Transient ischaemic attack secondary to paraneoplastic erythrocytosis.

A 67-year-old woman with a history of hypertension and type 2 diabetes mellitus was admitted to the hospital due to aphasia and left-sided hemiparesis during the past 5 h with resolution of symptoms within 24 h. On admission laboratory analysis showed haemoglobin 19.2 g/dL and haematocrit 55.1%. Cerebral CT scan was also performed on admission revealing periventricular leucoaraiosis. Studies to investigate the cause of erythrocytosis were started and elevated erythropoietin levels were found. In order to investigate a secondary cause of erythrocytosis an abdominal ultrasound was conducted reve

Factors associated with health related quality of life of patients with stroke in Sri Lankan context.

BACKGROUND: Stroke is a major global health concern which affects the health related quality of life (HRQOL). As the prevalence of stroke is increasing especially in lower-middle income countries, it is vital to identify the factors associated with the HRQOL of affected individuals. Available literature for post stroke HRQOL and the associated factors are mainly from high income countries. Therefore, we conducted this study to identify the factors associated with HRQOL of stroke survivors using a stroke specific measure. METHODS: A longitudinal study was conducted with the participation of 25

Adult aphasic status epilepticus: A systematic review and patient-level synthesis of clinical phenotypes and multimodal diagnostic evidence.

BACKGROUND: Aphasic status epilepticus (ASE) is an uncommon manifestation of focal status epilepticus in which aphasia is the sole or predominant clinical feature. Recognition may be difficult because consciousness can be relatively preserved, convulsive activity may be absent, and the presentation can mimic acute stroke. OBJECTIVE: To characterize diagnostic phenotypes of adult ASE and synthesize clinical, electrographic, and neuroimaging features across published cases. METHODS: We conducted a systematic review and patient-level synthesis of adult ASE cases. Eligible patients were adjudica

Effect of semantic and metacognitive rehabilitation in semantic variant primary progressive aphasia: a single-case experimental study.

BACKGROUND: The semantic variant of primary progressive aphasia (svPPA), a rare neurodegenerative disorder, leads to anomia due to a progressive loss of word meaning. In the absence of pharmacological treatment, speech therapy is preferred in clinical practice. However, semantic rehabilitation, which has shown beneficial effects in vascular aphasia when used in combination with metacognitive training, is poorly studied in degenerative pathologies and specifically in svPPA. The aim of this study was to assess the effect of semantic therapy combined with metacognitive training on naming performa

Potential scalp acupuncture and brain stimulation targets for common neurological disorders: evidence from neuroimaging studies.

BACKGROUND: Scalp acupuncture is a promising potential therapy for neurological disorders. However, the development of its stimulation targets-both in identifying novel targets and refining the precision of their localization-has advanced slowly, largely due to the inadequate integration of brain science findings. This study leverages advances in brain neuroimaging to identify evidence-based cortical targets, enhancing the potential of scalp acupuncture and other brain stimulation techniques. METHODS: Using the Neurosynth Compose platform, systematic meta-analyses of neuroimaging studies were

A Comparison of Item Acquisition and Response Generalization for Semantic Versus Phonological Treatment of Aphasia.

PURPOSE: The purpose of this work is to examine whether therapy-related improvements in trained versus untrained items (acquisition and response generalization, respectively) are differentially affected by phonological versus semantic language treatments and to investigate individual variables associated with treatment response. METHOD: Sixty-three participants with chronic poststroke aphasia were included in this retrospective analysis of data from a large, multisite clinical trial with an unblinded cross-over design in which all participants underwent 3 weeks of semantic treatment and 3 wee

An Investigation of Compensation and Adaptation to Auditory Perturbations in Individuals With Acquired Apraxia of Speech.

Two auditory perturbation experiments were used to investigate the integrity of neural circuits responsible for speech sensorimotor adaptation in acquired apraxia of speech (AOS). This has implications for understanding the nature of AOS as well as normal speech motor control. Two experiments were conducted. In Experiment 1, compensatory responses to unpredictable fundamental frequency (F0) perturbations during vocalization were investigated in healthy older adults and adults with acquired AOS plus aphasia. F0 perturbation involved upward and downward 100-cent shifts versus no shift, in equal

Digital Therapeutics: Emerging New Therapy for Neurologic Deficits after Stroke.

Digital therapeutics is an evidence-based intervention using high-quality software, with the sole purpose of treatment. As many healthcare systems are encountering high demands of quality outcomes, the need for digital therapeutics is gradually increasing in the clinical field. We conducted review of the implications of digital therapeutics in the treatment of neurological deficits for stroke patients. The implications of digital therapeutics have been discussed in four domains: cognition, speech and aphasia, motor, and vision. It was evident that different forms of digital therapeutics such a

Cognitive and cognate-based treatments for bilingual aphasia: a case study.

Two consecutive treatments were conducted to investigate skill learning and generalization within and across cognitive-linguistic domains in a 62-year-old Spanish-English bilingual man with severe non-fluent aphasia. Treatment 1 was a cognitive-based treatment that emphasized non-linguistic skills, such as visual scanning, categorization, and simple arithmetic. Treatment 2 was a lexically based treatment that trained cognates (cross-linguistic word pairs that are similar in meaning and form, such as rosa/rose) and non-cognates (cross-linguistic word pairs with shared meaning but different form

Two systems for colour-naming defects: verbal disconnection vs colour imagery disorder.

Two subjects affected by pure alexia and showing no central dyschromatopsia or generalized aphasia, performed poorly on traditional tasks with visually-presented colour stimuli and on tasks with objects presented verbally. Three experiments were conducted to evaluate the possible role of mental colour imagery in recalling the colours of objects from memory. It was concluded that Case I, with left occipital lobe softening, had preserved imagery systems, but failed to recode the colours of mentally generated colour images, just as he failed to name visually presented colours, suggesting a langua

A Conversational Agent for Early Detection of Neurotoxic Effects of Medications through Automated Intensive Observation.

We present a fully automated AI-based system for intensive monitoring of cognitive symptoms of neurotoxicity that frequently appear as a result of immunotherapy of hematologic malignancies. Early manifestations of these symptoms are evident in the patient's speech in the form of mild aphasia and confusion and can be detected and effectively treated prior to onset of more serious and potentially life-threatening impairment. We have developed the Automated Neural Nursing Assistant (ANNA) system designed to conduct a brief cognitive assessment several times per day over the telephone for 5-14 day

Experiences and perspectives of UK speech and language therapists on telehealth assessment with people living with post-stroke aphasia.

BACKGROUND: Stroke care in the UK was significantly affected by the COVID-19 pandemic, with many services switching to telehealth. Post-pandemic, a UK survey of speech and language therapists (SLTs) working with people with aphasia (PWA) showed the vast majority planned to continue to use telehealth alongside in-person intervention. Telehealth is considered a cost-effective and feasible method of service delivery; however, there is limited evidence to support its use in the assessment of people with post-stroke aphasia. AIMS: To investigate what barriers and facilitators SLTs experience when
