Sensory neurologic disorders: Tinnitus.

Tinnitus is the sensation of hearing a sound with no external auditory stimulus present. It is a public health issue correlated with multiple comorbidities and precipitating factors such as noise exposure, military service, and traumatic brain injury, migraine, insomnia, small vessel disease, smoking history, stress exposure, anxiety, depression, and socioeconomic status. Clinical experience and a recent literature review point at tinnitus as a neuropsychiatric condition involving both auditory and nonauditory cortical areas of the brain and affecting brain-auditory circuitry. In fact, brain-e

The aging auditory system: anatomic and physiologic changes and implications for rehabilitation.

Over the last century, research in the area of age-related hearing loss has provided a vast amount of knowledge regarding age-related effects on the anatomy and physiology of the auditory system. As we enter the new millennium, researchers are beginning to shift their attention towards developing methods of modulating the effects of age-related hearing loss and the development of efficacious intervention strategies to meet all of an individual's hearing-related rehabilitative needs. The purpose of this review is to provide a framework for considering how the biological aspects of the aging aud

Differential susceptibility to gentamicin ototoxicity between albino and pigmented guinea pigs.

The known chemical affinity of melanin pigment for aminoglycoside antibiotics has led to the suggestion that higher concentrations of these drugs will bind to the pigmented inner ear and produce greater ototoxicity compared to the nonpigmented albino cochlea. Although this has provided a compelling hypothesis, results from the few investigations to address this question have been equivocal. In the present study, cochlear microphonic (CM) thresholds were recorded from albino and pigmented guinea pigs both before and two weeks after exposure for 14 consecutive days to 100 mg/Kg gentamicin. Cochl

Functional organization of some auditory nuclei in the guinea fowl demonstrated by the 2-deoxyglucose technique.

The auditory pathway of the Guinea Fowl was labeled with [C14]2-deoxy-D-glucose after stimulation with pure tones, harmonic tones and species-specific calls. In addition to other auditory nuclei, which showed more or less uniform labeling with the present technique, the n. mesencephalicus lateralis dorsalis (MLD) of the midbrain, as well as field L and parts of the hyperstriatum ventrale in the telencephalon, showed a stripe-pattern of labeling after stimulation with a pure tone. The position and orientation of the tone-activated striped areas in field L, observed after stimulation with differ

The effects of aging on evoked otoacoustic emissions and efferent suppression of transient evoked otoacoustic emissions.

OBJECTIVE: There is still controversy regarding the effects of aging on evoked otoacoustic emissions (EOAEs), as well as on the efferent system measured by contralateral acoustic stimulation of EOAEs. The purpose of this study was to investigate the deterioration in EOAEs and efferent suppression (ES) in a representative sample statistically controlling for the differences in hearing thresholds. METHODS: Seventy-one ears (20-79years) were included in the study, 47 of which had normal hearing thresholds, and 24 ears had a sensorineural high-frequency hearing loss caused by presbycusis. The eff

Reinterpreting the Frequency Dependence of Cortical Auditory-Evoked Response Amplitudes in Light of Current Understanding of Cortical Tonotopic Organization.

Tonotopy is a fundamental feature of auditory cortical organization, yet its influence on cortical auditory-evoked responses (AERs) remains unclear. Consequently, key properties of cortical AERs-such as their marked amplitude reduction with increasing stimulus frequency-still lack a coherent mechanistic explanation. To address this gap, we combined a meta-analysis of frequency-specific AER amplitudes with forward simulations of AERs informed by current knowledge of auditory cortical tonotopic layout and functional organization. The meta-analysis used a semi-systematic search covering all known

Serotonergic Modulation of Sensory Representation in a Central Multisensory Circuit Is Pathway Specific.

Many studies have explored how neuromodulators affect synaptic function, yet little is known about how they modify computations at the microcircuit level. In the dorsal cochlear nucleus (DCN), a region that integrates auditory and multisensory inputs from two distinct pathways, serotonin (5-HT) enhances excitability of principal cells, predicting a generalized reduction in sensory thresholds. Surprisingly, we found that when looked at from the circuit level, 5-HT enhances signaling only from the multisensory input, while decreasing input from auditory fibers. This effect is only partially expl

A tale of two ears: development of binaural auditory processing in the big brown bat.

Big brown bat (Eptesicus fuscus) pups are born naked, blind, and with closed external ear canals. We examined the ears of newborn bat pups (n = 82; 42 females) and found that in a majority of pups (80%), the ear canals open asymmetrically. What remains unknown is whether this developmental asymmetry influences hearing. We recorded auditory brainstem response (ABR) waveforms in a subset of pups with both ears open by placing subdermal needle electrodes near the base of the auditory bullae and repeatedly presenting broadband acoustic clicks and averaging the evoked neural response. The resultant

Effects of long-term non-traumatic noise exposure on the adult central auditory system. Hearing problems without hearing loss.

It is known that hearing loss induces plastic changes in the brain, causing loudness recruitment and hyperacusis, increased spontaneous firing rates and neural synchrony, reorganizations of the cortical tonotopic maps, and tinnitus. Much less in known about the central effects of exposure to sounds that cause a temporary hearing loss, affect the ribbon synapses in the inner hair cells, and cause a loss of high-threshold auditory nerve fibers. In contrast there is a wealth of information about central effects of long-duration sound exposures at levels ≤80 dB SPL that do not even cause a tempora

The injured cochlea as a target for inflammatory processes, initiation of cell death pathways and application of related otoprotectives strategies.

One of the causes of sensorineural hearing loss is the loss of auditory hair cells following exposure to environmental stresses. Auditory hair cell death in response to cochlear trauma occurs via both necrosis and apoptosis. Apoptosis of hair cells involves the caspase and MAPK/JNK pathways which are activated by oxidative stress and secretion of inflammatory cytokines in response to trauma. Identification of the pathways that lead to apoptosis provides therapeutic targets for the conservation of hearing. Antioxidants reduce the level of reactive oxygen species and reactive nitrogen species ge

Subcortical modulation in auditory processing and auditory hallucinations.

Hearing perception in individuals with auditory hallucinations has not been well studied. Auditory hallucinations have previously been shown to involve primary auditory cortex activation. This activation suggests that auditory hallucinations activate the terminal of the auditory pathway as if auditory signals are submitted from the cochlea, and that a hallucinatory event is therefore perceived as hearing. The primary auditory cortex is stimulated by some unknown source that is outside of the auditory pathway. The current study aimed to assess the outcomes of stimulating the primary auditory co

Cochlear neural contributions to triple network changes in tinnitus, hyperacusis & misophonia? A perspective review.

What do tinnitus, the perception of sounds without an internal or external source of noise, hyperacusis, the pathological hypersensitivity to noise, or misophonia, an intolerance to certain everyday noises, have in common, and what differentiates them? A large number of excellent studies focused in the last few decades on identifying the neural correlates of tinnitus, hyperacusis, or misophonia on the basis of central triple-network changes. In this perspective review we explicitly examine, possible differential and causal involvement of peripheral components as a presumptive trigger that may

Psychophysics and neurophysiology of repetition noise processing in a vertebrate auditory system.

The psychophysics and neurophysiology of repetition noise (RN) processing was studied in the goldfish. RN is the sum of a noise waveform with its delayed (by T s) repetition, which may be attenuated (by A dB), and inverted relative to the undelayed signal. Such a signal has a periodic spectrum with peaks separated by 1/T Hz, and a prominence in its autocorrelation function at T s. In usual environments, RN contains information about sound-reflecting surfaces. Delays in the range of 0.5-20 ms create pitch sensations in man. Psychophysical experiments using classical respiratory conditioning inv

Medial olivocochlear function in children with poor speech-in-noise performance and language disorder.

OBJECTIVES: Contralateral masking of transient-evoked otoacoustic emissions is a phenomenon that suggests an inhibitory effect of the olivocochlear efferent auditory pathway. Many studies have been inconclusive in demonstrating a clear connection between this system and a behavioral speech-in-noise listening skill. The purpose of this study was to investigate the activation of a medial olivocochlear (MOC) efferent in children with poor speech-in-noise (PSIN) performance and children with language impairment and PSIN (SLI + PSIN). METHODS: Transient evoked otoacoustic emissions (TEOAEs) with a

The neural representations underlying asymmetric cross-modal prediction of words.

Cross-modal prediction serves a crucial adaptive role in the multisensory world, yet the neural mechanisms underlying this prediction are poorly understood. The present study addressed this important question by combining a novel audiovisual sequence memory task, functional magnetic resonance imaging (fMRI), and multivariate neural representational analyses. Our behavioral results revealed a reliable asymmetric cross-modal predictive effect, with a stronger prediction from visual to auditory (VA) modality than auditory to visual (AV) modality. Mirroring the behavioral pattern, we found the sup

Delayed Auditory Brainstem Responses (ABR) in children after sight-recovery.

Studies in non-human animal models have revealed that in early development, the onset of visual input gates the critical period closure of some auditory functions. The study of rare individuals whose sight was restored after a period of congenital blindness offers the rare opportunity to assess whether early visual input is a prerequisite for the full development of auditory functions in humans as well. Here, we investigated whether a few months of delayed visual onset would affect the development of Auditory Brainstem Responses (ABRs). ABRs are widely used in the clinical practice to assess b

Using resting state functional connectivity to unravel networks of tinnitus.

Resting state functional connectivity (rs-fc) using fMRI has become an important tool in examining differences in brain activity between patient and healthy populations. Studies employing rs-fc have successfully identified altered intrinsic neural networks in many neurological and psychiatric disorders, including Alzheimer's disease, schizophrenia, and more recently, tinnitus. The neural mechanisms of subjective tinnitus, defined as the perception of sound without an external source, are not well understood. Several inherent networks have been implicated in tinnitus; these include default mode

Auditory evoked brain stem potentials (seven waves within the first 10 milliseconds after the stimulus) obtained from the intercalated nuclei within the auditory pathway by averaging are suitable for a functional diagnosis of the brain stem. Disorders affecting the auditory pathway (direct or indirect tumour pressure or infiltrates, demyelinisation processes, traumatic brain stem injuries, ischemia) from the acoustic nerve through the cochlear nuclei, the superior olivary nuclei, the nuclei of the lateral lemnisci, the inferior collicular nuclei, to the medial geniculate bodies can be identifi

Two types of actions of norepinephrine on identified auditory efferent neurons in rat brain stem slices.

Whole cell voltage-clamp recordings were performed on auditory olivocochlear neurons in the ventral nucleus of the trapezoid body (VNTB) of brain stem slices from neonatal rats. Each neuron was identified by retrograde labeling with Fast Blue injected into the cochlea. Bath application of norepinephrine (NE; 1-10 microM) reversibly induced an inward current in 26 of 38 labeled neurons that were voltage clamped at -75 mV. This was responsible for the membrane depolarization to NE observed in current-clamp mode. The NE-induced inward current appeared to be more prominent at -55 mV than at -75 mV

Chronic in utero alcohol exposure affects auditory function in rats and in humans.

In one experiment, brainstem auditory evoked potentials (BAEPs) were used to study the postnatal development of the peripheral and brainstem auditory pathways in rat pups prenatally exposed to alcohol. The results indicated that prenatal alcohol exposure retards development of the peripheral and brainstem auditory pathways and that prolonged auditory transmission times, despite a "catch-up" trend, persist in maturity. BAEP latency-intensity profiles demonstrated that a significant proportion (19%) of the alcoholized rats had recruitment-type sensorineural hearing loss. In a second study, a gro
