A treatment for dysprosody in childhood apraxia of speech.

PURPOSE: Dysprosody is considered a core feature of childhood apraxia of speech (CAS), especially impaired production of lexical stress. Few studies have tested the effects of intervention for dysprosody. This Phase II study with 3 children investigated the efficacy of a treatment targeting improved control of relative syllable durations in 3-syllable nonwords representing strong-weak (SW) and weak-strong (WS) stress patterns (e.g., BAtigu or baTIgu). Treatment sessions were structured along the principles of motor learning (PML) approach. METHOD: Three children, age 7 to 10 years, with mild

Vowel acoustics in adults with apraxia of speech.

PURPOSE: To investigate the hypothesis that vowel production is more variable in adults with acquired apraxia of speech (AOS) relative to healthy individuals with unimpaired speech. Vowel formant frequency measures were selected as the specific target of focus. METHOD: Seven adults with AOS and aphasia produced 15 repetitions of 6 American English vowels in /hVC/ context (hid, head, hat, hot, hub, hoot). Vowel formant frequency measures (F1, F2) were Bark transformed and compared with data from archival sources. RESULTS: Measures of vowel acoustics in speakers with AOS did not differ from th

Cognitive approach to the assessment of limb apraxia.

A new instrument for the assessment of the different levels of gesture processing, as identified by recent cognitive models of apraxia, is presented. The battery comprises thirteen tasks -- eight assess the production of meaningful gestures both on command and on imitation, four tasks assess the ability to recognize and identify gestures, and one task assesses imitation of meaningless gestures. The battery encompasses a novel test of gesture production on visual command. A total of 60 healthy British volunteers were tested with the entire battery. On the whole, participants made more errors wi

[Functional imaging insights into the pathophysiology of apraxia].

Apraxias are disorders of motor cognition that cannot be explained by basic sensorimotor deficits or aphasia. The relatively high frequency of apraxia (approximately half of all patients with left-hemispheric stroke suffer from apraxia during the acute phase) as well as its prognostic value for determining the outcome of rehabilitative therapy clearly convey the necessity of more comprehensive research into the pathophysiology of apraxia in order to develop new therapeutic strategies. In recent years, functional imaging (PET and fMRI) has helped to provide important new insights into the patho

Grasping tools: effects of task and apraxia.

In patients with apraxia the ability to access tool-use-knowledge and to integrate it into a movement plan is frequently disturbed. The present study investigated potential differences between a 'transport'- and a 'use'-task as well as the influence of apraxia on the preceding grasping movement, when the tool is presented with its handle pointing away from the participant. 20 controls, 10 patients with right brain damage (RBD-group) and 17 patients with left brain damage (LBD-group) were tested. 10 LBD-patients were classified as moderately to severely apraxic according to erroneous demonstrat

Evolutionary autonomous agents and the nature of apraxia.

BACKGROUND: Evolutionary autonomous agents are robots or robot simulations whose controller is a dynamical neural network and whose evolution occurs autonomously under the guidance of a fitness function without the detailed or explicit direction of an external programmer. They are embodied agents with a simple neural network controller and as such they provide the optimal forum by which sensorimotor interactions in a specified environment can be studied without the computational assumptions inherent in standard neuroscience. METHODS: Evolutionary autonomous agents were evolved that were able

Neurodevelopmental implications of ocular motor apraxia.

Ocular motor apraxia (OMA), a disorder of saccadic initiation, may be congenital or acquired. While the acquired form is frequently associated with significant neuropathology, the congenital form is often regarded as relatively benign. Many children with congenital OMA who were observed clinically have shown neurodevelopmental disturbance over time. A retrospective review was taken of 34 consecutive patients (22 males and 12 females), seen over a 20-year period, to evaluate the frequency and type of associated neurodevelopmental problems. Age at presentation ranged from 8 weeks to 14 years, wi

Tapping, grasping and aiming in ideomotor apraxia.

Very few studies have investigated sensorimotor control in apraxia using tasks that differ in movement complexity. Nevertheless, there is some evidence to suggest that spontaneous behaviour, although relatively preserved, can be rather clumsy or awkward, and that patients with ideomotor apraxia may have subtle kinematic abnormalities in movements made in the laboratory. It remains unclear whether patients with ideomotor apraxia perform normally on movements such as visually guided aiming, that may not depend on higher-order, more cognitive, processes and that are relatively unguided by overlea

[Alterations in the imitation of gestures (conduction apraxia)].

INTRODUCTION AND AIMS: The aim of this presentation is to report the performance pattern of a patient who suffered ideomotor apraxia with a disorder pattern of the conduction apraxia (CA) type. This clinical picture was originally reported by Ochipa et al. in 1994 as an alteration in the pathway that joins the two lexicons; later, in 2000, Cubelli et al. claimed that there is no evidence for the existence of such a pathway and suggested that the symptoms were due to an alteration affecting the mechanisms governing visuomotor conversion. CASE REPORT: A 51 year old patient who, following a trau

Crossed apraxia of speech: a case report.

The present study reports on the first case of crossed apraxia of speech (CAS) in a 69-year-old right-handed female (SE). The possibility of occurrence of apraxia of speech (AOS) following right hemisphere lesion is discussed in the context of known occurrences of ideomotor apraxias and acquired neurogenic stuttering in several cases with right hemisphere lesion. A current hypothesis on AOS-the dual route speech encoding (DRSE) hypothesis-and predictions based on DRSE were utilized to explore the nature of CAS in SE. One prediction based on the DRSE hypothesis is that there should be no differ

Familial congenital ocular motor apraxia.

Congenital ocular motor apraxia (COMA) is a unique ocular motor disorder which is characterized by a deficit in initiation of voluntary horizontal eye movement with reserved reflex eye movement. Although a portion of cases with COMA were found to be associated with other abnormalities, COMA in most patients is an isolated disorder. The most characteristic appearance of these patients is compensatory head thrusts which usually become less evident with increasing age. Since Cogan first described COMA in 1952, many cases have been reported. The majority of these occurred sporadically with only a

Dysarthria and orofacial apraxia in corticobasal degeneration.

The authors evaluated dysarthria and orofacial apraxia (OFA) in 10 patients with a clinical diagnosis of corticobasal degeneration (CBD). Nine patients were slightly dysarthric according to the French version of the Frenchay Dysarthria Assessment, which evaluates the motricity of the components of the vocal tract. The severity of dysarthria assessed by an intelligibility score was correlated to the global severity of the disease, but not to the duration of the disease. Voluntary movements of the tongue and the lips were impaired in all patients. OFA, evaluated with simple and sequential gestur

Task demands and limb apraxia in stroke.

The present study was designed to examine the frequency and severity of apraxia in patients with left- or right-hemisphere stroke in both pantomime and imitation conditions and to compare the frequency of apraxia in each stroke group across the three patterns of apraxia described in Roy's model (Roy, 1996). Ninety-nine stroke patients and 15 age-matched healthy adults performed eight transitive gestures to pantomime and to imitation. Gestural performance was quantified as accuracy on five performance dimensions; a composite score, an arithmetic combination of the five performance dimensions, w

Pathologic correlates of apraxia in Alzheimer disease.

OBJECTIVE: To examine the neuroanatomical correlates of apraxia in Alzheimer disease. PATIENTS: Twenty-three patients with clinically overt Alzheimer disease. DESIGN: Anterograde study and neuropathologic case series. Clinical severity was assessed using the Global Deterioration Scale. Ideomotor praxis was examined on transitive and intransitive movements and meaningless gestures, and dressing ability was evaluated clinically. Constructive praxis was tested using a 3-dimensional figure copying task. Correlations between neurofibrillary tangle and senile plaque densities and praxis test perfo

A reconceptualisation of apraxia of speech: a synthesis of evidence.

This paper presents a reconceptualisation of apraxia of speech (AOS) and reviews a range of acoustic and articulatory data which have been reported on apraxia of speech. Data from both domains, and previous theoretical debate are discussed as support for this cognitive-based reconceptualisation.
