Small Molecule Attenuates Bacterial Virulence by Targeting Conserved Response Regulator.

Antibiotic tolerance within a biofilm community presents a serious public health challenge. Here, we report the identification of a 2-aminoimidazole derivative that inhibits biofilm formation by two pathogenic Gram-positive bacteria, Streptococcus mutans and Staphylococcus aureus. In S. mutans, the compound binds to VicR, a key response regulator, at the N-terminal receiver domain, and concurrently inhibits expression of vicR and VicR-regulated genes, including the genes that encode the key biofilm matrix producing enzymes, Gtfs. The compound inhibits S. aureus biofilm formation via binding to

C-Met Inhibitors are Potential Novel Therapeutic Agents Against Listeria monocytogenes Infection Through Blocking the Bacteria Entry into Nonphagocytic Cells.

High lethality of infections caused by Listeria monocytogenes still remains a major clinical problem in spite of their susceptibility to a wide spectrum of antibiotics. The refractoriness towards treatment is primarily due to its amazing capacity to invade non-phagocytic cells and replicate there in, imparting the dual protection from immune response and antimicrobials. Therefore, generating new anti-infective drugs against intracellular infections has emerged as an urgent issue in the therapeutics of listeriosis. Researches have demonstrated that, internalization of Listeria monocytogenes int

Therapeutic management of inflammatory heart diseases.

Inflammatory heart diseases include a wide range of clinical entities whose manifestations largely depend on whether the pericardium, myocardium, and/or endocardium are affected by infectious agents or sterile inflammation. Advancements in understanding the inflammatory pathogenesis of pericarditis have led to the successful development of targeted anti-inflammatory therapies. While non-steroidal anti-inflammatory drugs (NSAIDs) and colchicine remain first-line therapeutic options in those with acute pericarditis, interleukin-1 (IL-1) inhibitors have been recently approved for the treatment of

Serious musculoskeletal infections in children receiving anti-tumor necrosis factor-alpha therapy: a case series.

Anti-tumor necrosis factor-alpha (anti-TNF-alpha) agents are widely used to treat children with juvenile idiopathic arthritis (JIA) whose disease is resistant to conventional therapy. Although generally well tolerated, use of these agents has been associated with an increased risk of infection. In particular, in patients treated with anti-TNF-alpha agents, there is an increased susceptibility to infection by intracellular organisms such as tuberculosis, and common infections may present atypically or be more severe. We report four cases of serious musculoskeletal infections among 31 children w

Does rituximab aggravate pre-existing hypogammaglobulinaemia?

Rituximab, an anti-CD20 chimeric antibody, is the first monoclonal agent to be used in the therapy of cancer. It has been hailed as one of the most important therapeutic developments of the decade. While transient peripheral B cell depletion is common after rituximab therapy, immunoglobulin levels are generally not affected. This is because CD20 is expressed on pre-B and mature B lymphocytes but not on stem cells or plasma cells. Two adult patients with pre-existing primary antibody deficiency who presented with recurrent infections immediately following rituximab use for the treatment of refr

MRSA new treatments on the horizon: current status.

There is a choice of anti-MRSA antibiotic available with proven efficacy in the treatment of complicated skin and skin structure infection (cSSSI). Additional anti-MRSA antibiotics are in development, which have the potential to influence how such infections are managed. The emergence of resistance to current anti-MRSA agents, toxicity, and general lack of oral agents with proven efficacy for deep seated infection justify the development of new agents. However, there is a relative dearth of information specific to patients with orthopaedic-related infection. Combination therapy is often used i

Multidrug-resistant bacteria quorum-sensing inhibitors: A particular focus on Pseudomonasaeruginosa.

Many widely used conventional antibiotics have failed to show clinical efficacy against Pseudomonas aeruginosa (P. aeruginosa) due to the strain's rising resistance to most clinically relevant antimicrobials. P. aeruginosa uses quorum sensing to regulate its virulence and biofilm development, which contributes to its pathogenicity and drug resistance. This mechanism is responsible for the resurgence and persistence of infections. Therefore, targeting the virulence and pathogenicity of P. aeruginosa through quorum sensing (QS) is regarded as a potential target for anti-infective therapy. Howeve

Development of a platform for the discovery and practical synthesis of new tetracycline antibiotics.

Tetracyclines have proven to be safe and effective antibiotics over decades but to date all approved members of the class have been discovered and manufactured by chemical modification of fermentation products, which greatly limits the number of new structures that can be explored as future medicines. This review summarizes research leading to the development of a platform synthetic technology that enabled the discovery of the clinical candidate eravacycline, as well as other promising new tetracycline antibiotics, and provides the basis for a practical route for their manufacture. [Infectious diseases of the lower gastrointestinal tract and their differential diagnoses].

Infectious colitis comprises histologically a spectrum from normal mucosa to severe inflammation with ulceration. In many cases, a diagnosis is made microbiologically or virologically. Nevertheless, histology still plays an important role, given its potential time and cost-advantages as well as its possibility to provide a statement on differential diagnoses, leading to different therapeutic treatment strategies (anti-infective drugs, surgery, immunosuppressive agents). The pathologists' role is especially important in three scenarios: 1) The infectious agent is directly visible by light micro

A perspective on the next generation of antibacterial agents derived by manipulation of natural products.

Natural products have been a major source of anti-infective drugs for many decades. With urgent need for new antibacterial agents to combat drug-resistant bacteria, the investigation of both new and existing classes of natural products has once again become an important focus. In this review, we highlight how a medicinal chemistry/semi-synthetic approach to natural product manipulation continues to offer a valuable strategy to overcome limitations in current therapy. Approaches to address toxicity and to improve the solubility, bioavailability and the spectrum of activity are demonstrated. Exa

[Sensory-motor polyneuropathy associated with lung tuberculosis: presentation of two new cases].

The association of active tuberculosis with changes in the peripheral nervous system is not well defined, although it is included amongst the neuro-muscular disorders of the peripheral nerve diseases due to infective agents. We describe the case of two patients with sensory-motor polyneuropathy and coexistant active pulmonary tuberculosis which was untreated prior to diagnosis of the peripheral nervous system disorder. In both cases the condition was sub-acute and did not improve until treatment was begun. Anti-tuberculosis treatment was followed by marked improvement of both the pulmonary tub

Background/Objectives: Biofilm-related infections represent a major concern in clinical practice because of their poor responsiveness to standard antimicrobial treatments. Over the past decade, many efforts have been made to identify new compounds capable of inhibiting this particularly resistant form of bacterial life. Although several compounds have shown interesting anti-biofilm properties, none have reached the clinic. Methods: In the present work, we describe the synthesis and biological assessment as anti-biofilm agents, of a novel library of twenty-one imidazo[2,1-b][1,3,4]thiadiazol-2-
