Perioperative safety in the longitudinal assessment of bariatric surgery.

BACKGROUND: To improve decision making in the treatment of extreme obesity, the risks of bariatric surgical procedures require further characterization. METHODS: We performed a prospective, multicenter, observational study of 30-day outcomes in consecutive patients undergoing bariatric surgical procedures at 10 clinical sites in the United States from 2005 through 2007. A composite end point of 30-day major adverse outcomes (including death; venous thromboembolism; percutaneous, endoscopic, or operative reintervention; and failure to be discharged from the hospital) was evaluated among patien

Is preoperative manometry in restrictive bariatric procedures necessary?

BACKGROUND: Restrictive bariatric procedures are frequently considered for patients with morbid obesity, because the weight loss and reduction of comorbidities are good. An impact on gastroesophageal reflux disease (GERD), which is common in this population, may be anticipated. Converse results of GERD symptoms are reported for patients after adjustable gastric banding (AGB), sleeve gastrectomy (SG), and Roux-en-Y gastric bypass (RYGBP). METHODS: A literature search was performed and, with our personal experience, are summarized. RESULTS: Esophageal manometry is a practical tool to identify

Nutritional and psychological considerations after bariatric surgery.

The anatomic and physiological changes resulting from bariatric surgery can affect both nutrition and psychological attitudes. Modifications of the gastrointestinal tract lead to a diminished ability to absorb nutrients, electrolytes, and bile salts, as well as deficiencies in iron, calcium, and other vitamins and minerals. Dehydration, lactose intolerance, and protein calorie malnutrition are other common sequelae. Alterations in bone metabolism increase long-term risk for osteopenia and osteoporosis. Noncompliance with postsurgical nutritional regimens has been estimated to occur in from one

Vitamin D and the bariatric surgical patient: a review.

OBJECTIVE: Adiposity has been inversely associated with vitamin D concentration across a range of body mass index values and cultural groups. As obesity has increased markedly worldwide, a greater number of patients with severe obesity have been treated with gastric restrictive and/or malabsorptive surgical procedures. The purpose of this review was to describe current knowledge about vitamin D and severe obesity, and the impact of obesity surgery on vitamin D status. RESEARCH METHODS AND PROCEDURES: A systematic review was conducted with search terms obesity, vitamin D, osteoporosis, bone di

Gastrointestinal complications of bariatric surgery: diagnosis and therapy.

Severe or morbid obesity, with body mass indexes exceeding 35 to 40, are often refractory to all therapies other than surgery. The increasing number of patients undergoing bariatric surgery will result in increasing numbers of patients with gastrointestinal complications. The types of complications vary with type of surgery, whether restrictive, malabsorptive, or both, depending on what anatomical and physiologic changes occur postoperatively. One complication of bariatric surgery (gallstones) is due to weight loss after surgery, not the surgery itself. Based on previous meta-analyses, most of

Factors influencing patient choice for bariatric operation.

BACKGROUND: No study has surveyed the factors that influence morbidly obese patients' preference for a particular bariatric operation. METHOD: 469 consecutive patients in 2 major bariatric surgery centers in the United States (US, 124) and Australia (AU, 345) were prospectively studied to determine referral pattern and reason for their choice of operation. RESULTS: The predominant operation was laparoscopic adjustable gastric banding (LAGB) in both US (75%) and AU (83%) centers. Gender (70% female), BMI (45 kg/m2) and age (42.5 years) were similar in both cohorts. In Australia, 53% had refer

[Bariatric surgery for pseudotumor cerebri. Case report].

INTRODUCTION: We report here a case of a patient with pseudotumor cerebri (PTC) associated with morbid obesity. PTC, also called benign intracranial hypertension, is a known complication of morbid obesity with resolution by reduction of obesity. CLINICAL CASE: A 42-year-old female with morbid obesity and secondary PTC underwent a modified jejunocolonostomy. RESULTS: The patient has lost 43% of excess body weight and the intracranial pressure is normal. CONCLUSIONS: PTC is a benign pathology, and the cause may be morbid obesity. Bariatric surgery is useful in the management of these patients

[The role of bariatric surgery and endoscopy in the management of MASLD].

The epidemic of metabolic dysfunction-associated steatotic liver disease (MASLD), amplified by shifting dietary habits and increasing sedentary lifestyles, has elevated metabolic dysfunction-associated steatohepatitis (MASH) to a major public health challenge, burdened by the threat of cirrhosis and hepatocellular carcinoma. In this alarming context, bariatric surgery has progressively established itself as a key therapeutic solution, transforming the prognosis for patients with severe forms of the disease. Its efficacy extends beyond simple weight loss: it improves metabolism, reduces hepatic

Prevalence of post-bariatric hypoglycemia in the United States.

BACKGROUND: Post-bariatric hypoglycemia (PBH) is a complication of bariatric surgery in which postprandial hypoglycemia can be severe, frequent, and debilitating. The incidence reported in the literature varies from .1% to 75% of all bariatric procedures, largely due to differences in methodologies and definitions used to identify incident cases. OBJECTIVES/METHODS: To define the burden of PBH, the incidence and prevalence must be ascertained, with accurate definitions of incident cases used. We performed a systematic review of published literature with the following goals: 1) Outline differe

Unplanned re-hospitalization after bariatric surgery.

Bariatric surgery is a standard treatment for obesity and a number of its complications. Although surgical complications are relatively rare, some patients must return to the emergency department or to a facility far removed in place and time from the original surgery. The purpose of this update is to outline the main reasons for short, medium, and long-term emergency department visits and re-hospitalizations in patients who have undergone bariatric surgery. In the short term, patients may experience non-specific (pulmonary embolism, rhabdomyolysis) and specific (hemorrhage, fistula) complicat

Sexual Health in Bariatric Surgery: A Bibliometric Study.

BACKGROUND: Bariatric surgery not only treats obesity-related diseases but also positively impacts sexual health. However, research in this field is limited. Analyzing the overall status of the literature is essential to guide researchers. This study aims to bibliometrically examine publications on bariatric surgery and sexual health. METHODS: The study was conducted using publications on bariatric surgery and sexual health in the WoS database (N = 217). The Web of Science database was searched on June 19, 2025, using a topic, which includes the title, abstract, author keywords, and keyword p
