared with doxorubicin (DOX) and deoxydoxorubicin (DEOX) in the human tumor clonogenic assay (HTCA) using human tumor cell lines. For all cell lines tested, IODO had lower ID50 value and thus greater in vitro potency and cytotoxicity than DOX. DEOX had lower average ID50 values than either IODO or DOX in all cell lines except HEC1A, where DEOX was equal to IODO. Analysis of variance likewise confirmed significantly greater activity for IODO versus DOX in most cell lines tested. Previous in vivo studies demonstrate

Natural history of myocardial deformation in children, adolescents, and young adults exposed to anthracyclines: Systematic review and meta-analysis.

OBJECTIVE: Anthracyclines are widely used to treat solid and hematologic malignancies, but are known to cause cardiotoxicity. As more childhood cancer survivors reach adulthood due to improvements in oncologic treatments, they become susceptible to late and progressive anthracycline-induced cardiotoxicity. Nonetheless, diagnostic criteria for early detection of cardiac dysfunction are not well defined in children, adolescent, and young adults (CAYA, ages 1-40 years). We present a natural history of the changes in myocardial deformation in CAYA patients after anthracycline therapy. METHODS: We

A scanning calorimetric study of the interaction of anthracyclines with neutral and acidic phospholipids alone and in binary mixtures.

High sensitivity differential scanning calorimetry was employed to study the thermotropic behavior of multilamellar vesicles of neutral and acidic phospholipids and binary mixtures thereof in the presence of anthracycline antibiotics. Adriamycin and its lipophilic analogue, N-trifluoroacetyladriamycin-14-valerate (AD32) were investigated and compared to chlorpromazine and quinidine with respect to their ability to affect the pretransition and the main transition of the phospholipids suspended in physiological buffer. With liposomes of neutral dipalmitoylphosphatidylcholine the observed effects

Time-Varying Spatial Ventricular Gradient Orientation Is Associated With Anthracycline Heart Failure.

BACKGROUND: Anthracycline use is limited by cardiotoxicity resulting in heart failure (HF). The spatial ventricular gradient (SVG) is a vectorcardiographic measurement of myocardial electrical heterogeneity associated with sudden death and incident HF. It is unclear if serial measurement of SVG after anthracycline initiation can help predict subsequent HF. OBJECTIVES: The objective of the study was to determine if time-varying SVG measurements are associated with anthracycline HF. METHODS: Retrospective, single-center study of adult patients with cancer treated with anthracyclines between 19

Rho/PAK signaling: a key driver of anthracycline and taxane resistance in breast cancer.

Resistance to anthracyclines and taxanes remains a major barrier to effective breast cancer treatment, particularly in aggressive subtypes such as triple-negative breast cancer. Although these agents form the backbone of systemic chemotherapy, variability in response limits long-term survival benefits and underscores the urgent need for predictive biomarkers and novel therapeutic strategies. Emerging evidence implicates Rho family small GTPases and their downstream effectors, p21-activated kinases (PAKs), as central regulators of cytoskeletal remodeling, epithelial-mesenchymal transition, and

Early anthracycline cardiotoxicity in adolescents and young adults with sarcoma: a prospective echocardiographic study.

BACKGROUND: Adolescents and young adults (AYAs) with sarcomas often receive high-dose doxorubicin (Dox), but data on early cardiotoxicity in this population are limited. OBJECTIVES: To prospectively evaluate early echocardiographic changes in AYAs with sarcoma treated with high-dose Dox. METHODS: AYAs (15-39 years) with sarcoma treated at a tertiary cancer centre (2018-22) were prospectively enroled. Echocardiograms were performed at baseline, 1 and 2 years after cancer therapy initiation and interpreted by a single cardiologist. The primary endpoint was a >10% absolute reduction in left ven

Targeting the Apelin-APJ Axis: A Promising Strategy to Mitigate Anthracycline-Induced Cardiotoxicity.

Anthracycline-induced cardiotoxicity remains a major clinical challenge, often progressing to heart failure years after therapy. Conventional cardioprotective agents, including angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, and beta-blockers, are widely used to preserve cardiac function; however, their effectiveness is limited by their inability to comprehensively address the complex, multifactorial pathophysiology of anthracycline-induced cardiotoxicity. This underscores the critical need for more effective and mechanism-based cardioprotective strategies that directl

Plasma Proteins Associated With the Immune Response to Anthracycline Cardiotoxicity in Patients With Hematologic Malignancies.

BACKGROUND: Anthracyclines such as doxorubicin are well recognized to induce dose-dependent cardiotoxicity. However, traditional imaging and blood biomarkers of myocardial injury are inadequate for early risk stratification. We aimed to discover plasma proteins that correlate with the subsequent development of anthracycline-induced systolic dysfunction in patients, with validation studies in preclinical models. METHODS: We used an aptamer-based proteomics platform to measure over 7000 proteins in the plasma obtained at baseline (before anthracycline chemotherapy) in a prospectively enrolled c

Natural ERRα Activator Formononetin Ameliorates Anthracycline Cardiotoxicity via Metabolic Improvement.

BACKGROUND: Anthracycline-induced cardiotoxicity (AIC) limits life-saving chemotherapy and is driven by early metabolic remodeling. The nuclear receptor ERRα (estrogen-related receptor α) is a master regulator of cardiac energy metabolism, but the temporal dynamics of its downregulation, its causal role in AIC pathogenesis, and whether it can be pharmacologically activated to confer protection remain undefined. METHODS: We performed temporal protein analysis in a porcine AIC model. Using cardiomyocyte-specificgain- and loss-of-function mouse models, we assessed the causal role of ERRα. Mechan

Coronary Artery Calcium Score as a Predictor of Anthracycline-Induced Cardiotoxicity: The ANTEC Study.

Background: Many risk factors for cancer therapy-related cardiovascular toxicity overlap with risk factors for atherosclerosis. According to the ESC 2022 Cardio-Oncology Guidelines, coronary computed tomography angiography and coronary artery calcium score are not recommended as part of routine risk assessment prior to oncological treatment. The aim of this study was to prospectively assess the influence of coronary artery calcium score (CAC score) on cancer therapy-related cardiac dysfunction in patients with moderate and high risk of cardiovascular toxicity, qualified for anthracycline treat

Evaluation of Gemtuzumab Ozogamicin and Anthracycline Dosing for Favorable Risk Acute Myeloid Leukemia.

Gemtuzumab ozogamicin (GO) is a CD33-targeting antibody-drug conjugate approved for the treatment of CD33-positive de novo and relapsed and refractory acute myeloid leukemia (AML). Subset analyses have demonstrated improved clinical outcomes in patients with favorable-risk disease. It is unclear whether the addition of GO to cytarabine and anthracycline chemotherapy (7+3) improves clinical outcomes compared with other conventional regimens for AML. We evaluated the real-world experience with GO added to 7+3 chemotherapy for patients with favorable risk AML. This retrospective analysis included

Redefining anthracycline-related subclinical cardiotoxicity: 'Absolute' and 'relative' change in longitudinal strain.

AIMS: Anthracycline chemotherapy (AC) for breast cancer can cause cancer therapy-related cardiac dysfunction (CTRCD) with resultant heart failure, traditionally defined as a reduction in left ventricular (LV) ejection fraction on echocardiography. In recent years, global longitudinal systolic strain (GLS) has been used to identify subclinical cardiac dysfunction prior to development of overt CTRCD. Recent harmonized guidelines have incorporated GLS into definitions for CTRCD to identify cardiac dysfunction and inform decisions regarding cardioprotective strategies. METHODS AND RESULTS: We eva
