BACKGROUND: Stent under-expansion is a main cause of acute coronary syndrome (ACS), which can lead to serious clinical outcomes. The rotational atherectomy of underexpanded coronary stents (academically called stent ablation, SA) by intravascular ultrasound (IVUS) may provide more visual reference in the intervention. We aim to analyze the procedural and long-term outcomes of the optimized strategy of SA in patients with ACS and to provide real-world data on this technique.
METHODS: A total of 11 patients with ACS who underwent SA between April 2017 and January 2019 were analyzed. Clinical fo

BACKGROUND/PURPOSE: Coronary artery calcification is a marker of advanced atherosclerosis and a predictor of adverse clinical outcomes. Rotational atherectomy (RA) can effectively modify calcified lesions, optimizing procedural outcomes. We interrogated the most commonly reported adverse events involving rotational atherectomy systems (Rotablator and Rotapro) by analyzing post-marketing surveillance data from the Food and Drug Administration Manufacturer and User Facility Device Experience (MAUDE) database.
METHODS/MATERIALS: We queried MAUDE from September 1, 2016, through December 31, 2019.

BACKGROUND: Debulking and ablative techniques are sometimes used for the treatment of in-stent restenosis (ISR) secondary to resistant stent under-expansion (SU). The safety and effectiveness of orbital atherectomy (OA) in this cohort of patients has not been reported.
METHODS: We retrospectively evaluated consecutive patients treated with OA for ISR secondary to balloon undilatable SU at two academic tertiary care centers between October 2016 and June 2019. Angiographic or intravascular imaging identified SU. Technical success was defined as residual 0% stenosis with TIMI III flow.

The presence of severe coronary artery calcification is associated with higher rates of angiographic complications during percutaneous coronary intervention (PCI), as well as higher major adverse cardiac events compared with non-calcified lesions. Incorporating orbital atherectomy (OAS) for effective preparation of severely calcified lesions can help maximize the benefits of PCI by attaining maximal luminal gain (or stent expansion) and improve long-term outcomes (by reducing need for revascularization). Areas covered: In this manuscript, the prevalence, risk factors, and impact of coronary ar

Atherectomy, the removal of plaque from diseased vessels, is theoretically appealing with respect to various procedures used for revascularization of lower extremity vessels. Instead of damaging the native vessel by pushing aside plaque with a balloon or stent, the plaque is removed. Many atherectomy devices have been designed in attempts to achieve this goal. The SilverHawk device is the latest percutaneous device employing this treatment strategy. We discuss patient selection, technical considerations, and strategies for both above and below the knee revascularization, as well as pitfalls in

INTRODUCTION: Rotational atherectomy is usually performed in patients with angiographically determined high risk coronary lesions. The aim of this study was to evaluate the rate of major adverse cardiac events (death, Q-wave infarction or new revascularization) after rotational atherectomy, as well as to identify the clinical characteristics associated with this incidence.
PATIENTS AND METHODS: The study population included 800 patients treated with rotational atherectomy from 1993 to 1999: 512 (64%), for de novo lesions, and 288 (36%) for restenosis. Balloon dilation and coronary stenting wa

OBJECTIVES: This study evaluated the clinical safety and long-term results of rotational atherectomy (RA) followed by low-pressure balloon dilatation (percutaneous transluminal coronary angioplasty [PTCA]) for the treatment of in-stent restenosis (ISR).
BACKGROUND: In-stent restenosis is associated with a high incidence of recurrence after interventional treatment. Because ISR is due to neointimal hyperplasia, rotational ablation may be a more effective treatment than PTCA.
METHODS: Between November 1995 and November 1996, 100 consecutive patients with first-time ISR were treated by RA. Quan

OBJECTIVES: Data on the safety and efficacy of percutaneous coronary intervention (PCI) with adjunctive rotational atherectomy (RA) in left main (LM) disease are limited. Current guidelines recommend using a small-size RA burr initially, with upsizing if required. The safety of upfront use of a large-size RA burr (≥1.75 mm) is unknown. The authors assessed the safety of an upfront large rotablation burr in the treatment of LM calcification and the procedural and long-term outcome of this approach in comparison to a small-size burr.
METHODS: Retrospective analysis of all patients undergoing PC

BACKGROUND: Intravascular lithotripsy (IVL) is a novel calcium modification strategy with little evidence of efficacy compared to widely used rotational atherectomy (RA).
AIMS: We sought to compare the efficacy and safety level between IVL and RA in moderate-to-severe coronary artery calcification (CAC).
METHODS: We searched three databases until March 2025 for eligible observational studies and randomized controlled trials. The primary endpoint was major adverse cardiac event (MACE) while secondary endpoints were clinical outcomes, periprocedural complication, and procedural outcomes. Mante

BACKGROUND: A combined strategy of atherectomy followed by intravascular lithotripsy (dual-prep strategy) for severely calcified lesions may optimize stent deployment, but mechanistic evidence from prospective optical coherence tomography (OCT) imaging is limited. This study was designed to comprehensively characterize patterns of calcium modification during a dual-preparation strategy and to explore imaging determinants of calcium fracture and stent expansion using serial and 3-dimensional OCT assessment.
METHODS: This was a prespecified, multicenter, single-arm OCT substudy of the DUAL-PREP

OBJECTIVES: Calcified coronary artery disease (CAD) is prevalent in elderly patients with degenerative severe aortic stenosis (AS). Preparation of such calcified CAD using rotational atherectomy (RA) in those patients with severe AS is controversial and may carry a high risk of complications. The authors aimed to compare in-hospital outcomes following RA in patients with severe AS before transcatheter aortic valve implantation (TAVI) vs patients without AS.
METHODS: The authors retrieved data from the Prospective Segeberg TAVI Registry from January 2016 to October 2021. All AS patients who un

Coronary perforation (CP) is a rare but one of the most serious complications of rotational atherectomy (RA). The presence of a "tenting sign" on intravascular ultrasound, which reflects outward deformation of the normal vessel wall due to extreme wire bias, may indicate an elevated risk of CP following RA. We report a case of CP in the left circumflex coronary artery that occurred at a site where the tenting sign was observed on IVUS before RA.

Coronary perforation (CP) is an uncommon but serious complication of orbital atherectomy (OA). Optical coherence tomography (OCT) evidence of catheter "tenting" against the healthy vessel wall may indicate unsatisfactory guidewire bias and elevated perforation risk during OA. We describe OCT findings of a case with OA-related CP.

BACKGROUND: Modified balloons (MB) and rotational atherectomy (RA) are recommended tools for treatment of coronary plaques with superficial calcium. Knowledge about in-hospital safety is limited.
METHODS: Patients with coronary artery disease who underwent coronary angiography with RA or MB angioplasty in Germany were identified via ICD and OPS codes from 2017 to 2020. Acute coronary syndromes were excluded. Since patients were not randomized toward MB or RA, potential confounding factors were taken into account using the propensity score methods. Thereby, inverse probability weighting was ap

The prognostic implications of cardiac troponin elevation after percutaneous coronary intervention (PCI) with atherectomy have not been established. The aim of this study was to investigate the incidence of periprocedural myocardial injury (PMI) and its association with cardiovascular events in patients with severely calcified lesions who underwent PCI with atherectomy. The study analyzed 346 patients (377 lesions) who underwent PCI with atherectomy between January 2018 and December 2021. Peak post-PCI high-sensitivity cardiac troponin (hs-cTn) was measured. The primary outcome was target lesi

This study evaluated whether intravascular ultrasound (IVUS) examination before rotational atherectomy (RA) can predict the optimal route of passage of the RA burr along the vessel. 30 patients with calcified lesions who underwent IVUS before and immediately after RA were enrolled. IVUS analyses were performed at the minimum lumen area (MLA) site and at 0.5 mm intervals. Each IVUS cross-section was divided into 4 quadrants around the center of the lumen, and pre- and post-RA IVUS cross-section images were merged. Of 1140 cross-sections, 498 (44%) contained debulked regions. When the guidewire

There has been a resurgence of atherectomy as a strategy to treat peripheral atherosclerosis. Distal embolization occurs frequently during peripheral endovascular interventions but is of particular concern during atherectomy. We describe a case of distal embolization following atherectomy with the Pathway JetStream system successfully managed with aspiration thrombectomy and infrapopliteal angioplasty. A detailed review of the incidence, treatment, and prevention of DE in this context are presented.

BACKGROUND & AIMS: Heavily calcified coronary artery disease remains one of the most formidable challenges in contemporary percutaneous coronary interventions, often leading to suboptimal stent expansion, procedural complications, and poor long-term outcomes. Over the years, a wide range of calcium modification techniques have been developed, including cutting and scoring balloons, rotational atherectomy, orbital atherectomy, high-pressure balloons, intravascular lithotripsy, and laser atherectomy, each with unique mechanisms, strengths, and limitations. This article aims to provide a comprehe
