SIGNIFICANCE: The molecular mechanisms driving the progression from nonalcoholic fatty liver (NAFL) to fibrosing steatohepatitis (NASH) are insufficiently understood. Techniques enabling the characterization of different lipid species with both chemical and spatial information can provide valuable insights into their contributions to the disease progression.

Amyloid deposition in two sublines of the A strain mouse, a reputedly reliable murine model of spontaneous amyloidosis, was reevaluated using combined green polarization birefringence after Congo red staining and a fibrillar ultrastructure for amyloid identification. Both sublines were found to be devoid of amyloid at an age when the A strain mouse had been reported to have a high incidence of papillonephritis, amyloidosis, and death. This finding indicates a need for reassessment of this and other traditional models of spontaneous amyloidosis.

We report the case of a 36-year-old man with a recent diagnosis of gout, who presents with a fullness behind his right knee that fluctuates in size with time. An ultrasound revealed a Baker cyst that contained both large and punctate internal hyperechoic foci. Cyst aspiration revealed negatively birefringent crystals, which were consistent with gout.

Tissue sections from injection marks from 30 drug addicts and sections from endocrine organs and kidneys from an additional 33 addicts were studied together with endocrine organs and kidneys from 20 'normal' persons. All 83 persons were submitted for medico-legal autopsy at the Institute of Forensic Medicine in Copenhagen. A 65 year old man who underwent coronary artery bypass graft (CABG) for triple vessel disease was found to have enlarged thymus. Biopsy of the thymic mass revealed localised amyloid deposits demonstrating strong apple green birefringence on polarised microscopy. Localized thymic amyloidosis is an extremely rare finding with present case as the seventh in the world and the first in India. Review of literature of thymic amyloidoma and its close differential sclerosing thymoma is hereby described.

A patient referred to us for recurrent chest wall gouty tophus, but who was determined to actually have a chylous cyst, is described herein. Chylous cysts of the neck or chest wall can be caused by thoracic duct injury. Chyle contains 4-40 gm/liter of lipids, mostly triglycerides, and these can form birefringent crystals upon drying, leading to a false diagnosis of gout.

We have recently observed a strongly birefringent material of varying shapes and sizes in a benign breast cyst fluid specimen from a 52-yr-old woman with suspicious mammographic microcalcifications. The finding of calcium oxalate crystals in the breast cyst fluid should be regarded as a recognition of a particular type of calcification, easily overlooked with the conventional light microscopy. To date, we are unaware of any cytologic reports of this type of calcification in the breast.
