[School refusal].

Pediatrician are often questioned by school refusal which relies on a wide range of psychopathological features and necessitates specific approaches. This disabling condition remains underestimated and is still increasing. A poor prognosis associated with a prolonged school absence is the common hallmark of school refusals, regardless of its heterogeneity. Its seriousness warrants early identification and prompt intervention by childhood healthcare professionals, teachers and social workers. A specialized treatment is needed, closely linked with families and school. Promising developments come

Decreased chaos of heart rate time series in children of patients with panic disorder.

This study examined the differences of heart rate variability measures between children of parents with panic disorder and children of healthy controls using linear as well as nonlinear techniques. Supine and standing heart rate variability indices were measured in all children using power spectral analysis and a measure of chaos, the largest Lyapunov exponent (LLE) of heart rate time series. No significant differences emerged between the children of panic disorder parents and children of normal controls on any of the spectral heart rate variability measures. However, children of patients with

Farm-based therapy: An innovative treatment approach for children, adolescents and young adults.

PURPOSE: To describe the rationale and components of Farm-based Therapy (FBT) as well as examine its preliminary efficacy. METHODS: FBT consists of five empirically supported components, or hands-on experience, sustainability, the life cycle, community, and mindfulness, that were developed in collaboration with clients, their caregivers, and clinicians. We explored the preliminary efficacy of FBT for improving anxiety and mood in new clients, and their parents, using the Revised Child Anxiety and Depression Scale (RCADS). RESULTS: We found that participants (N = 45) reported that their overa

Anxiety and substance use during adolescence.

BACKGROUND: The primary goal of this study was to examine the relationship between anxiety symptomatology and substance use (alcohol use and drug use) during adolescence, systematically by gender and race/ethnicity. METHODS: Self-report surveys were administered to 905 15-17-year-old adolescents (54% girls) in the spring of 2007. RESULTS: RESULTS from multiple group analyses indicated that the relationship between anxiety and substance use differs by gender and race/ethnicity. For Caucasian and African American boys, higher levels of social anxiety and separation anxiety were related to less

Developmental Epidemiology of Pediatric Anxiety Disorders.

This review summarizes the developmental epidemiology of childhood and adolescent anxiety disorders. It discusses the coronavirus disease of 2019 (COVID-19) pandemic, sex differences, longitudinal course, and stability of anxiety disorders in addition to recurrence and remission. The trajectory of anxiety disorders-whether homotypic (ie, the same anxiety disorder persists over time) or heterotypic (ie, an anxiety disorder shifts to a different diagnosis over time) is discussed with regard to social, generalized, and separation anxiety disorders as well as specific phobia, and panic disorder. Forced immigration-related family separation and adverse childhood experiences: effects on mental health among Latinx immigrant youth in the United States.

INTRODUCTION: First-generation Latinx immigrant youth face risks for experiencing unique immigration-related traumas, which could exacerbate their mental health. This study aimed to investigate the potential influence of forced immigration-related family separation on the association between traditional adverse childhood experiences (ACEs) and mental health (i.e., depression, anxiety, and PTSD symptoms) among Latinx immigrant youth from the Northern Triangle (NT; El Salvador, Guatemala, and Honduras). METHODS: Primary surveys assessing forced immigration-related family separation, ACEs, and m

Interactive music therapy as a treatment for preoperative anxiety in children: a randomized controlled trial.

UNLABELLED: In this study, we examined whether interactive music therapy is an effective treatment for preinduction anxiety. Children undergoing outpatient surgery were randomized to 3 groups: interactive music therapy (n = 51), oral midazolam (n = 34), or control (n = 38). The primary outcome of the study was children's perioperative anxiety. We found that children who received midazolam were significantly less anxious during the induction of anesthesia than children in the music therapy and control groups (P = 0.015 and P = 0.005, respectively). We found no difference in anxiety during the i

Familial analysis of panic disorder and agoraphobia. Gruppo Italiano Disturbi d'Ansia.

In this paper we studied the distribution of age at onset in 144 patients with panic disorder (PD) and/or agoraphobia (according to DSM-III criteria). We also investigated the incidences in their first-degree relatives of PD, agoraphobia, affective disorders, other anxiety disorders and alcoholism. Our aim was to detect the existence of a specific susceptibility system in these families, considering a wide spectrum of disorders. We found an earlier onset of PD and agoraphobia in the subgroup of our probands who previously met criteria for a separation anxiety disorder in childhood. The presenc

Developmental trajectories of anxiety disorders in offspring at high risk for panic disorder and major depression.

The objective of this study was to evaluate the longitudinal course of psychiatric disorders in children of parents with and without panic disorder and major depression as they transition through the period of risk from early to late childhood. Over a 5-year follow-up, we compared the course of psychiatric disorders in offspring of parents with panic disorder, major depression, or neither disorder. Subjects consisted of 233 offspring (from 151 families) with baseline and follow-up assessments. Subjects were comprehensively assessed with structured diagnostic interviews. Anxiety disorders at ba

Screening for anxiety disorders in children.

Anxiety disorders are highly prevalent and have negative consequences on individual and societal level. This study examined the usefulness of screening for anxiety disorders in primary school children. More specifically, the value of the screening method to discriminate between and to predict anxiety disorders was studied. Children and their parents were selected if the children had self-reported scores on the screening questionnaire Screen for Child Anxiety Related Emotional Disorders-71 (SCARED-71) within the top-15% (High-anxious) or from two points below to two points above the median (Med

Anxiety among youth with food allergy.

OBJECTIVE: Anxiety is among the major psychological concerns for children living with food allergy (FA). Yet research exploring the variables driving anxiety symptoms in FA remains sparse, and most studies still utilize homogeneous samples to assess anxiety symptoms. The current study seeks to evaluate the rates of clinically significant anxiety symptoms among a diverse sample of youth with FA and examine whether a heightened risk perception of FA outcomes and FA burden (vs. FA medical history) is associated with anxiety in youth. METHODS: 94 youth ages 10-14 and their parents were recruited

Memory deficits in children with and at risk for anxiety disorders.

There are limited data on the neurocognitive correlates of childhood anxiety disorders. The objective of this study was to examine whether visual and verbal memory deficits of nonemotional stimuli are (1) a shared feature of three common childhood anxiety disorders (social phobia, separation anxiety disorder, and generalized anxiety disorder) or whether these deficits are restricted to specific anxiety disorders, and (2) present in offspring who possess at least one of the following established risk factors for anxiety disorders, parental history of panic disorder (PD), or major depressive dis

Neurogenesis and generalization: a new approach to stratify and treat anxiety disorders.

Although an influence of adult neurogenesis in mediating some of the effects of antidepressants has received considerable attention in recent years, much less is known about how alterations in this form of plasticity may contribute to psychiatric disorders such as anxiety and depression. One way to begin to address this question is to link the functions of adult-born hippocampal neurons with specific endophenotypes of these disorders. Recent studies have implicated adult-born hippocampal neurons in pattern separation, a process by which similar experiences or events are transformed into discre

Comparative evaluation of intranasal dexmedetomidine and midazolam as premedication for children undergoing corrective surgery for congenital heart disease: A randomized clinical trial.

BACKGROUND: Premedication is essential in paediatric surgery for smooth induction and anxiety relief. Common agents-ketamine, midazolam, dexmedetomidine, and fentanyl-are administered via intramuscular injection, intravenous infusion, or oral routes, but efficacy varies and no consensus exists on an optimal protocol. Intranasal delivery offers a promising, more comfortable alternative with potential to improve outcomes. METHODS: This double-blind randomized trial evaluated intranasal dexmedetomidine (1 mcg/kg) (group D) versus midazolam (0.2 mg/kg) as premedication in 90 children (2-12 years)

Sleep-Related Problems and the Effects of Anxiety Treatment in Children and Adolescents.

This study examined (a) demographic and clinical characteristics associated with sleep-related problems (SRPs) among youth with anxiety disorders, and (b) the impact of anxiety treatment: cognitive-behavioral therapy (CBT; Coping Cat), medication (sertraline), their combination, and pill placebo on SRPs. Youth (N = 488, ages 7-17, 50% female, 79% White) with a principal diagnosis of generalized anxiety disorder, separation anxiety disorder, or social phobia participated. SRPs were reported by parents and youth. Findings differed by informant and by type of SRP, with evidence that SRPs are asso

Predictors of treatment outcome in an effectiveness trial of cognitive behavioral therapy for children with anxiety disorders.

A substantial number of children with anxiety disorders do not improve following cognitive behavioral therapy (CBT). Recent effectiveness studies have found poorer outcome for CBT programs than what is typically found in efficacy studies. The present study examined predictors of treatment outcome among 181 children (aged 8-15 years), with separation anxiety, social phobia, or generalized anxiety disorder, who participated in a randomized, controlled effectiveness trial of a 10-session CBT program in community clinics. Potential predictors included baseline demographic, child, and parent factor
