BACKGROUND: Specific noninvasive signal processing was applied to identify drivers in distinct categories of persistent atrial fibrillation (AF).
METHODS AND RESULTS: In 103 consecutive patients with persistent AF, accurate biatrial geometry relative to an array of 252 body surface electrodes was obtained from a noncontrast computed tomography scan. The reconstructed unipolar AF electrograms acquired at bedside from multiple windows (duration, 9±1 s) were signal processed to identify the drivers (focal or reentrant activity) and their cumulative density map. The driver domains were catheter a

INTRODUCTION: Descriptions for left atrial macroreentry tachycardia (LAMRT) in patients without obvious structural heart disease or previous surgery or catheter radiofrequency (RF) ablation have been sparse.
METHODS AND RESULTS: Ten of 226 patients (7 women, mean age 57 ± 14) with LAMRT underwent electroanatomic mapping and catheter ablation. None of the 10 patients had structural heart disease or history of previous surgery or catheter ablation. In all patients, the reentry circuits were located within a large low-voltage (bipolar voltage ≤ 0.5 mV) area in left atrium (LA), which contained 2

UNLABELLED: Characterization of the Critical Isthmus in VT in TOF.
INTRODUCTION: The complexity of postoperative ventricular reentrant tachycardias may limit success of catheter ablation. The objective of this analysis was to compare the usefulness of dynamic substrate mapping (DSM) versus color-coded isopotential mapping of the noncontact mapping system for the identification of the critical diastolic pathway of postoperative ventricular reentrant tachycardias (VT) after surgical repair of tetralogy of Fallot (TOF).
METHODS: Postoperative VT had been studied applying isopotential maps with

A 76-year-old man with a history of atrial septal defect repair underwent radiofrequency (RF) ablation of typical atrial flutter. During electrophysiological study, incessant sharp potentials were recorded, originating from the ostium of the inferior vena cava (IVC), and dissociated from atrial activity. During sinus rhythm, these potentials propagated to the atria and caused premature complexes when falling beyond the atrial refractory period. Electro-anatomical mapping revealed the presence of the earliest potential in the postero-lateral ostium of the IVC, propagating to the septal region.

BACKGROUND: Reentry utilizing cavotricuspid isthmus (CTI) is accepted as the mechanism underlying typical atrial flutter (AFLT). However, it is unclear how the right atrial (RA) posterior wall (PW) participates in AFLT circuit. We sought to investigate this by using noncontact electroanatomic mapping.
METHODS: Fifteen patients (pts) undergoing ablation for typical AFLT participated. Multipolar catheters were deployed in RA and coronary sinus. RA shell was created during AFLT. Entrainment was performed to confirm CTI dependence and assess participation of various RA regions (septum-Sep, PW, la

Multiple imaging modalities are required in patients receiving cardiac resynchronization therapy. We have developed a strategy to integrate echocardiographic and angiographic information to facilitate left ventricle (LV) lead position. Full three-dimensional LV-volumes (3DLVV) and dyssynchrony maps were acquired before and after resynchronization. At the time of device implantation, 3D-rotational coronary venous angiography was performed. 3D-models of the veins were then integrated with the pre- and post-3DLVV. In the case displayed, prior to implantation, the lateral wall was delayed compared

INTRODUCTION: Conduction disturbances and arrhythmias characterize the cardiac feature of myotonic dystrophy type 1 (MD1); a myocardial involvement has been suggested as part of the cardiac disease. The aim of our study was to investigate the underlying myocardial alterations using electroanatomic mapping (CARTO) and their possible correlation with genetic and neurological findings.
METHODS AND RESULTS: Right atrial and ventricular CARTO maps were obtained in 13 MD1 patients. Thirteen age-matched patients with paroxysmal supraventricular tachycardia and normal heart served as controls. Unipol

Typical atrial flutter with reentry around the tricuspid valve can easily be cured by ablating the cavotricuspid isthmus. In the reported case, transvenous access to the tricuspid valve was not possible because of a total cavopulmonal connection in congenital mitral atresia. Successful angioplasty of a small fenestration between the total cavopulmonal connection and the genuine right atrium (RA) allowed anterograde transvenous access to the RA. Electroanatomic RA mapping confirmed peritricuspid reentry, and successful ablation was performed.

Catheter ablation of the posterior isthmus is an effective tool to cure typical atrial flutter. In some cases, however, bidirectional block cannot be obtained despite extensive RF applications. Anatomic obstacles or abnormalities are thought to be the most common reasons for failed or prolonged procedures. We present a case of recurrent typical atrial flutter that seemed to be refractory to all ablation attempts in the region of the posterior isthmus although no anatomic abnormalities could be detected. Despite extensive RF application, bidirectional conduction was unchanged. Using a novel non

INTRODUCTION: The genesis of repolarization abnormalities of ECG waveforms in atrial septal defect (ASD), which typically is characterized by right ventricular (RV) volume overload, has not been explored, particularly in association with postoperative hemodynamic improvement. The aim of this study was to evaluate the effects of reduced RV overload after ASD closure on depolarization and repolarization abnormalities on body surface maps (BSMs).
METHODS AND RESULTS: BSMs of 14 children with ASD were recorded preoperatively and at early postoperative (1-6 months) and late postoperative (>9 month

Voltage-based methods currently form the diagnostic and therapeutic basis of clinical cardiac electrophysiology. These include both noncontact-based approaches (e.g., body surface electrocardiogram) and established, contact-based 3D mapping systems. At the physiological level, voltage is the measure of an electric field that includes both the local and the sum of the surrounding activation sources. As a result, details of complex rhythm patterns like those observed in atrial fibrillation may be obscured. Dipole density mapping represents an alternative approach to detect and visualize the elec

Sudden cardiac death (SCD) is a challenging health problem in the western world. Analysis of cardiac repolarization from the electrocardiogram (ECG) provides valuable information for stratifying patients according to their risk of suffering from arrhythmic events that could end in SCD, as well as for assessing efficacy of antiarrhythmic therapies. In this paper, we start by exploring the cellular basis of ECG repolarization waveforms under both normal and pathological conditions. We then describe basic preprocessing steps that need to be accomplished on the ECG signal before extracting repolar

BACKGROUND: Atrial fibrillation (AF) ablation guided by complex fractionated electrograms (CFE) has been described, but the spatial and temporal stability of the electrograms (EGMs) has been questioned.
OBJECTIVE: The purpose of this study was to prospectively assess the spatial and temporal stability of CFE in patients with persistent AF.
METHODS: Twenty-four patients were studied. For 12 patients, two high-density CFE maps were performed during AF at baseline (0 minute) and 20 minutes later using the EnSite NavX system. Six-second bipolar EGMs were collected throughout the left atrium (LA)

BACKGROUND: Catheter ablation in pediatric patients remains technically challenging. The RHYTHMIA high-density mapping system is increasingly used in pediatric electrophysiology; however, procedural experience in patients weighing less than 20 kg is limited.
METHODS: We retrospectively analyzed 125 pediatric patients who underwent catheter ablation for supraventricular arrhythmias using the RHYTHMIA system. The patients were divided into two groups according to body weight: ≥20 kg group (n = 111) and the <20 kg group (n = 14). The mapping time, number of mapping beats, and mapping electrodes

BACKGROUND: Accurate identification of the earliest activation site is essential for successful catheter ablation of premature ventricular complexes (PVCs).
OBJECTIVE: This study aimed to compare first deflection (FD) and near-field (NF) detection using the Turbomap modality on the Ensite X system.
METHODS: A retrospective analysis was conducted on 59 patients (71 ablation sites) who underwent first-time PVC ablation. All cases were remapped using Turbomap to extract FD and NF parameters at the earliest activation sites. Comparisons were based on differences in local activation time (ΔLAT),

Recent advances in technology and clinical electrophysiology have led to precise characterization of re-entry circuits and effective ablation strategies of atypical atrial flutter. Combination of activation and entrainment mapping is the key to identifying the re-entry circuit. The presence of a slow-conducting isthmus, localized re-entry, dual-loop re-entry or bystander loops may lead to misleading activation maps but can be identified by electrogram analysis and entrainment mapping. The ECG pattern of atypical atrial flutter is poorly predictive of circuit anatomy but may still provide mecha

BACKGROUND: Body surface microvolt QRS alternans has been shown to predict ventricular tachyarrhythmias in patients with cardiomyopathy; however, the mechanism is unclear.
OBJECTIVES: The aim of this study was to determine the spatiotemporal relationship of beat-to-beat bipolar electrogram (EGM) activation alternans (AA) to the ventricular tachycardia (VT) circuit and to surface QRS alternans.
METHODS: Intraoperative mapping during ventricular pacing and induced VT was performed in 9 patients with ischemic cardiomyopathy using a 112-bipole endocardial array. AA was quantified at EGM sites du

BACKGROUND: Endocardial electrogram (EGM) characteristics in nonischemic cardiomyopathy (NICM) have not been explored adequately for prognostication.
OBJECTIVE: We aimed to study correlation of bipolar and unipolar EGM characteristics with left ventricular ejection fraction (LVEF) and ventricular tachycardia (VT) in NICM.
METHODS: Electroanatomic mapping of the left ventricle was performed. EGM characteristics were correlated with LVEF. Differences between groups with and without VT and predictors of VT were studied.
RESULTS: In 43 patients, unipolar EGM variables had better correlation wit

Background Dispersion and gradients in repolarization have been associated with life-threatening arrhythmias, but are difficult to quantify precisely from surface electrocardiography. The objective of this study was to evaluate electrocardiographic imaging (ECGI) to noninvasively detect repolarization-based abnormalities. Methods and Results Ex vivo data were obtained from Langendorff-perfused pig hearts (n=8) and a human donor heart. Unipolar electrograms were recorded simultaneously during sinus rhythm from an epicardial sock and the torso-shaped tank within which the heart was suspended. Re
